The Classics state: Shock injures the Gallbladder, fear injures the Kidney. Generally, matters that are terrifying and arrive suddenly are called "Shock" (Jing); if they arrive leisurely, allowing for gradual contemplation, they are called "Fear" (Kong). Thus, shock is rapid, fear is slow. Shock syndromes occur in adults but are most common in children. Because their Shen (Spirit) is not firm and Gallbladder Qi is not full, upon encountering slightly unusual sounds or sights, they are startled abruptly. The injury from shock spreads from the Heart to the Gallbladder, then from the Gallbladder to the Liver, causing the Heart's Ministerial Fire and the Liver/Gallbladder's Xiang Fire and Wind-Wood to rise suddenly. Symptoms manifest as convulsions, coma with delirious speech, cold limbs and jue, vomiting milk with body heat, eyes rolling up, and jaw stiffness. All these ailments are essentially symptoms of the Heart, Liver, and Gallbladder, actually having no external Wind pathogen. This is because external shock agitates the internal Wood-Fire-Wind. Therefore, one should only use the character "Shock" to name the disease, which is appropriate. Because internal wind boils up, a "Wind" character is added; because the disease comes rapidly, an "Acute" character is added, thus creating the disease name "Acute Convulsive Wind" (Ji Jing Feng). This is already far-fetched. As for currently mixing it up and calling it "Acute Convulsive Wind," this is even more absurd. It is generally because children's Yin Qi is not full; external Wind-Warmth, Wind-Heat, Wind-Fire, as well as Cold transforming into Heat, and Dryness-Fire syndromes, easily damage Yin. When Yin is damaged, Blood cannot nourish tendons; when fluids are damaged, meridians become stagnant. Excessive Heat can also cause internal Wood-Fire-Wind to arise sequentially. The manifested symptoms are similar to those of shock. However, they do not actually arise from shock, and the treatment methods differ greatly. If indeed caused by shock, treatment should soothe and nourish the Heart-Shen, sedate fright and stabilize timidity, use sweet-cool herbs to clear internal heat, use soft-moist herbs to extinguish Liver-wind, or add a little aromatic herbs to open orifices and unblock collaterals, relieving blockage. Rigid-hot, drying, and astringent, or exterior-releasing drugs must absolutely not be used. If there is no shock but only external pathogenic invasion, some require cool release, some warm release, some bitter-cold fire-clearing, some sweet-warm Yang-supporting. Tonification or purgation should naturally be applied according to the Six Excesses (Liu Yin), having nothing to do with the character "Shock." Unfortunately, current physicians, when encountering non-shock syndromes, unable to distinguish which of the Six Qi caused the injury and unable to determine the disease name, forcibly mix in the "Shock" character, using the excuse of casually calling it "Acute Convulsive Wind Syndrome" to deceive and cover up. Patients' families also believe it deeply, thinking that since protection for children is hard to be perfect, how could there be no shock? Their prescribed formulas are chaotic and indecisive, not knowing that treating shock always focuses on the Heart, Liver, and Gallbladder. Treating seasonal pathogens requires discussing the Lung, Stomach, Spleen, Kidney, San Jiao, Ying and Wei, and meridians. They are vastly different. Furthermore, there is another disease name called "Slow Convulsive Wind," which is especially bizarre and unreasonable. It must be urgently refuted. There is a discussion regarding pediatric vomiting and diarrhea, which should be viewed together. (Commentary by Hua Xiuyun)
Xu's Commentary: The word "Wind" is used excellently. All Liver and Gallbladder diseases are related to Wind. The Liver is the organ of Wind. The Classics state that Wind is the chief of all diseases. Not only external Wind is called Wind, but internal Wind is also called Wind. Therefore, adults presenting with facial paralysis, coma, and syncope are also called Zhong Feng (Stroke/Wind Strike), similar to children. Why should children's diseases not be named Wind?
