Postpartile cholera is caused by the dual injury of Qi and Blood, leading to deficiency and impairment of the Zang-Fu organs; or it results from undigested food and exposure to wind-cold pathogens. When Yin and Yang fail to harmonize, clear and turbid energies interfere with each other, causing Qi chaos between the intestines and stomach. The upright Qi and pathogenic factors contend, and cold and heat are unbalanced, resulting in vomiting above and diarrhea below; hence it is called cholera. The classics state: If there is thirst and desire for water, use Wuling San (Five-Ling Powder); if there is much cold and no desire for water, use Lizhong Wan (Regulate the Middle Pill); if there is severe deficiency-cold and cold extremities, adding Fuzi Laifu Dan (Aconite Reviving Pill) is also excellent.
Baizhu San (Atractylodes Macrocephala Powder)
Treats postpartum cholera with vomiting, diarrhea, abdominal pain, irritability, thirst, and cold extremities.
Ingredients: Baizhu (Atractylodis Macrocephalae Rhizoma), Juhong (Citri Reticulatae Pericarpium Rubrum), Maidong (Ophiopogonis Radix, heart removed), Ganjiang (Zingiberis Rhizoma Recens), Renshen (Ginseng Radix et Rhizoma) (each 1 qian), Gancao (Glycyrrhizae Radix et Rhizoma) (0.5 qian).
Add one cup of water and three slices of fresh ginger, decoct and take orally.
Wenzhong San (Warm the Middle Powder)
Treats postpartum cholera with persistent vomiting and diarrhea.
Ingredients: Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Danggui (Angelicae Sinensis Radix) (each 1 qian), Caodoukou Ren (Cardamomi Fructus Seed), Ganjiang (Zingiberis Rhizoma Recens) (each 0.8 qian), Houpo (Magnoliae Officinalis Cortex, processed with ginger juice) (1.2 qian).
Add one cup of water, decoct and take orally.
If vomiting is severe and oral medication cannot be retained, use Fulonggan (Dragon's House Soil) ground into fine powder (3 qian), mixed with rice soup. Swallow slowly to ensure retention.
Postpartum metrorrhagia is mostly due to fright, worry, and anger causing disharmony of the Zang-Qi. Or, taking hemostatic drugs too early causes lochia (postpartum residual blood) to fail to dissipate, leading to stagnation, fullness, and hardening, thus forming metrorrhagia. Xue Lizhai stated: If blood stasis causes lower abdominal distension and fullness, use Shixiao San (Pain-Relieving Powder), see "Blood Syncope" chapter; if blood deficiency causes lower abdominal emptiness and痞 (feeling of obstruction), use Xiong Gui Tang (Ligusticum and Angelica Decoction); if Liver fire causes reckless bleeding, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder); if Spleen depression fails to control blood, use Jiawei Guipi Tang (Modified Restore the Spleen Decoction); if Spleen deficiency fails to hold blood, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction); if rich flavors accumulate heat, use Qingwei San (Clear the Stomach Powder) plus Huaihua (Sophorae Flos); if wind and heat contend, use Sijunzi Tang (Four Gentlemen Decoction) plus Fangfeng (Saposhnikoviae Radix) and Zhike (Aurantii Fructus Immaturus). One parturient had metrorrhagia with lower abdominal distension and pain; after using Qi-regulating and blood-invigorating agents, the bleeding surged like a spring, limbs became flaccid, accompanied by aversion to cold, vomiting, and frequent diarrhea. I used Liujunzi Tang (Six Gentlemen Decoction) plus Paohei Ganjiang (Charred Ginger Rhizome) for four doses, showing slight improvement, then cured with Shiquan Dabu Tang (Ten Great Tonifying Decoction) for over thirty doses. Another parturient had metrorrhagia due to anger, with blood surging, collapsing with locked jaw, deviated eyes, and convulsions of hands and feet; this was wind generated from Liver channel blood depletion. I used Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) material for one dose, all symptoms receded, but with poor appetite and afternoon fever; assisted with Sijunzi Tang, Chaihu (Bupleuri Radix), and Mudanpi (Moutan Cortex) led to recovery.
