Once the measles rash has been promoted to the exterior and red shadows appear on the skin, one must strictly avoid wind-cold and raw/cold foods. If these are violated, the Cou Li (skin pores and interstices) will become closed and tight, causing toxic factors to stagnate. This leads to a transformation into a whole-body cyanosis, with toxic factors attacking inward, resulting in symptoms such as restlessness, abdominal pain, asthma, chest oppression, and mental confusion. The rash desires to emerge but cannot, leading to imminent danger and death; both physicians and patients must be extremely cautious.
The treatment of measles relies entirely on proper care and dietary restrictions. One must avoid chicken, fish, roasted foods, salt, vinegar, and the "Five Pungent Spices" (Wu Xin, pungent spices like garlic and scallions) until after forty-nine days have passed. Diet should be light and bland. Indulging in food can cause other diseases. If chicken or fish is mistakenly consumed, it may result in lifelong skin eruptions resembling gooseflesh or fish scales, or trigger a recurrence of the rash during epidemic periods. If pork or mutton is mistakenly consumed, diarrhea, Sha syndrome, and malignant sores often occur during months when measles typically appear. If salt and vinegar are mistakenly consumed, coughing ensues, leading to increased cough cases during those months. If the Five Pungent Spices are mistakenly consumed, frequent episodes of fright-heat, red eyes, and foul breath may arise. These are all matters that families dealing with smallpox and measles must heed.
Smallpox and measles do not erupt without heat. When measles are about to erupt, the body invariably becomes feverish, accompanied by restlessness, dizziness, or muscle stiffness. Once the rash emerges, the body cools down, and all symptoms resolve; this indicates a very mild case where the rash recedes quickly. If the fever remains high even after the rash emerges, it indicates excessive toxicity, and Da Qing Tang (Great Blue Decoction) should be used to resolve the toxin. For scanty and painful urination, Huang Lian Jie Du Tang (Coptis Decoction for Resolving Toxicity) combined with Bai Hu Tang (White Tiger Decoction), or Da Lian Qiao Yin (Large Forsythia Drink), should be used to clear interior heat. For constipation, Liang Ge San (Cool the Diaphragm Powder) from the *Ju Fang* (Formulas of the Bureau), augmented with Niu Bang Zi (Arctii Fructus), is indicated.
Measles cases often involve coughing, reflecting the sudden emergence and retreat pattern of the pathogen. However, since the measles toxin requires coughing to help disperse it, some degree of coughing within ten days after eruption is appropriate. Physicians must not treat the cough simply because it is frequent; caution is advised.
Measles pathology involves the Lung and Spleen-Stomach. When the Lung is affected by Fire Evil, coughing increases, causing the rash to suddenly appear on the face/head and spread to the four limbs. When the Large Intestine is affected by Fire Evil, it affects the Spleen and Stomach upward, leading to diarrhea. If diarrhea occurs too early, coughing decreases but transforms into asthma, as both asthma and cough belong to the Lung. Cough represents excess (solidity), while asthma represents deficiency. (Note: There are also cases of asthma due to lung distension caused by external closure of toxic factors.) Cough indicates the outward movement of toxins, while asthma indicates the inward sinking of toxins. Inward sinking presents with closed eyes, profuse phlegm, chest fullness, abdominal distension, pale complexion, and incomplete expulsion of toxins, which is a critical condition. Therefore, measles favor coughing over asthma, and most severely contraindicate diarrhea.
