Zhang Zhongjing said: When diarrhea occurs with pain in the anal canal, a warming method should be applied. One may use hot salt powder for moxibustion-like熨烫 (hot compress). Another method is to use roasted Zhi Shi (Immature Bitter Orange) for hot compress. *Qianjin Yi Fang* states that roasted Zhi Shi is suitable. If using roasted salt or similar materials like roasted Bombyx Batryticatus (Can Sha), they can also be used for hot compress. Pang Anshi stated that mixing Zhi Shi powder with salt yields better results. Using Zhi Shi powder alone is inappropriate. Zhi Shi must be roasted slice by slice; this refers to Zhi Qiao (Aurantium Fructus).
Pang Anshi stated: Cold accumulation in the umbilicus cannot be immediately treated with hot compress. If cold qi attacks the heart and abdomen, death is inevitable. One must first warm it with medicine, and after a long period, apply hot compress.
He also said: For cold accumulation below the umbilicus, or where Yang Qi is blocked leading to obstruction of urination and defecation, although many medicines are taken without effect, applying hot compress with roasted salt to the area below the umbilicus will lead to unblocking within moments. If the patient has already taken excessive amounts of Ba Dou (Crotonis Fructus), Gan Sui (Kansui Radix), Da Huang (Rhei Radix et Rhizoma), or Qing Fen (Calomelas), slight unblocking will result in severe diarrhea and damage the body. Careful examination is required.
Pang Anshi said: In Shanghan (Cold Damage), if the doctor places fire under the bed, uses fire around the whole body to force sweating, applies hot compress, or mistakenly uses moxibustion, these are all caused by Fire Evil. Zhang Zhongjing said: In Shanghan with a floating pulse, if the doctor uses fire to force sweating, causing Wang Yang (Loss of Yang Qi), there will inevitably be panic and madness, or restlessness in lying down and rising up. Gui Zhi Qu Shao Yao Jia Shu Xi Mu Li Long Gu Jiu Ni Tang (Cinnamon Twig Decoction minus Peony plus Chinese Asparagus Root, Oyster Shell, and Dragon Bone to Rescue from Deviation) is indicated.
He also said: For fire deviation followed by purgation, resulting in irritability due to burning needles, Gui Zhi Gan Cao Tang (Cinnamon Twig and Licorice Decoction) is the primary treatment. Guo Yong stated that it can be used as a substitute for Jiu Ni Tang.
Pang Anshi said: After moxibustion and burning needles, if symptoms resemble those of fire coercion, they should all be treated with fire coercion methods. For irritability, palpitations, and madness, Feng Yin Tang (Wind-Driving Decoction) from *Jin Gui Yao Lue* is particularly excellent. Chai Hu Jia Long Gu Mu Li Tang (Bupleurum Decoction plus Dragon Bone and Oyster Shell) is also generally applicable.
*Qian Jin Fang* states: The condition is not suitable for moxibustion because fire becomes evil, dispersing through the blood vessels. Injury to the vessels is tolerable, but injury to the zang organs is severe. Swelling at the Jing-Well and Shu-Acupoints, spontaneous yellow sweat, external rotting of meridians, and muscle corruption turning into abscesses and pus constitute Huo Dan (Fire Jaundice), which is iatrogenic injury. Guo Yong stated: *Qian Jin Yi Fang* contains a chapter on Huo Dan, which was misplaced after the Hu Huo (Fox-Sore) chapter. It also combined the contraindication of moxibustion for fine-rapid pulses into one section. It further stated that Xie Xin Tang (Drain the Epigastrium Decoction) should be used. These statements are erroneous and unreliable. The contraindication of moxibustion for fine-rapid pulses has already been seen in this chapter. Only leaving the Huo Dan syndrome seems related to Fire Evil, so it is recorded here. *Wai Tai Mi Yao* uses Xie Xin Tang to treat erosion in the upper part belonging to Hu Huo, placing Huo Dan and fine-rapid pulse afterwards. This error has existed for a long time.
