The *Mai Jue* (Pulse Classic) states: The pulse for epilepsy is floating, surging, large, long, slippery, firm, and rapid. If accompanied by phlegm obstructing the Heart orifices leading to mania, a deficient pulse indicates treatability, while an excess pulse indicates death. It also states: A pulse that fluctuates between large and small, long and short, are all aberrant pulses indicating mental confusion. Dian syndrome (catatonic schizophrenia/epilepsy) belongs to Yin; Kuang syndrome (manic psychosis) belongs to Yang. Dian patients tend to laugh excessively, while Kuang patients tend to anger. Generally, both are caused by phlegm knotting in the chest and heart region; treatment should focus on regulating the Heart Spirit and resolving phlegm knots. Some cases arise from pathogenic invasion, which should be treated by dispelling pathogens. Dian syndrome involves the Spirit failing to reside in its abode; Kuang syndrome involves hallucinations and visions persisting for years without cure. Damage to the Heart Channel constitutes a true disease. If there is heat accumulation in the Heart Channel, clear the Heart and remove heat; if phlegm confuses the Heart orifices, resolve phlegm and calm the Spirit. If the patient cries and groans during an episode, it is possession by evil spirits, not Kuang. Burn mulberry paper, take one cun cube with wine. For sudden manic speech resembling ghostly chatter, needle the area under the nail of the thumb to stop it immediately. Kuang disease is characterized by little sleep, lack of hunger, laughing and singing, and ceaseless erratic movement. Dian disease is characterized by persistent unhappiness. Staring straight ahead with the head tilted back is called "Jiang" (stiff); face down is called "Pu" (prostrate). When the pulses at the three positions (Cun, Guan, Chi) show excess in both Yin and Yang, it is generally due to phlegm knotting in the chest and heart region; treatment should settle the Heart and resolve phlegm knots, following the same method as for epilepsy.
