*Qianjin Fang* states: Hongjiaojiao (Hong Kong foot / gouty arthritis) is caused by contracting wind-toxin. Furthermore, medical classics state: The cold, heat, wind, and dampness of the earth all transform into vapors. Since the feet constantly tread upon them, Hongjiaojiao thus forms. However, although the explanations differ, the essence is the same. From this perspective, cold, heat, wind, and dampness can all cause this disease, not merely wind-toxin. The disease of Hongjiaojiao initially presents without noticeable symptoms; it may only manifest due to other illnesses, or suddenly cause severe oppression and confusion, becoming apparent only after two or three days. Symptoms start from the feet: they may present as slow weakness, pain, and numbness; sudden collapse while walking; swelling and fullness in both calves; withered and thin feet and knees; palpitations in the heart; insensitivity in the lower abdomen; generalized muscle spasms; vomiting and nausea upon seeing food, aversion to the smell of food; chest fullness and rapid breathing; or generalized body soreness and pain. These are the different manifestations of the condition. Generally speaking, if cold invades the Yang meridians, the illness will necessarily be cold in nature; if summer-heat invades the Yin meridians, the illness will necessarily be hot in nature. This statement is indeed true. Regarding pulse diagnosis: if the pulse is floating and wiry, the origin is wind; if soggy and weak, the origin is dampness; if surging and rapid, the origin is heat; if slow and choppy, the origin is cold. Wind-origin cases are cured by inducing sweating; damp-origin cases by warming; heat-origin cases by purging/downward draining; cold-origin cases by hot compresses/ironing. Whenever Hongjiaojiao is contracted, acupuncture and moxibustion should be employed promptly. Using only herbal decoctions for washing is a major prohibition for physicians. Observing the etiology of Hongjiaojiao, it arises from Kidney Qi deficiency. However, women also suffer from Hongjiaojiao, which must be due to Blood Sea (Chong and Ren vessels) deficiency. Both men and women should tonify according to the four seasons; otherwise, Qi and Blood will decline daily, inevitably causing recurrence every year during steaming-hot weather, which is logically inevitable. Treatment methods generally do not exceed these principles. Additionally, one must adjust and regulate lifestyle according to the four seasons. One must not sit or stand for long periods in cold and damp places, nor sit outdoors in damp areas during summer months. If one can be cautious in these aspects and treat according to patterns following these principles, recovery is assured.
Another discussion: Regarding the disease named Hongjiaojiao, ancient people used various names, but modern people call it Hongjiaojiao. When men contract this disease, the pathology resides in the Kidney; when women contract it, the pathology resides in the Blood Sea. Cold, heat, wind, dampness, anger, and sorrow can all cause this disease. The symptoms of Hongjiaojiao vary in severity (light/heavy), type (dry/wet), nature (Yin/Yang), and constitution (deficiency/excess). One should follow the four seasons, and the disease will rapidly flare up annually during steaming-hot weather. Furthermore, acupuncture and moxibustion are highly recommended. Using herbal decoctions for washing is a major prohibition for physicians. Refer to the subsequent formulas based on specific patterns for treatment, and recovery is assured.
