The pulse is soft and weak; the weak pulse appears at the Guan position (three fen below the Guan), the Ru (soft) pulse appears at the Dian position (three fen above the Guan), the Wei (fine) pulse appears at the upper part (Cunkou), and the Se (choppy) pulse appears at the lower part (Chi position). A Wei pulse indicates insufficiency of Yang Qi; a Se pulse indicates lack of Blood. If Yang Qi is insufficient, wind-stroke with sweating may be accompanied by irritability. If there is lack of Blood, there will be cold limbs and chills. Jianzhong Tang (Middle-Building Decoction) is indicated. (Formula found in the section on "Conditions Where Sweating Is Contraindicated".)
When Yang Qi is deficient, purgation must not be used; purging will lead to epigastric fullness and obstruction (Xia Pi).
In all cases where Qi movement occurs in the heart and abdomen, up, down, left, or right, purgation must not be used.
If Qi movement is on the right, purgation must not be used; purging will deplete internal fluids, causing dry throat, dry nose, dizziness, and palpitations. (Zhuye Tang (Bamboo Leaf Decoction) is indicated. Formula found in the section on "Conditions Where Sweating Is Contraindicated".)
If Qi movement is on the left, purgation must not be used; purging will cause continuous abdominal cramping, exacerbate the Qi movement, and cause fatigue despite body heat. First administer Ganjiang Gancao Tang (Dried Ginger and Licorice Decoction), then Jianzhong Tang. (Formulas found in the Taiyang Syndrome section and the section on "Conditions Where Sweating Is Contraindicated", respectively.)
If Qi movement is on the upper side, purgation must not be used; purging will cause heat and irritability in the palms, cold body, spontaneous sweating of cold sweat, and a desire for water to be poured over oneself. Zhuye Tang (Bamboo Leaf Decoction) is the primary treatment. (Formula found in the section on "Conditions Where Sweating Is Contraindicated".)
If Qi movement is on the lower side, purgation must not be used; purging will cause abdominal fullness, dizziness upon sudden rising, passage of undigested food after eating, and hard epigastric obstruction. Banxia Xiexin Tang (Pinellia Heart-Draining Decoction) is the primary treatment. (Formula found in the Pi Syndrome section.)
A sensation of closure in the throat contraindicates purgation.
Pang Anshi stated: When pulses such as Ru, Ruo, Wei, Xu, and Xi appear together, purgation must not be used.
Sini syndrome (Four-Chill Syndrome) contraindicates purgation; this also applies to patients with deficiency constitutions.
Pang Anshi stated: If indications for purgation are fully present but Sini symptoms appear, it is due to Qi and Blood stagnation resulting from missed purgation. If only the hands and feet are slightly cold, the palms remain warm, and the fingers occasionally become warm, purgation may be administered without undue hesitation.
In typhoid fever, frequent vomiting contraindicates purgation, even if Yangming syndrome signs are present.
Pang Anshi stated: Even if indications for purgation exist, if urination is difficult or scanty, purgation must not be performed.
Forcibly purging when purgation is not indicated can lead to severe diarrhea that cannot be controlled, resulting in death.
