Patient Wang. Asthma triggered by cold pathogen. Phlegm obstructs Qi mechanism, inability to lie flat. (Cold pattern)
Guizhi Tang (Cinnamon Twig Decoction) components: Cinnamomi Ramulus (Guizhi, 桂枝) 1 qian; Poria (Fuling, 茯苓) 3 qian; Dried Ginger (Ganjiang, 干姜) 1 qian; Schisandra Fructus (Wuweizi, 五味子) 1 qian (pounded together with ginger); Apricot Seed (Xingren, 杏仁) 1.5 qian; Honey-fried Licorice Root (Zhigancao, 炙甘草) 0.4 fen; White Peony Root (Baishao, 白芍) 1 qian; Prepared Ephedra Herba (Zhimahuang, 制麻黄) 0.5 fen.
Patient Bu (19 years old). Asthma attacks upon sudden exposure to cold. Pulse diagnosis: left side large, right side level. This indicates new external pathogen activating latent old pathogen. Proposal: expel latent pathogen, regulate Qi. Adopt the method of Xia Qing Long Tang (Minor Blue Dragon Decoction).
Patient Xu (41 years old). Chronic asthma for twenty years, a deep-seated stubborn disease, no medicine has been effective. Onset due to fright, worry, and cold invasion. Generally, worry injures the Lung. Cold enters the Back-Shu points, internally combining with the Lung system. Latent pathogen obstructs Qi and impedes phlegm. Upon attack, wheezing is severe, unable to lie down. It is like thieves hiding inside; unless they act and steal, it is hard to forcibly capture them. Although treatment should employ searching and expelling drugs during the attack, after the pathogen is removed, one must nourish Zheng Qi (Upright Qi). Now in middle age, presumably no major harm, but spirit declines daily, condition worsens. Shen Qi Wan (Kidney Qi Pill) minus Rougui (Cinnamon Bark) and Nuxi (Achyranthes Root). During attacks, Tingli Dazao Xie Fei Tang (Descurainia Seed and Jujube Decoction to Drain the Lung) or Zaojiao Wan (Soap Pod Pill).
Patient Chen (48 years old). Asthma preventing lying flat. After blood loss, chest feels slightly clear. Weijing Tang (Reed Rhizome Decoction) plus Tingli (Descurainia Seed) and Dazao (Jujube).
Patient [Surname Unknown] (13 years old). Chronic asthma with cough.
Guizhi Mu (Cinnamon Twig Wood), Xingren (Apricot Seed), Juhong (Tangerine Peel), Houpo (Magnolia Bark), Chao Banxia (Stir-fried Pinellia Tuber), Chao Baijiezi (Stir-fried White Mustard Seed).
Patient Ma (32 years old). Chronic asthma, frequent phlegm-wheezing. (Asthma combined with Phlegm-Fluid retention) Zhen Wu Wan (True Warrior Pill).
Patient Zhu (51 years old). Chronic asthma with cough and wheezing. Attacks upon exertion. Xiao Qing Long Tang (Minor Blue Dragon Decoction) minus Mahuang (Ephedra) and Xixin (Asarum), add Tang Chao Shigao (Sugar-roasted Gypsum Fibrosum).
Patient Zou (7 years old). Chronic asthma, Lung disease. Long-term leads to Qi leakage and sweating. Spleen and Stomach Yang are weak. Phlegm-fluids remain stagnant. Symptoms include regurgitation/vomiting after eating. In the three months of summer, Heat injures Zheng Qi (Upright Qi). Suitable to frequently take Si Jun Zi Tang (Four Gentlemen Decoction) to benefit Qi. No need to attack and expel phlegm-fluids. (Qi deficiency pattern)
Ren Shen (Ginseng Radix), Fuling (Poria), Baizhu (Atractylodes Macrocephala Rhizome), Zhigancao (Honey-fried Licorice Root).
Asthma (Xiao) and Wheezing (Chuan) are slightly different. The severity and urgency of their symptoms also vary slightly. Generally, asthma often accompanies wheezing, whereas wheezing may not accompany asthma. One must understand the causes of wheezing: if caused by external pathogens blocking and constraining, once the pathogen disperses, wheezing stops and does not recur. This is the Excess type of wheezing. If due to root deficiency, Kidney Qi deficiency with rebellious Qi, and turbid Yin rushing upward causing wheezing, this will likely become critical within one or two days. Medication is difficult to be effective. This is the Deficiency type of wheezing. As for asthma, it is also caused by initial external pathogen invasion, failure to release via the exterior, pathogen lying latent inside, remaining in the Lung-Shu points. Thus, it occurs frequently and stops frequently, lingering over years. There are also Phlegm-Asthma, Salty-Asthma, Vinegar-Asthma, from excessive consumption of raw/cold foods, and Infantile Congenital Asthma among other conditions. Although the cases are not exhaustive, reviewing Mr.'s treatment methods, generally focusing on warming and unblocking the Lung, and grasping Kidney Essence from below. For long-term attacks leading to Middle Jiao deficiency, one must supplement Middle Qi. Agents that are acrid-dispersing, bitter-cold, resolving phlegm, or breaking Qi are all avoided. This can be called treating the disease by seeking its root. If this condition could be treated by a physician knowledgeable in acupuncture and moxibustion, applying moxibustion according to acupoints, it would be easier to eradicate the root. Alas, such people are hard to find. (Commentary by Hua Yutang)
