This book is the original edition printed by Gao Shuzong in the Ming Dynasty, rarely circulated domestically and internationally. In the summer of Wuxu year, I retrieved it from an old chest, studied it repeatedly, and roughly grasped its meaning. Applying the methods described, I often achieved remarkable effects. Scholars gradually began to recommend me as a knowledgeable physician, largely due to the power of this book. Only "Xinfa" (Essential Methods) was available in the market, while "Yiyao" (Medical Essentials) was seldom seen. In his later years, Master Zhu Zhenheng took content not fully covered in these two books, deliberated on additions and deletions, and finalized this edition. Although it represents one school's viewpoint and inevitably has some inconsistencies between earlier and later parts, its essence far surpasses those two books. At that time, people compared Master Danxi among physicians to Master Kaoting (Zhu Xi) among Confucian scholars. He spent years writing; the chapter on "Chengyi" (Sincerity of Will) was not finalized until his twilight years, showing that ancient sages did not dare to speak lightly when establishing words for posterity. However, the original book had been dormant for a long time, with corners and edges damaged by insects. Fortunately, the characters remained clear; upon opening the volume, brilliance and precious aura leaped onto the paper. This could not have been achieved without spiritual protection. Coincidentally, high officials in Jiangsu Province established a Medical Research Institute in the south of the city, gathering famous physicians from Wu region. I served as a member there and occasionally mentioned this book. Everyone regretted not having seen it and feared it would be lost over time. Thus, we raised funds to reprint it for public benefit, hoping that scholars studying Danxi's learning might find it useful.
Recorded respectfully by Xiao Shulin, a humble scholar from Qiantang, in the first month of Jiyou year, Xuantong Era.
Apoplexy is generally caused by blood deficiency and phlegm. Treatment prioritizes resolving phlegm, followed by nourishing and moving blood, or viewing it as blood deficiency with fire and dampness. The general principle is to remove phlegm primarily, and ginger juice is indispensable when tonifying. *Neijing* states: "Where pathogenic factors gather, Qi must be deficient." Liu Hejian considered it an internal injury heat disease; Zhang Zhongjing considered it an external pathogenic invasion. Wind pathogens mostly affect the Lung organ. Hemiplegia is generally associated with phlegm (see *Yiyao*). For cases with severe phlegm obstruction, facial paralysis, or inability to speak, emesis is indicated. For mild cases where consciousness is retained, Gua Di San (Melon Seed Powder) or Xi Xian San (Thin Saliva Powder) may be used. Alternatively, half a jin of shrimp can be boiled with sauce, green onion, and pepper. Eat the shrimp first, then drink the broth, inducing vomiting to draw out wind-phlegm. However, there are cases of deficiency where emesis is contraindicated. For sudden collapse, emesis is indicated. For sudden collapse due to Qi deficiency, Ginseng Radix et Rhizoma (Renshen) and Astragali Radix (Huangqi) should be used to tonify. Enuresis belongs to Qi deficiency and should be treated with Ginseng Radix et Rhizoma (Renshen) and Astragali Radix (Huangqi). For Qi deficiency with phlegm, decoct Ginseng Radix et Rhizoma (Renshen) thickly and add Arundo Rhizoma Juice (Zhuli) and Ginger Juice (Jiangzhi). For blood deficiency, Siwu Tang (Four Substances Decoction) is suitable for tonification; all herbs should be stir-fried with ginger juice to prevent cloying and generating phlegm; adding Arundo Rhizoma Juice (Zhuli) and Ginger Juice (Jiangzhi) also treats concurrent phlegm. For excess phlegm-Qi and good appetite, Jingli (Ampelopsis Japonica Juice) is used; for Qi deficiency and poor appetite, Zhuli (Arundo Rhizoma Juice) is used. These two substances resolve phlegm, open collaterals, and move Qi and Blood. When added to formulas like Siwu Tang (Four Substances Decoction), a small amount of ginger juice must be added to assist. In apoplexy patients, if tendons hurt upon movement, it indicates lack of blood nourishing the tendons, named "Jinku" (Tendon Withering), which is incurable. Fat individuals with white complexion often have phlegm-dampness; Aconiti Lateralis Radix Praeparata (Fuzi) and Aconiti Radix (Wutou) are used to unblock meridians. Upon sudden collapse, press Renzhong (GV26) until recovery, then use phlegm-resolving formulas such as Erchen Tang (Two-Cured Decoction), Sijunzi Tang (Four Gentlemen Decoction), or Siwu Tang (Four Substances Decoction), modified as needed. Thin individuals with Yin deficiency and fire-heat use Siwu Tang (Four Substances Decoction) plus Achyranthis Bidentatae Radix (Niuxi), Arundo Rhizoma Juice (Zhuli), Scutellariae Radix (Huangqin), and Phellodendri Cortex (Huangbai); add phlegm-resolving herbs if phlegm is present. A fat patient with apoplexy, facial paralysis, and numbness in limbs, treated for phlegm bilaterally, used Fritillariae Bulbus (Beimu), Trichosanthis Fructus (Gualou), Pinelliae Rhizoma Preparatum (Nanxing), Pinelliae Rhizoma (Banxia), Citri Reticulatae Pericarpium (Chenpi), Atractylodis Macrocephalae Rhizoma (Baizhu), Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), Phellodendri Cortex (Huangbai), Notopterygii Rhizoma et Radix (Qianghuo), Saposhnikoviae Radix (Fangfeng), Schizonepetae Spica (Jingjie), Sparaxis Herba (Weilingxian), Menthae Haplocalycis Herba (Bohe), Cinnamomi Ramulus (Guizhi), Glycyrrhizae Radix et Rhizoma (Gancao), and Trichosanthis Radix (Tianguafen). If consuming much wheat noodles, add Typhonii Rhizoma (Baifuzi), Arundo Rhizoma Juice (Zhuli), Ginger Juice (Jiangzhi), and one spoonful of wine to unblock meridians. A woman over sixty with left-sided hemiplegia, aphasia, but good appetite, was treated with Saposhnikoviae Radix (Fangfeng), Schizonepetae Spica (Jingjie), Notopterygii Rhizoma et Radix (Qianghuo), Pinelliae Rhizoma Preparatum (Nanxing), Myrrhae Resina (Moyao), Olibanum (Ruxiang), Akebiae Caulis (Mutong), Poria (Fuling), Magnoliae Officinalis Cortex (Houpo), Platycodonis Radix (Jiegeng), Glycyrrhizae Radix et Rhizoma (Gancao), Ephedrae Herba (Mahuang), Scorpio (Quanxie), Carthami Flos (Honghua). Pulverized and taken with warm wine, it was effective. In spring, with faint pulse, induce vomiting with light salt water and mustard juice, one bowl every morning. On the fifth day, continue with Atractylodis Macrocephalae Rhizoma (Baizhu), Citri Reticulatae Pericarpium (Chenpi), Poria (Fuling), Glycyrrhizae Radix et Rhizoma (Gancao), Magnoliae Officinalis Cortex (Houpo), and Acori Tatarinowii Rhizoma (Shangpu), two doses daily. Later, use Chuanxiong Rhizoma (Chuanxiong), Gardeniae Fructus (Shanzhi), Sojae Semen Preparatum (Douchi), Melonis Fructus (Gua Di), Mung Bean Powder (Lvdou Fen), mustard juice, and salt water to induce rapid vomiting, stopping food intake. Then administer Sijunzi Tang (Four Gentlemen Decoction), followed by pills made of Angelicae Sinensis Radix (Danggui), Scutellariae Radix prepared with wine (Jiuqin), Carthami Flos (Honghua), Akebiae Caulis (Mutong), Magnoliae Officinalis Cortex (Houpo), Arctii Fructus (Niuzi), Atractylodis Rhizoma (Cangzhu), Pinelliae Rhizoma prepared with ginger (Jiangnanxing), Achyranthis Bidentatae Radix (Niuxi), and Poria (Fuling), bound with wine paste into pills the size of tung seeds. After ten days, slight night sweats occurred, limbs moved, and speech returned. Another apoplexy case used Fritillariae Bulbus (Beimu), Trichosanthis Fructus (Gualou), Pinelliae Rhizoma Preparatum (Nanxing), Pinelliae Rhizoma (Banxia), Coptidis Rhizoma prepared with wine (Jiulian), Scutellariae Radix prepared with wine (Jiuqin), Phellodendri Cortex prepared with wine (Jiubai), Saposhnikoviae Radix (Fangfeng), Schizonepetae Spica (Jingjie), Notopterygii Rhizoma et Radix (Qianghuo), Menthae Haplocalycis Herba (Bohe), Cinnamomi Ramulus (Guizhi), and Sparaxis Herba (Weilingxian). A fat person with apoplexy was first vomited, then treated with Atractylodis Rhizoma (Cangzhu), [missing], Scutellariae Radix prepared with wine (Jiuqin), Phellodendri Cortex prepared with wine (Jiubai), Akebiae Caulis (Mutong), Poria (Fuling), Achyranthis Bidentatae Radix (Niuxi), Carthami Flos (Honghua), Cimicifugae Rhizoma (Shengma), Magnoliae Officinalis Cortex (Houpo), and Glycyrrhizae Radix et Rhizoma (Gancao). A fat person with facial paralysis and hand paralysis, with strong pulse, used Pinelliae Rhizoma Preparatum (Nanxing), Pinelliae Rhizoma (Banxia), Menthae Haplocalycis Herba (Bohe), Cinnamomi Ramulus (Guizhi), Sparaxis Herba (Weilingxian), Scutellariae Radix prepared with wine (Jiuqin), Phellodendri Cortex prepared with wine (Jiubai), Trichosanthis Radix (Tianguafen), Fritillariae Bulbus (Beimu), Schizonepetae Spica (Jingjie), Trichosanthis Fructus (Gualou), Atractylodis Macrocephalae Rhizoma (Baizhu), Citri Reticulatae Pericarpium (Chenpi), Zingiberis Rhizoma Recens (Shengjiang), Glycyrrhizae Radix et Rhizoma (Gancao), Saposhnikoviae Radix (Fangfeng), Notopterygii Rhizoma et Radix (Qianghuo), and Arundo Rhizoma Juice (Zhuli). A right-sided hemiplegia case used: Coptidis Rhizoma prepared with wine, Phellodendri Cortex prepared with wine, Saposhnikoviae Radix (each 0.5 liang), Pinelliae Rhizoma (1 qian), Notopterygii Rhizoma et Radix (5 qian), Scutellariae Radix prepared with wine, Ginseng Radix et Rhizoma (Renshen), Atractylodis Rhizoma (Cangzhu) (each 1 liang), Chuanxiong Rhizoma (Chuanxiong), Angelicae Sinensis Radix (Danggui) (each 5 qian), Ephedrae Herba (Mahuang) (3 qian), Glycyrrhizae Radix et Rhizoma (Gancao) (1 qian), Pinelliae Rhizoma Preparatum (Nanxing) (1 liang), Aconiti Lateralis Radix Praeparata (Fuzi) (3 slices). Made into pills the size of bullets, dissolved in wine. A fat person with worry, thoughts, and Qi stagnation causing right-hand paralysis, [missing] Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). With phlegm, add Pinelliae Rhizoma (Banxia) and Arundo Rhizoma Juice (Zhuli) [missing]. Apoplexy symptoms include facial paralysis, abnormal speech, drooling at the mouth corner, or whole-body/half-body paralysis; all are treated here. These result from chronic weakness of Yuan Qi, external pathogenic invasion, and excesses in alcohol and sexual activity. Use equal parts of Ginseng Radix et Rhizoma (Renshen), Saposhnikoviae Radix (Fangfeng), Ephedrae Herba (Mahuang), Notopterygii Rhizoma et Radix (Qianghuo), Cimicifugae Rhizoma (Shengma), Platycodonis Radix (Jiegeng), Gypsum Fibrosum (Shigao), Scutellariae Radix (Huangqin), Schizonepetae Spica (Jingjie), Gastrodiae Rhizoma (Tianma), Pinelliae Rhizoma Preparatum (Nanxing), Menthae Haplocalycis Herba (Bohe), Cinnamomi Ramulus (Guizhi), Puerariae Lobatae Radix (Gegen), Paeoniae Radix Rubra (Chishao), Armeniacae Semen Amarum (Xingren), Angelicae Sinensis Radix (Danggui), Chuanxiong Rhizoma (Chuanxiong), Atractylodis Macrocephalae Rhizoma (Baizhu), Asari Radix et Rhizoma (Xixin), and Zanthoxyli Fructus (Zhuyazaojiao). Decoct with ginger and green onion. Add half a cup of Arundo Rhizoma Juice (Zhuli) to drink together, apply moxibustion, and achieve slight sweating for cure. A man near sixty, indulging in rich food, suffered from dysentery in midsummer, and engaged in sexual activity. Suddenly, while using the toilet, his hands flailed, eyes opened without light, urine flowed out, sweat poured like rain, throat sounded like sawing, breathing was very faint, pulse was large and irregular, terrifying in appearance. This indicates Yin deficiency preceding sudden Yang collapse. Immediately order preparation of Ginseng Paste, and moxa Qihai (CV6) with cones the size of a little finger, 18 moxibustions, after which the right hand could move. Three more moxibustions, lips moved slightly. The paste was ready, give one cup. By midnight, after three cups, eyes could move. After two jin, he could speak and ask for porridge. After five jin, diarrhea stopped. After dozens of jin, he recovered. Postpartum apoplexy in women must not be treated as typical apoplexy with Xiao Xuming Tang (Minor Continue Life Decoction). It requires major tonification of Qi and Blood, then treatment of phlegm. Differentiate Qi and Blood deficiency based on radial pulses. General principles for treating apoplexy: Clear Heart Fire so Lung Metal clears, Liver Wood does not become excessive, thus Spleen is not injured; Tonify Kidney Water so Heart Fire descends, Lung receives no heat; Spleen and Lung are safe, Yangming is solid; Yangming solidity moistens Zongjin (tendons), binding bones and facilitating joints. Du Qingbi's Tongshen San (Spirit-Unblocking Powder): seven dried and powdered white silkworms, mixed with half a cup of ginger juice, induces immediate vomiting of wind-phlegm. Use seven more shortly after, repeat until phlegm is exhausted. Then use rhubarb the width of two fingers, wrapped in paper and roasted, swallow with saliva. After a meal interval, use rhubarb again. If jaw is tightly closed, decoct silkworm juice and instill through bamboo tube into nose, left side for men, right for women. Regarding apoplexy, *Neijing* and subsequent texts attribute it to external wind-pathogen. However, geography varies north and south, cannot be uniform. Liu Hejian's view of improper regimen leading to water failing to control fire is correct. From modern perspective, external wind-apoplexy exists but is rare. Cannot entirely attribute it to improper regimen denying external cause. Scholar Xu stated "Qi stroke" exists, caused by Seven Emotions, with floating-rapid, floating-tight, slow-slow pulses, all wind pulses. Slow-floating is treatable; large-rapid-urgent is fatal. If truly external, Dongyuan's theory of affecting vessels, fu-organs, and zang-organs is excellent. Limbs unable to rise may resemble Wei syndrome (flaccidity), requiring careful differentiation. *Ju Fang* treating apoplexy and Wei syndrome together is a great error. *Fahui* details this. Zhang Hezi's three methods are indeed applicable for acute pathogenic attack, severe phlegm, and excess heat; otherwise, not applicable.
