In the first month of the Jichou year of Guangxu (1889), the principal qi and guest qi of Wind-Wood were identical. My student, Mr. Chen Xizhou, was thirteen years old at that time. He had previously suffered from Zheng Sha (True Sha disease) and, after recovery, observed a dietary restriction for one hundred days before consuming oily or meat foods. Before the Mangzhong solar term in May, he suddenly experienced difficulty and pain in swallowing, dizziness, dry retching, body heat feeling like fire, coughing, and mental restlessness. His pulse was floating, slippery, soft, and rapid, and his tongue coating was thick and white in the fissures. This was residual pathogenic factor following Sha disease. A decoction was prepared using Gancao (Licorice Root), Jiegeng (Platycodon Root), Gegen (Kudzu Root), Jingjie (Schizonepeta Spike), Niubangzi (Arctium Fruit), Chantui (Cicada Slough), Lianqiao (Forsythia Fruit), Xiang Beimu (Fritillaria Thunbergii Bulb), Zhike (Aurantii Fructus), Mutong (Akebia Stem), Zhuye (Bamboo Leaf), Zhu Dengxin (Zizaniae Spadix treated with Cinnabar), and Xihe Liu (Salix Matsudana Twig). Two pills of Yuxue Jiuku Dan (Jade Snow Rescue Poisonous Evil Pill) were taken with the decoction. Subsequently, Sha zhen (Sha rash) appeared all over the body and spread thoroughly, indicating the disease was about to resolve completely. Suddenly, the tendons of the scrotum retracted, hung down, and became swollen; the urethra felt astringent and painful, and urination was scanty and dripping. Immediately, Chaihu Siwu Tang (Bupleurum Four Substance Decoction) was used to clear the Liver and promote dampness drainage. The prescription included Chaihu (Bupleuri Radix), Fuxiong (Chuanxiong Rhizoma), Tiaoqin (Scutellariae Radix), Zhuye (Bamboo Leaf), and Zhu Dengxin (Zizaniae Spadix treated with Cinnabar), each 5 fen; Xianshengdi (Rehmanniae Radix Recens), Guishen (Angelicae Sinensis Radix), Chishaoyao (Paeoniae Radix Rubra), Lianqiao (Forsythia Fruit), Xiang Beimu (Fritillaria Thunbergii Bulb), Xiakucao (Prunellae Spica), Tianhuafen (Trichosanthis Radix), and Gongyingying (Taraxaci Herba), each 1 qian; Gancao (Licorice Root), Jiegeng (Platycodon Root), and Mutong (Akebia Stem), each 4 fen. After one dose and three servings, the illness was cured. (Recorded by Jialiu)
Another case: A six-year-old Tan patient suffered from Li Xie (Plague Pathogen), which is Wen Xie (Warm Pathogen) and Shi Li (Seasonal Plague). It entered through the mouth and nose, and turbid rebellious qi circulated through the Sanjiao (Triple Burner), resulting in Ma Zhen (Measles). Symptoms included red eyes, black soot-like nostrils, vomiting roundworms, diarrhea with roundworms, profuse sweating, panting, thirst, and desire to drink. The treatment should employ bitter, pungent, and cold herbs, following the method of Liu Hejian. Laypeople do not understand this; they merely attribute it to Fa Sha (emitting Sha) and use only Jingjie (Schizonepeta Spike), Fangfeng (Saposhnikoviae Radix), and Chanqiao (Cicada Slough) to raise and lift. Fire, aided by wind, burns fiercely and cannot be restrained, leading to fluid depletion and transformation of the disease. Liangge San (Cool the Chest Powder) was modified by removing Xiaoshi (Mirabilite) and Dahuang (Rhubarb Root), and adding Shigao (Gypsum Fibrosum), Niubangzi (Arctium Fruit), and Chishaoyao (Paeoniae Radix Rubra).
Lanhou Sha (Rotting Throat Sha) is not recorded in ancient texts; it originated in recent times and is highly contagious. Practitioners often claim it is due to wind-heat toxin in the Taiyin (Tai Yin Lung) and Yangming (Yangming Stomach) meridians. The cause of rotting must also be carefully examined. For example, just as an object is forced by intense fire, appearing dry while ignoring the steaming effect of damp-heat郁蒸 (stagnation and steaming), which causes rotting. For this condition, if it is wind-heat, treatment should focus on clearing and venting; if damp-heat, clearing and draining; if phlegm-fire congealing, clearing and descending. When pathogenic factors reach the exterior, Sha rash emerges; when Sha rash emerges, rotting stops naturally. If cold-cold herbs are overused, heat will sink inward, and the harm would be incalculable. There are treatable and untreatable conditions. Bad breath indicates Huiyang (Return of Yang); yellowish color, whether pale or deep, is caused by phlegm-fire and is treatable. Others, such as rotting extending to the uvula, nasal congestion, blurred vision upon closing eyes, and cases where original qi is deficient, pathogenic toxins are deeply hidden, and the color is white like parchment skin, are untreatable. In summary, these diseases are contracted due to abnormal qi of heaven and earth; thus, constitutions vary in deficiency and excess, and symptoms vary in severity. Other Sha and throat conditions have been detailed by ancients, so no further elaboration is needed.
