A pulse that is large, slippery, and tight, or floating without root, indicates a fatal condition.
A pulse that is rapid and accompanied by the inability to eat indicates a fatal condition.
Cold and lethargic limbs, inability to turn over, diarrhea with loss of Yang qi, and panting indicate a fatal condition.
A pulse that is fine and rapid, or grand and excess, indicates survival.
A pulse that is deficient, small, and choppy indicates a fatal condition; the appearance of Guan-Ge (obstruction and reversal) syndrome with sweating on the head indicates a fatal condition.
Dribbling urine, red color, and dysuria are all caused by internal heat; it is appropriate to use methods to clear heat and promote urination.
Urinary retention is due to unfavorable Qi movement. Deficiency of Qi leads to stagnant Qi flow; deficiency of Blood leads to failure of Qi to ascend. Excess phlegm obstructs Qi transport.
Treatment principles: Tonify Qi for Qi deficiency, nourish Blood for Blood deficiency, and guide out phlegm for excess phlegm. First administer the corresponding base medicine, then employ emesis to lift the Qi.
Once the Qi ascends, water naturally flows downward; this can be supplemented with Wuling San (Five-Ingredient Poria Decoction).
Qingfei Yinzi (Clear Lung Decoction): Treats heat in the Upper Jiao (Upper Burner) affecting the Qi level, causing urinary difficulty accompanied by fever and thirst.
Ingredients: Alismatis Rhizoma (Zexie, 5 qian), Polyporus (Zhuling, 3 qian), Poria (Fuling, 2 qian), Tongsao, Akebiae Caulis (Mutong, 2 qian), Junci Medulla (Dengxin, 1 qian), Plantaginis Semen (Cheqianzi, 1 qian), Polygoni Avicularis Herba (Qumai), Agrostemma Flos (Jimianhua), Succinum (Hupo, 3 qian).
Zishen Wan (Kidney-Nourishing Pill): Treats heat in the Lower Jiao (Lower Burner) affecting the Blood level, causing urinary difficulty without thirst.
Ingredients: Phellodendri Cortex (Huangbai, wine-fried), Anemarrhenae Rhizoma (Zhimu, wine-fried, 1 qian), Cinnamomi Ramulus (Gui, a small amount). If blood stasis causes Qi obstruction, or if dead blood causes lin syndrome, add Talcum (Huashi), Poria (Fuling), and Alismatis Rhizoma (Zexie).
Niuxi Gao (Achyranthes Bidentata Paste): Treats lin syndrome caused by dead blood (blood stasis).
Mr. Li’s treatment method: Heat in the Upper Jiao is primarily treated with Gardeniae Fructus (Zhizi) and Scutellariae Radix (Huangqin); for the Middle Jiao, add Coptidis Rhizoma (Lian) and Paeoniae Radix Alba (Shaoyao); for the Lower Jiao, add Phellodendri Cortex (Huangbai); for hot lin syndrome, promote diuresis using Gardeniae Fructus (Zhizi) and similar herbs; for Qi deficiency, use Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) plus Akebiae Caulis (Mutong).
For urinary difficulty: For Qi deficiency, use Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) plus Cimicifugae Rhizoma (Shengma), followed by emesis; for Blood deficiency, use Siwu Tang (Four-Substance Decoction), followed by emesis; for phlegm-Qi stagnation, use Erchen Tang (Two-Aged Decoction) plus Akebiae Caulis (Mutong) and Cyperi Rhizoma (Xiangfu), followed by emesis.
For lin syndrome caused by the Three Causes:
Wuling San (Five-Ingredient Poria Decoction) plus Abelmoschi Semen (Kuizi), Talcum (Huoshi), and Polygoni Avicularis Herba (Qumai); add Aconiti Lateralis Radix Praeparata (Fuzi) for cold; add Scutellariae Radix (Huangqin) for heat; add blood-invigorating herbs like Gardeniae Fructus (Zhizi), Gypsum Fibrosum (Shigao), and Pyrosia Spora (Shiwei) for blood stasis; for Qi stagnation causing lower abdominal fullness and closure, add Aucklandiae Radix (Muxiang) and Aquilariae Lignum Resinatum (Chenxiang).
Fahui San (Hair Ash Powder): Treats conditions arising from excessive sexual activity and improper diet, where holding urine leads to bladder torsion and obstruction, causing urgent fullness below the umbilicus. Administer two he with vinegar.
Galii Radix (Gansui) and garlic are pounded into a cake, applied to the navel, sealed with clay, and moxibusted thirty times to treat urinary obstruction; Abelmoschi Semen (Kuizi) may also be added.
