According to Traditional Chinese Medicine (TCM), there are three main causes for liver-related pathological changes in the human body:
1. External Causes – Climate
In the Heavenly Stems and Earthly Branches system, Jia (甲) and Yi (乙) correspond to the Liver and Gallbladder, which belong to Wood (Mu, 木); Wood corresponds to Wind (Feng, 风). In Five Element Theory, Wind represents the expansive function of Qi (Vital Energy). Wood can absorb nutrients and moisture from the soil; excessive absorption leads to soil deterioration. Wind movement dries the soil, causing it to lose nourishment and stagnation of Spleen Qi (Pi Qi, 脾气). This is referred to as "Wood overcoming Earth" (Mu Ke Tu). When treating liver diseases, one should first regulate the Spleen Earth; otherwise, Spleen Qi dysfunction will lead to abdominal distension. It is commonly said, "Why are you losing your temper?" Here, "temper" refers to Spleen Qi operation. When functioning normally, it manifests as borborygmus (intestinal rumbling) during hunger; when dysfunctional, it causes abdominal bloating. Common abdominal distension in patients with Hepatocellular Carcinoma (Liver Cancer) or Liver Cirrhosis is actually a disease of the Spleen, not merely the Liver. "Treating the Liver by regulating the Spleen" essentially means treating the Spleen rather than directly treating the Liver. Once Spleen Qi functions properly, flatulence occurs, abdominal distension subsides, and the disease is cured.
Wind is the expansive action of Qi. Changes occur due to the influence of Qi's contracting nature (Metal), ascending nature (Fire), descending nature (Water), and stabilizing nature (Earth). Therefore, during a common cold, improper diet or treatment may introduce Wind pathogen into the Liver, leading to damp-heat, wind-heat, wind-cold, or wind-dryness syndromes in the Liver. Hepatitis is classified into types A, B, C, D, E, X, etc. This is actually caused by pathogens such as wind-cold, summerheat, dampness, and heat from colds being introduced into the Liver via antibiotics in injections used for anti-inflammatory purposes, rather than being an intrinsic inflammation of the Liver itself. See "What is Hepatitis B?". The Liver and Gallbladder belong to Wood, govern Wind, and control the expansion of Qi. Wind is dynamic, so the Liver and Gallbladder are called the "Zang organs of Wind." Wind movement excites Fire, meaning Wind aids the ascending of Qi, known as "Wood generating Fire."
Hemiplegia, hemifacial paralysis, stroke, and common colds are all caused by the Liver and Gallbladder's inability to regulate Wind. Hence, it is said that "hundreds of diseases arise from Wind"; see the "Stroke Chapter."
2. Internal Causes – Emotions
People who live long have gentle words and deeds, calm hearts, smooth Liver Qi, and rarely get angry, thus enjoying longevity.
All forms of Qi stagnation and depression belong to the Liver. Anger is the emotion associated with the Liver. In Liver disease, shouting sounds urgent; patients tend to be irritable. When angry, the complexion turns cyan (greenish-blue), which is the color of the Liver. Thus, in Liver disease, the complexion turns cyan, whereas jaundice is not a Liver disease (see "Jaundice is not Yellow Gallbladder"). Anger causes acute expansion of Liver Qi, preventing timely air supply, leading to stagnant circulation of Liver Qi and a cyan complexion.
Why do women live longer than men?
The *Ling Shu* (Spiritual Pivot), Chapter 13 "Jing Jin" (Muscle Sinews), states: The sinews of the Foot Jueyin Meridian originate from the big toe, ascend to bind before the medial malleolus, follow the tibia, bind below the medial condyle of the femur, ascend along the medial thigh, and bind/connect at the genitalia, linking various sinews. The Liver Meridian of Foot Jueyin surrounds the genitalia. Women excrete toxins accumulated in the Liver through monthly menstruation. Since Liver toxins are expelled once a month, less toxin accumulation leads to smooth Qi flow. Smooth Liver Qi makes women gentler and longer-lived. Men lack menstruation, leading to blood accumulation in the Liver. Excessive blood accumulation in the Liver is the reason for men's greater physical strength and rougher temperament. Unexpelled toxins due to blood stasis make men prone to Liver diseases and shorter lifespans. The average female lifespan exceeds males due to menstrual influence on the Liver. Today, many women undergo hysterectomies, preventing toxin expulsion via menstruation, thereby reducing longevity.
3. Non-External/Internal Causes: Diet and Traumatic Blood Stasis
Sour flavor enters the Liver and has a收敛 (astringent/contracting) effect on Liver Qi. Seeing, smelling, or thinking of sour things causes salivation. Eating sour foods may suddenly improve vision, all due to the astringent effect of Sour flavor on Liver Qi.
The *Su Wen* (Basic Questions), Chapter 5 "Yin Yang Ying Xiang Da Lun" (Great Treatise on Yin-Yang Correspondences) states: Qibo replied: The East generates Wind, Wind generates Wood, Wood generates Sour, Sour generates the Liver, the Liver generates sinews, and sinews generate the Heart. The Liver opens into the eyes. Liver Qi connects to the eyes; when harmonious, the eyes distinguish five colors. Thus, the eyes are governed by the Liver.
Diabetes patients using insulin long-term suffer Liver toxicity, impairing Liver astringency, leading to visual impairment. See "Diabetes Chapter." Hepatitis B results from the accumulation of cold-phlegm/toxins from cold medicines and antibiotics in the Liver. Liver toxicity should not be treated with astringents; using Sour herbs like Schisandra (Wuweizi, 五味子) can cause fulminant hepatitis (see "Fulminant Hepatitis Chapter"). For the Liver, dispersing and draining is tonification; toxicity requires draining, not astringing.
Some Liver Cancer or hepatobiliary stone patients recover by eating Sour foods like pineapple or Schisandra. Kidney stone patients recover by eating sour plums because these stones are salty in nature; Sour balances Salt, inducing Liver astringency and therapeutic effects.
Liver cancer caused by Hepatitis B treated with excessive Western anti-inflammatories (TCM: excessive reduction of Fire) prevents Liver astringency. Pineapple's Sourness restores astringency, curing the condition. However, patients with acid regurgitation should avoid Sour foods like pineapple, as it worsens the condition (acid regurgitation indicates excessive Liver astringency). This applies when Hepatitis B was over-treated with anti-inflammatories, allowing recovery through Liver astringency.
Dryness Qi enters the Lung (Metal). Metal represents Qi contraction; Wood represents Qi expansion. Metal overcomes Wood. Eating dry foods makes Liver Qi dry and unable to expand, causing bile (the essence of condensed Liver Qi) to leak externally, resulting in bitter taste and dry mouth. Dry weather also causes these symptoms due to Dryness affecting the Liver.
The *Su Wen*, Chapter 32 "Ci Re Pian" (Chapter on Acupuncture for Heat Diseases) states: In Liver Heat Disease, urine turns yellow first, accompanied by abdominal pain, lethargy, fever, delirium,惊 (fright), hypochondriac fullness/pain, restlessness, and inability to lie down. Symptoms worsen on Geng/Xin (Metal) days and improve with sweating on Jia/Yi (Wood) days. If Qi rebels, death occurs on Geng/Xin days. Acupuncture targets the Foot Jueyin and Shaoyang meridians. Rebellion causes headache and pulsating vessels. Many Liver disease patients die on Geng/Xin days; those dying on other days likely had misdiagnosed Liver disease.
(II) How TCM Diagnoses and Treats Liver Diseases
Recovery requires correct diagnosis. Ignoring the Twelve Meridians leads to errors. Modern medicine often causes fatal outcomes in Liver disease due to misdiagnosis based on meridian pathology. Treatment must understand Liver meridian pathology, its interactions with the Five Zang organs (generation/restriction cycles), and Six Meridian transmission.
1. Diagnosis by Time:
The Liver belongs to Zheng Jueyin Wood, connected to Wind. Patients must avoid wind exposure. Liver disease begins in Spring (Wood). By Summer (Fire), Fire overcomes Metal, preventing Metal from overcoming Wood, so Liver disease should heal in Summer. If not healed in Summer, it worsens in Autumn (Metal overcomes Wood). If survival continues to Winter (Water, Mother of Wood), the Mother Qi supports the Child (Liver), mitigating severity. However, the disease recurs in Spring, as the Liver returns to its dominant season ("rising in its own position").
2. Diagnosis by Pulse:
The *Ling Shu*, Chapter 4 "Xie Qi Zang Fu Bing Xing" describes Liver pulses: Extreme tension indicates harsh speech; slight tension indicates "Fei Qi" (mass under ribs); extreme slackness indicates vomiting; slight slackness indicates water retention; extreme large indicates internal abscess; slightly large indicates Liver Bi; extreme small indicates polydipsia; slightly small indicates Xiao Dao (wasting-thirst); extreme slippery indicates hernia; slightly slippery indicates enuresis; extreme choppy indicates fluid overflow; slightly choppy indicates spasms.
Normal Liver pulse is soft and flexible like a bamboo pole swaying in the wind, rooted in Stomach Qi in Spring. Pathological Liver pulse is full, solid, and slippery like touching a long pole. Terminal Liver pulse is rapid, forceful, and tight like a newly strung bowstring.
3. Diagnosis by Meridian Symptoms:
*Ling Shu*, Chapter 13 "Jing Jin": Foot Jueyin Liver sinews originate at the big toe... Bind at genitals. Pathologies include pain at the big toe/medial malleolus/knee/thigh, spasms, genital dysfunction. Cold injury causes retraction; Heat injury causes flaccidity. Treatment involves clearing heat/dampness or burning needles for spasms.
*Ling Shu*, Chapter 10 "Jing Mai": Pathologies include lower back pain, hernias, lower abdominal swelling, dry throat, dull complexion. Main disorders: chest fullness, vomiting, diarrhea, hernias, urinary issues. Excess conditions require draining; deficiency requires tonifying; Cold requires retention of needle; Heat requires quick removal; Sunken conditions require moxibustion. Pulse at Cun Kou (Radial artery) larger than Ren Ying (Carotid) indicates excess; smaller indicates deficiency.
*Sù Wèn*, Chapter 22 "Zang Qi Fa Shi Lun": Liver disease presents with hypochondriac pain radiating to the lower abdomen, irritability (Excess). Deficiency presents with blurred vision, deafness, fear. "Liver suffers from urgency; eat Sweet to relax it." "Liver desires dispersion; eat Pungent to disperse it. Use Pungent to tonify, Sour to drain."
4. Diagnosis by Physiological Function:
The *Su Wen*, Chapter 10 "Wu Zang Sheng Cheng" states: The Liver governs sinews and manifests in nails. Blood returns to the Liver during sleep. The Liver provides blood for vision, walking, grasping, and holding. Wind exposure after sleep causes blood coagulation, leading to Bi (obstruction) or Jue (cold extremities).
Paralysis after spinal surgery is often misattributed to severed nerves. In TCM, anesthesia causes Liver toxicity, and anti-inflammatories damage the Kidney. With weak Kidneys, Liver toxins cannot be excreted, causing leg paralysis. Clearing Liver drug toxicity and tonifying the Kidney can restore function. Understanding "Liver governs sinews" explains this.
Diet, climate (wind/cold/summerheat/damp/dryness/fire), and blood stasis cause Liver pathology. Identifying damp-heat, deficiency, Qi stagnation, or stasis is key to cure.
Ignoring the five flavors, seven emotions, and external pathogens while focusing solely on Hepatitis B anti-inflammatories leads to cirrhosis and cancer. Men have more Liver disease due to blood accumulation without menstruation. Easterners (Yellow race, Earth element) are more prone to Wood-overcoming-Earth issues (Spleen/Stomach/Kidney diseases mimicking Liver disease), often exacerbated by antibiotic misuse. Westerners (White race, Metal element) are prone to Heart/Lung issues due to Fire-overcoming-Metal, often misdiagnosed as heart disease but stemming from lung/inflammatory issues.
Local climates and diets dictate disease patterns. Blindly adopting foreign medical statistics and drugs harms patients, creating artificial "civilization diseases" in mortality rankings.
(III) Hepatitis B – Are Viruses Less Toxic than Medical Authorities' Words?
1. Introduction:
Fire represents ascending Qi. The Heart belongs to Fire. The Liver belongs to Wood (expansive Qi). Wood generates Fire (Mother-Child relationship). Heart Fire consumes Liver Wood energy, potentially causing deficiency.
Chinese call it "Liver Fire"; Western medicine calls it "Hepatitis" (adding 'fire' to 'inflammation'). Calling it "Liver Fire" devalues it, as herbal teas can cure it. Calling it "Hepatitis" increases its value, driven by scientific instruments and patient willingness to sacrifice for science, leading to financial ruin and death, yet deemed "scientific."
2. Origin of Hepatitis B:
Food improves vitality; no food causes fatigue/sleepiness. Utensils do not cause fatigue. Disposable utensils create pollution.
Illness has causes. What causes the fatigue/sleepiness of Hepatitis B? Is it a virus? Experts found changes in liver enzymes (SGOT, SGPT) in labs, coining the term "Hepatitis B."
Antibiotics for colds damage all Zang-Fu organs. Hepatitis B is a product of this damage. Focusing only on the Liver ignores other organ damage. Treating Hepatitis B further damages Heart, Spleen, and Kidney, causing cirrhosis and cancer.
The country inventing Hepatitis B did not aggressively market it, yet our country promoted it vigorously, ranking cirrhosis/cancer high in mortality and promoting vaccines uniquely.
Colds are exterior syndromes (Biao Zheng) where the Five Zang fail to regulate atmospheric factors (Wind/Summerheat/Cold/Damp/Dryness/Fire). TCM classifies colds as Wind-Cold, Summerheat-Damp, etc., not all as "Fire inflammation."
Modern medicine treats colds without distinguishing Exterior/Interior or pathogenic factors, using antibiotics indiscriminately. This introduces pathogens into the Liver (and all organs), termed "Hepatitis," causing fatigue. This is the origin of Hepatitis B.
It is the medication in the needle, not the needle itself, causing Hepatitis B. Antibiotic toxicity triggers Liver responses to expel toxins via skin (itching) or Kidneys (urine).
Qi and Blood circulation vary by time and meridian. Body activity affects Zang-Fu and meridians.
The *Nei Jing* states: "The Liver stores Blood; the Spleen controls Blood." "Blood returns to the Liver during sleep." During sleep, the Spleen gathers blood to the Liver, which detoxifies it via bile decomposition, excreting toxins via Kidney urine. Liver dysfunction causes bitter taste, dry mouth, or dark urine upon waking.
Sleeping charges the Liver like a battery. Upon waking, Liver blood supplies the Heart, energizing the body. Depletion causes fatigue until recharging.
The *Ling Lan Mi Dian Lun* states: "The Liver is the General, planning resides therein." The Liver processes toxins, preventing them from entering the Heart ("Poison attacking the Heart"), ensuring clean blood circulates. Bile bitterness aids Liver detoxification.
Western drugs damage the Stomach and Liver, causing fatigue. During colds, unregulated anti-inflammatories enter the Liver via IV/antibiotics, damaging it. The Liver cannot store/charge blood, so the Heart lacks blood to generate vitality. Fatigue persists in Liver diseases like Hepatitis B because the Liver is blocked by antibiotic toxins and Wind pathogens (viruses), unable to recharge.
