Chen, Left. In summer, Yang Qi floats outward while Yin Qi remains inward. The patient has a preference for cold and raw foods, causing the Cou Li (skin pores) to open, making them susceptible to external pathogens. Suddenly contracting the abnormal Qi of epidemic pestilence, it enters directly into the Middle Jiao via the mouth and nose. This results in Cold, Summerheat, Dampness, and Food Stagnation mutually obstructing the Middle Jiao. Clear and Turbid energies become confused and chaotic in the intestines and stomach. The Stomach loses its descending harmony, and the Spleen lacks ascending transport, leading to severe vomiting and diarrhea, characterized by sudden chaos and turmoil. Yang pathogens are confined internally, preventing Middle Yang from extending; thus, it cannot pulse the vessels, resulting in a hidden pulse (Mai Fu). It cannot reach the four limbs, causing cold limbs and calf cramps. One cause is Cold causing contraction; the other is Liver Wood attacking deficient Spleen Earth. Profuse sweating with irritability, a desire to sit in a well, thirst without desire to drink—this indicates excessive Yin below repelling Yang upward, known as Yin restlessness (Yin Zao). The body and flesh have suddenly wasted away, indicating severe damage to Spleen Earth, inability to intake grain Qi, exhaustion of the source of transformation and production, no hope for Yin pathogens to dissipate, and danger of Yang Qi detachment. Combining pulse and symptoms, the condition is critical, hanging by a thread! A prescription is proposed based on the intent of Bai Tong Tang combined with Si Ni Tang, adding human urine and pig bile, to urgently rescue the dispersing Yang, drive out the prevailing internal Yin, and make a last stand, hoping for efficacy.
Raw and Prepared Aconite Root (Fuzi) (3 qian each) Lightly dried Ginger (Ganjiang) (5 qian) Honey-fried Licorice Root (Zhigancao) (1 qian) Ginger-processed Pinellia Tuber (Jiang Banxia) (3 qian) Evodia Fruit (Wuyu) (7 fen) Coptis Rhizome (Chuanlian) (3 fen) Red Poria (Chiling) (4 qian) Tangerine Peel (Chenpi) (1 qian) Old Quince (Chemu Gua) (5 qian) Child's Urine (Tongbian) (taken with decoction, 1 cup) Pig Bile (Zhu Danzhi) (taken with decoction, 3-4 drops)
Second visit: Vomiting, diarrhea, and irritability have decreased, but the hidden pulse and cold limbs persist. Add Stir-fried Codonopsis Root (Ludangshen) (4 qian).
Luo, Left. Contracted the abnormal Qi of cold epidemic, complicated by Dampness and stagnation obstructing together. The Taiyin and Yangming meridians are affected. Clear and Turbid energies interfere with each other, ascending and descending functions fail. Sudden alternating vomiting and diarrhea occur, with hidden pulse and cold limbs, sunken eyes, wasted flesh, and profuse sweating like rain. The Spleen governs the four limbs; when Turbid Yin occupies the Central Region (Middle Jiao), Yin Qi cannot penetrate, hence the hidden pulse and cold limbs. When Yang Qi escapes outward, spontaneous sweating occurs; when Righteous Qi is severely deficient, eyes sink and flesh wastes. The tongue coating is white and greasy, indicating deficiency mixed with excess—a severe case of Yin Huoluan (Cold Cholera). Urgently prescribe Bai Tong Tang combined with Si Ni Tang and Fuzi Lizhong Tang with modifications, hoping for a turning point.
Prepared Aconite Root (Fuzi) (2 qian) Lightly dried Ginger (Ganjiang) (1 qian) Clear honey-fried Licorice Root (Zhigancao) (8 fen) Ginger-processed Pinellia Tuber (Jiang Banxia) (3 qian) Evodia Fruit (Wuyu) (7 fen) Child's Urine (Tongbian) (taken with decoction, one wine glass) Stir-fried Codonopsis Root (Ludangshen) (3 qian) Raw Atractylodes Macrocephala Rhizome (Baizhu) (2 qian) Red Poria (Chiling) (4 qian) Processed Magnolia Bark (Zhi Chuanpu) (1 qian) Coptis Rhizome (Chuanlian) (3 fen) Pig Bile (Zhu Danzhi) (taken with decoction, 3-4 drops) Hearth Soil (Zaoxin Huangtu) (1 liang) Decoct with Yin-Yang Water.
Zhu, Right. The pathogen of epidemic pestilence enters directly into the Middle Jiao via the mouth and nose, mutually obstructing with latent Summerheat, Dampness, and food stagnation. Both the Spleen and Stomach are diseased. Sudden colicky abdominal pain, irritability and distress, vomiting above, and diarrhea below occur. Limbs are cold, mouth is dry with desire to drink, pulse is hidden, and the tongue coating is thin, greasy, and yellow. Clear Qi stays below, Turbid Qi stays above; Yin and Yang are discordant, Qi is chaotic in the middle, causing upper vomiting and lower diarrhea. Dampness suppresses Heat, Qi mechanism is blocked, causing cold limbs and hidden pulse. Deep heat leads to deep cold reversal (Re Shen Jue Shen), a severe case of Huoluan. Urgently use Yu Lian Jie Du Tang with modifications, using acrid-open and bitter-descend methods, and aromatic transformation of turbidity, hoping to save one-tenth.
Premium Coptis Rhizome (Chuanlian) (8 fen) Light Evodia Fruit (Wuyu) (2 fen) Immortal Pinellia Tuber (Xian Banxia) (2 qian) Charred Immature Bitter Orange (Zhishi Tan) (1 qian) Scutellaria Root (Huangqin) (1.5 qian) Agastache Stem (Huoxiang Geng) (1.5 qian) Medicated Leaven (Liushen Qu) (3 qian) Red Poria and Polyporus (Chiling, Zhuling) (3 qian each) Stir-fried White Peony Root (Baishao) (1.5 qian) Yushu Dan (Grind and take with decoction, 4 fen) Decoct with Yin-Yang Water.
Second visit: Yesterday, Yu Lian Jie Du Tang was administered; vomiting and diarrhea gradually decreased, pulse began to rise, and limbs became slightly warm—a good omen. However, there is still irritability and dry retching, thirst preferring cold drinks. The anterior half of the tongue is red and crimson, the posterior half has a thin yellow coating, and urination is short and red. Vomiting injured the Stomach, diarrhea injured the Spleen. Spleen Yang and Stomach Yin are damaged, Wood Fire rushes upward, and latent Summerheat-Dampness lingers unresolved. Adhering to the original intent, add herbs that clear Summerheat and promote dampness. If no other complications arise, safety can be hoped for.
Premium Coptis Rhizome (Chuanlian) (8 fen) Light Evodia Fruit (Wuyu) (1 fen) Immortal Pinellia Tuber (Xian Banxia) (1.5 qian) Charred Immature Bitter Orange (Zhishi Tan) (8 fen) Scutellaria Root (Huangqin) (1.5 qian) Stir-fried White Peony Root (Baishao) (1.5 qian) Stir-fried Bamboo Shavings (Zhuru) (1.5 qian) Loquat Leaf (Pipaye) (4 leaves) Persimmon Calyx (Shidi) (5 pieces) Red Poria (Chiling) (3 qian) Fresh Reed Rhizome (Lugen) (remove nodes, 1 chi) Taro Stem (Tongcao) (8 fen) Sheren Dan (Take with decoction, 4 fen)
Third visit: Vomiting and diarrhea have stopped, pulse has risen, limbs are warm, and irritability/dry retching have decreased. Only fever, thirst, preference for cold drinks, short red urine, red tongue with yellow coating remain. Yin fluids are injured; latent Summerheat-Dampness steeps in the Membrane Source (Moyuan), and the Sanjiao's diffusion and transformation functions are impaired. Again propose generating fluids and clearing Summerheat, bitter-cold draining heat, and bland leaching dampness.
Trichosanthes Root (Tianhuafen) (3 qian) Immortal Pinellia Tuber (Xian Banxia) (1.5 qian) Honeysuckle Flower (Jinyinhua) (3 qian) Liu Yi San (Wrap-decoct, 3 qian) Red Poria (Chiling) (3 qian) Fresh Dendrobium Stem (Shihu) (3 qian) Ya Coptis (Chuanlian) (5 fen) Forsythia Fruit (Lianqiao) (3 qian) Taro Stem (Tongcao) (8 fen) Bamboo Shavings (Zhuru) (1.5 qian) Fresh Reed Rhizome (Lugen) (remove nodes, 1 chi) Loquat Leaf (Pipaye) (remove hairs, wrap-decoct, 4 sheets)
You, Left. Cold, Summerheat, Dampness, and stagnation mutually obstruct. Taiyin and Yangming meridians are affected. Yin and Yang reverse and confuse, clear and turbid mix. Sudden alternating vomiting and diarrhea, colicky abdominal pain, irritability and distress, pulse deep and seemingly hidden. This is Huoluan; do not underestimate it. Urgently propose aromatic transformation of turbidity and separating Yin and Yang.
Agastache Stem and Perilla Stem (Huoxiang Geng, Zisu Geng) (1.5 qian each) Charred Immature Bitter Orange (Zhishi Tan) (1 qian) Tangerine Peel (Chenpi) (1 qian) Ginger Coptis (Jiang Chuanlian) (5 fen) Areca Peel (Dafupi) (2 qian) Ginger Pinellia (Jiang Banxia) (2 qian) Processed Magnolia Bark (Zhi Chuanpu) (1 qian) White Cardamom Seed (Baokou Ren) (8 fen) Light Evodia Fruit (Wuyu) (2 fen) Medicated Leaven (Liushen Qu) (3 qian) Stir-fried Plantago Seed (Cheqianzi) (3 qian) Fresh Ginger (3 slices) Red Poria and Polyporus (Chiling, Zhuling) (3 qian each) Yushu Dan (Take with decoction, 4 fen)
Second visit: Yesterday, Zhengqi San combined with Zuojin Wan method was taken; vomiting and diarrhea gradually stopped, abdominal pain decreased. Pulse turned soggy and rapid. Fever appeared, mouth is dry with little desire to drink, tongue coating is gray-greasy and yellow. Latent pathogens show signs of exterior emergence, interior disease shows signs of transforming to exterior—all are favorable states. Still adhere to the original intent, add exterior-releasing herbs so latent pathogens disperse through sweat.
Light Fermented Soybean (Douchi) (2 qian) Tender Peucedanum Root (Qianhu) (1.5 qian) Perilla and Agastache Stems (Suhuo Geng) (1.5 qian each) Immortal Pinellia Tuber (Xian Banxia) (2 qian) Areca Peel (Dafupi) (2 qian) Mint Leaf (Bohe Ye) (8 fen) Processed Magnolia Bark (Zhi Chuanpu) (1 qian) Tangerine Peel (Chenpi) (1 qian) Stir-fried Aurantium (Zhike) (1 qian) Medicated Leaven (Liushen Qu) (3 qian) White Cardamom Husk (Baokou Qiao) (1 qian) Ginger Bamboo Shavings (Jiang Zhuru) (1 qian) Lotus Leaf (Heye) (1 corner)
Third visit: Illness started with vomiting and diarrhea, affecting Taiyin and Yangming. Now vomiting and diarrhea have stopped, but interior heat causes thirst, irritability, and insomnia. Tongue is rough and black, pulse is fine and rapid. Spleen and Stomach Yin are injured, Heart and Liver Fire blaze internally. Should nourish Yin, rescue fluids, and clear latent heat.
Fresh Dendrobium Stem (Shihu) (3 qian) Forsythia Husk (Lianqiao Qiao) (3 qian) Mulberry Leaf (Sangye) (3 qian) Cinnabar-treated Poria (Zhufushen) (3 qian) Fine Rehmannia Root (Shengdihuang) (3 qian) Black Gardenia (Heishanzhi) (1.5 qian) Moutan Bark (Fudanpi) (2 qian) Trichosanthes Root (Tianhuafen) (3 qian) Raw Licorice Root (Shenggancao) (6 fen) Fresh Reed Rhizome (Lugen) (remove nodes, 1 chi)
Li, Left. Summerheat and Dampness mixed with stagnation obstruct the Middle Jiao. Taiyin and Yangming meridians are affected. Alternating vomiting and diarrhea, colicky abdominal pain, deep pulse, cold limbs, gray-greasy tongue coating slightly yellow. This is contracting epidemic Qi entering the Middle Jiao via mouth and nose, confusing clear and turbid, failing ascending and descending. Pathogen entering Stomach causes vomiting; entering Spleen causes diarrhea. Dampness suppresses Heat, Qi pathway is blocked; Qi blockage prevents reaching collaterals, hence cold limbs. Shanghan Lun says: Vomiting and diarrhea constitute Huoluan. This is a severe case. Urgently propose aromatic transformation of turbidity and separating Yin and Yang.

Ju, Left. Epidemic pathogen, carrying Summerheat-Damp-Stagnation, mutually obstructs. Taiyin and Yangming meridians are affected. Colicky abdominal pain, irritability, vomiting above, diarrhea below, cold limbs, dry mouth with desire to drink, fine pulse nearly hidden, thin greasy yellow tongue coating. Clear Qi in Yin, Turbid Qi in Yang, Yin and Yang reversed, Qi chaotic in middle, causing this change. Dampness suppresses Heat, Qi mechanism blocked, hence cold limbs and unfavorable pulse. Severe Huoluan. Urgently propose Yu Lian Jie Du Tang with additions, acrid-open and bitter-descend, aromatic transformation of turbidity.

Zhao, Right. Abnormal Qi of cold epidemic, carrying Damp-Stagnation, mutually obstructs. Taiyin and Yangming meridians are affected. Clear and Turbid interfere, ascending and descending fail. Sudden alternating vomiting and diarrhea, hidden pulse, cold limbs. Sunken eyes, wasted flesh, sweating like ice. Spleen governs four limbs; Turbid Yin occupies Central Region. Yin Qi cannot penetrate, hence cold limbs and hidden pulse. No period for Yin to dissipate, signs of Yang scattering. Severe Yin Huoluan, critical! Tentatively propose Tongmai Si Ni Tang with additions, driving out prevailing internal Yin, restoring scattered external Yang, wondering if recovery is possible?
