Medical classics state: Wind is the leader of a hundred diseases. From this perspective, Stroke (Zhong Feng) is superior to Cold Damage (Shang Han), being acute and sudden. Qibo's general principles mention four types: first, Pian Ku (Hemiplegia); second, Feng Fei (Flaccidity); third, Feng Yi (Stupor); fourth, Feng Bi (Bi Syndrome), referring to the most severe conditions. External wind syndromes of the five zang organs are all recorded in *Qian Jin Fang* (Essential Prescriptions Worth a Thousand Gold), so they are not repeated here. Generally, human life relies on Yuan Qi (Primal Qi) as the root and Ying Wei (Nutritive and Defensive Qi) as the foundation. If the root qi is strong and Ying Wei is harmonious, and the Cou Li (interstices/skin pores) are dense, how can external pathogenic factors cause harm? It may be due to joy or anger, sorrow or worry, fear or fright, irregular diet, or overexertion causing injury, leading to first deficiency of True Qi, loss of regulation of Ying Wei, sparse Cou Li, allowing pathogenic factors to enter through deficiency. Upon contraction, it manifests as hemiplegia, muscle pain, phlegm-saliva obstruction, deviation of mouth and eyes, unilateral paralysis and numbness, confusion of spirit and intellect; or stiff tongue unable to speak, stubborn bi-syndrome with no sensation, absent-mindedness, fright and terror, spontaneous sweating and aversion to wind, tendon vessel spasms, with numerous variable manifestations. The treatment method should trace its origin. If caused by internal Seven Emotions, the method should regulate Qi, not treat Wind; if caused by external Six Excesses, one should also first regulate Qi, then according to the contracted Six Qi, treat according to symptoms. This is a good method. However, if hair stands straight with foaming from the mouth, head shaking upwards, face red like makeup, or head and face blue-black, sweat dropping like pearls, eyes closed and mouth open, sound like snoring sleep, enuresis and unconsciousness, these are all untreatable.
Furthermore: Stroke is when wind qi invades the person. Suddenly struck by wind, spirit is confused like drunkenness, limbs cannot hold themselves, saliva surges upward, sound like sawing, jaw locked tight, decoctions cannot be swallowed, life hangs in the balance. But if eyes are closed, mouth dry, sound like snoring sleep, and enuresis occurs, these are all untreatable. At this moment, first use the Nasal Insufflation Method (Chu Bi Fa). Wait for sneezing, then administer Xi Yan San (Thin Saliva Powder). If breath returns and spirit clears, then apply treatment methods according to previous prescriptions. Previous sages' formulas and discussions do not include emetic methods for stroke, or some argue for them, but examination shows neither can be used for vomiting. Generally, it is a temporary closure of Qi flow, accumulation of phlegm-saliva, causing stupor. Phlegm-saliva is what nourishes joints; how can it be vomited out? Once phlegm-saliva is expelled, joints lose nourishment; although said to recover consciousness, many become hemiplegic. If Xi Yan San does not contain mercury powder, and the medicine does not contain Veratrum (Li Lu) or Thunberg Fritillary Bulb (Gua Di), the medicine not only does not damage the Spleen and Stomach, but its effect is particularly remarkable. Is this not beautiful?
