The patient has heat in the stomach accompanied by restlessness, with phlegm and saliva accumulating. Food is vomited immediately after ingestion; this condition is called Heat Vomiting (Re Ou). It may be caused by stomach heat combined with summerheat, or by latent heat from Taiyang disease that has not resolved, as well as conditions like damp-jaundice; all are due to heat pathogens.
Xiao Chai Hu Tang (Minor Bupleurum Decoction) treats Heat Vomiting. (Formula see in the Treatise on Cold Damage section)
The treatment principle states: If a patient frequently undergoes sweating therapy, leading to weakened Yang qi, deficiency of Qi in the diaphragm, and a rapid pulse, the rapidity indicates an exogenous heat pathogen. This heat cannot digest food, resulting in a cold and deficient stomach, causing vomiting. Such cases should be treated as Cold Vomiting (Han Ou), and the above formula for Heat Vomiting should not be used.
A patient who was previously robust but is now emaciated, with gurgling sounds in the intestines, vomiting immediately after eating, expelling both food and drink, is diagnosed with Phlegm Vomiting (Tan Ou). This may be caused by Qi stagnation leading to phlegm accumulation at the gastric opening, or by excessive alcohol, sweet foods, or cold drinks causing fluid retention.
To treat impaired Heart Qi circulation, where Qi stagnation generates phlegm-fluids that accumulate without dispersing, causing fullness and hardness below the heart, gurgling sounds in the intestines, and vomiting after eating:
Ban Xia (Pinelliae Rhizoma, 2 liang, washed ten times with boiling water before use)
Ren Shen (Ginseng Radix et Rhizoma, 3 qian 3 fen, sliced)
Divide into four doses. For each dose, use three zhan of water and two qian of honey. Mix the water and honey thoroughly, add the herbs, and decoct until one-sixth remains. Strain and take warm. Another method includes adding seven slices of fresh ginger. The treatment principle states: Patients who vomit usually feel thirsty first; if they do not feel thirsty now, it is because there is Zhi Yin (branching fluid retention) below the heart. This condition belongs to Zhi Yin.
Fu Ling Ze Xie Tang (Poria and Alisma Decoction) treats the same as above. (Formula see in the Cholera section)
The patient experiences chest and abdominal distension and oppression, cold limbs, aversion to the smell of food, and vomiting immediately after eating. Food eaten in the morning is vomited in the evening, and food eaten in the evening is vomited in the morning; this condition is called Food Vomiting (Shi Ou). This is caused by dietary injury to the Spleen, leading to the inability to digest retained grain.
To treat dietary injury to the Spleen, undigested retained grain, morning food vomited in the evening, evening food vomited in the morning, upper Jiao Qi rebellion with heat, cold bi syndrome in the limbs, dysfunction of the San Jiao (Triple Burner); as well as deficiency-cold of the Stomach located in the upper part, and Qi depression from worry located in the lower part, where the two Qi compete, only exiting without entering, causing vomiting and inability to eat:
Hou Po (Magnoliae Officinalis Cortex, bark removed)
Sheng Jiang (Zingiberis Rhizoma Recens, 2 liang each)
Fei Zao (Jujubae Fructus, 3 liang, chopped, stir-fried together with the above three herbs)
Bai Zhu (Atractylodis Macrocephalae Rhizoma)
Shan Yao (Dioscoreae Rhizoma, stir-fried)
Ren Shen (Ginseng Radix et Rhizoma)
Chuan Xiong (Ligustici Chuanxiong Rhizoma)
Ju Pi (Citri Reticulatae Pericarpium)
Dang Gui (Angelicae Sinensis Radix)
Wu Wei Zi (Schisandrae Chinensis Fructus)
Huo Xiang (Agastaches Herba)
Gan Cao (Glycyrrhizae Radix et Rhizoma, honey-fried)
Pi Pa Ye (Loquats Leaf, brushed clean of hairs, processed with ginger juice)
Huang Qi (Astragali Radix, 1 liang each)
Grind all ingredients into a coarse powder. Take 4 qian per dose with one and a half zhan of water, three slices of ginger, and one jujube. Decoct until seven-tenths remain, strain, and take on an empty stomach. Alternatively, grind into a fine powder and mix with rice gruel for administration.
