庞曰∶脉浮紧,无汗,服汤未中病。其人发烦,目瞑,极者必衄。小衄而脉尚浮者,宜麻黄汤;
The general principle is that in spring, it is appropriate to use diaphoresis (sweating therapy).
In Taiyang Wind Strike syndrome, the pulse is floating at the Yang position (cun) and weak at the Yin position (chi). Floating Yang indicates spontaneous fever, while weak Yin indicates spontaneous sweating. The patient presents with shivering aversion to cold, chilly aversion to wind, mild fever as if covered by feathers, nasal congestion with roaring sounds, and dry retching. Guizhi Tang (Cinnamon Twig Decoction) is the primary treatment.
Ingredients: Guizhi (Ramulus Cinnamomi), Shaoyao (Radix Paeoniae Rubra), Shengjiang (Rhizoma Zingiberis Recens) each 1.5 liang; Gancao (Radix Glycyrrhizae) 1 liang; Dazao (Fructus Jujubae) 6 pieces.
Crush the herbs, add 3.5 sheng of water, and decoct over low heat until 1.75 sheng remains. Strain through raw cloth to remove dregs. Take one zhan warm. Shortly after, drink a bowl of hot porridge to assist the medicinal power. Cover the patient for about one hour; mild sweating over the whole body is beneficial, but profuse sweating like flowing water must be avoided, otherwise the disease will not be resolved. If sweating occurs after one dose and the illness resolves, stop subsequent doses; there is no need to finish the entire prescription. If no sweating occurs, administer again according to the previous method, allowing three doses within half a day. For severe cases, one prescription may be consumed within a day and night. If symptoms persist, prepare and administer another dose. If sweating still does not occur, up to two or three prescriptions may be needed. Avoid raw/cold foods, sticky/slippery foods, meat, wheat products, the "Five Pungent Spices," wine, dairy, and foul-smelling substances.
In Taiyang disease, if the patient experiences increased irritability and unresolved symptoms after the first dose of Guizhi Tang, first puncture Fengchi (GB20) and Fengfu (GV16), then administer Guizhi Tang (Cinnamon Twig Decoction) for recovery. Note: Fengchi (GB20) belongs to the Shaoyang meridian and is a meeting point of the Yang Linking Vessel (Yangwei Mai). It is chosen over Tianzhu (BL10) because the Yang Linking Vessel connects all Yang meridians, and the Foot-Taiyang Bladder meridian governs Qi among the Yang meridians.
Pang Anshi states: In all Guizhi Tang (Cinnamon Twig Decoction) syndromes, the patient typically has spontaneous sweating, infrequent urination, and warm hands and feet. If fingers are slightly exposed, they feel mildly cold; covering them restores warmth. The body is hot, with mild irritability and aversion to cold. Only then should Guizhi Tang (Cinnamon Twig Decoction) be used. If the patient has no sweating, frequent urination, cold extremities, no aversion to cold but aversion to heat, or has recently consumed alcohol, Guizhi Tang (Cinnamon Twig Decoction) must strictly be avoided. (A tight pulse invariably indicates no sweating; even if sweating is present, it cannot be mistaken for a Guizhi Tang syndrome.)
In Taiyang disease, if excessive sweating leads to continuous leakage of sweat, aversion to wind, difficulty in urination, slight stiffness of the four limbs making flexion and extension difficult, Guizhi Jia Fuzi Tang (Guizhi Decoction with Added Aconite Root) is indicated. (Add one prepared Fuzi (Radix Aconiti Lateralis Preparata), peeled and sliced, to Guizhi Tang (Cinnamon Twig Decoction) and decoct as before. Difficulty in urination indicates sufficient body fluids for sweating; if urination is frequent, do not mistake it for Yangdan Syndrome, which is an alternative name for Guizhi Tang (Cinnamon Twig Decoction).)
After purgation, if the pulse is hurried and there is chest fullness, Guizhi Qu Shaoyao Tang (Guizhi Decoction without Paeonia) is indicated. (Remove Shaoyao (Radix Paeoniae Rubra) from Guizhi Tang (Cinnamon Twig Decoction), retaining only four ingredients. Shaoyao (Radix Paeoniae Rubra) is sour; in cases of hurried pulse and chest fullness, there is fear of forming Jie Xiong (Chest Binding); thus, the assisting role of Shaoyao (Radix Paeoniae Rubra) is removed to fully utilize pungent and sweet properties for dispersing toxic vapors.)
If, after taking Guizhi Tang (Cinnamon Twig Decoction) or undergoing purgation, the patient still suffers from stiff pain in the head and neck, mild fever, absence of sweating, fullness and mild pain in the epigastrium, and inadequate urination, Guizhi Qu Gui Jia Baizhu Fuling Tang (Guizhi Decoction without Cinnamon Twig plus Atractylodes Macrocephala and Poria) is indicated, with Baizhu (Rhizoma Atractylodis Macrocephalae) and Fuling (Sclerotium Poriae) each at 1.5 liang. (Omit Guizhi (Ramulus Cinnamomi), add water to make 4 sheng, decoct to obtain 2 sheng.)
In Taiyang disease, if there is stiffness and discomfort in the nape and back, sweating, and aversion to wind, Guizhi Jia Gegen Tang (Guizhi Decoction with Added Kudzu Root) is used, adding 2 liang of Gegen (Radix Puerariae), increasing water to 4 sheng, and decocting to 2 sheng. This treats Rou Jing (Flaccid Spasm).
In Taiyang disease, if mild wheezing occurs after purgation, the exterior syndrome is unresolved. Guizhi Jia Houpo Suxing Tang (Guizhi Decoction with Added Magnolia Bark and Apricot Seed) is indicated. (Add 1 liang of Houpo (Cortex Magnoliae Officinalis) and 40 Suiren (Semen Pruni Armeniacae) to Guizhi Tang (Cinnamon Twig Decoction).)
This describes the Wind Strike syndrome with spontaneous sweating, applicable to Guizhi Tang (Cinnamon Twig Decoction).
Pang Anshi states: "Aversion to cold" (E Han) refers to feeling chilled without exposure to wind; "aversion to wind" (E Feng) refers to feeling chilled when exposed to wind. Both belong to exterior syndromes. In Taiyang disease, with headache, fever, body pain, severe joint pain, aversion to wind, absence of sweating, and wheezing, Mahuang Tang (Ephedra Decoction) is indicated.
Ingredients: Mahuang (Herba Ephedrae) 1.5 liang; Guizhi (Ramulus Cinnamomi) 1 liang; Gancao (Radix Glycyrrhizae) 0.5 liang; Xingren (Semen Pruni Armeniacae) 35 pieces.
Crush, add 2.5 sheng of water, boil Mahuang (Herba Ephedrae) for several boils, skim off the foam, add other herbs, and decoct to 1.25 sheng. Strain. Drink one zhan warm, continuing until finished. Do not use porridge to aid absorption. Cover warmly, nursing care similar to Guizhi Tang (Cinnamon Twig Decoction). If no sweating occurs, prepare two or three more doses.
Pang Anshi states: The pulse of Shang Han (Cold Damage) is tight, vigorous, and feels choppy upon deep pressure. A floating and tight pulse: floating indicates Wind, tight indicates Cold. Wind injures Wei Qi (Defensive Qi), Cold injures Ying Blood (Nutritive Blood). When both Ying and Wei are diseased, there is severe joint pain. External signs include fever, absence of sweating, and possibly wheezing. The patient feels only aversion to cold; the fingertips are mildly cold initially but warm up later, while the palms remain warm. This is Shang Han (Cold Damage) with no sweating, indicating Mahuang Tang (Ephedra Decoction).
Whenever the pulse is floating and rapid, or floating and tight, with no sweating and infrequent urination, even if the illness has lasted over ten days, Mahuang Tang (Ephedra Decoction) is still appropriate.
In Taiyang Wind Strike, if the pulse is floating and tight, with fever, aversion to cold, body pain, absence of sweating, and irritability, Da Qing Long Tang (Major Blue Green Dragon Decoction) is indicated. If the pulse is weak and faint, with spontaneous sweating and aversion to wind, it must not be taken; doing so would lead to cold extremities and muscle twitching, which is a misdirection. Da Qing Long Tang (Major Blue Green Dragon Decoction):
Mahuang (Herba Ephedrae) 3 liang; Guizhi (Ramulus Cinnamomi), Gancao (Radix Glycyrrhizae), Shigao (Gypsum Fibrosum) each 1 liang; Xingren (Semen Pruni Armeniacae) 20 pieces; Dazao (Fructus Jujubae) 5 pieces; Shengjiang (Rhizoma Zingiberis Recens) 1.5 liang.
Crush, add 5 sheng of water, boil Mahuang (Herba Ephedrae) for several boils, skim foam, add other herbs, and decoct to 2 sheng. Drink one zhan warm; mild sweating is the goal. Stop if the whole body is moist; if not, wait a moment and finish. Do not induce excessive sweating, as this causes Wang Yang (Loss of Yang Qi). Loss of Yang Qi leads to deficiency, aversion to wind, irritability, and insomnia.
In Shang Han (Cold Damage), if the pulse is floating and slow, the body is not painful but heavy, occasionally feeling lighter, and there are no Shaoyin signs, Da Qing Long Tang (Major Blue Green Dragon Decoction) is indicated. (Note: This should refer to Taiyin Syndrome. Taiyin Syndrome includes floating-slow pulse and warm extremities. Taiyin requires diaphoresis; the syndrome fits Qing Long Tang; it resembles Guizhi Tang syndrome but has no sweating and a tight pulse; it resembles Mahuang Tang syndrome but lacks body pain and has a floating-slow pulse.)
In Taiyang disease, with stiffness in the nape and back, absence of sweating, and aversion to wind, Gegen Tang (Kudzu Decoction) is indicated.
Ingredients: Gegen (Radix Puerariae) 2 liang; Mahuang (Herba Ephedrae) 1.5 liang; Guizhi (Ramulus Cinnamomi) 1 liang; Gancao (Radix Glycyrrhizae) 1 liang; Shaoyao (Radix Paeoniae Rubra) 1 liang; Dazao (Fructus Jujubae) 6 pieces; Shengjiang (Rhizoma Zingiberis Recens) 1.5 liang.
Crush, add 4 sheng of water, first boil Mahuang (Herba Ephedrae) and Gegen (Radix Puerariae) for several boils, skim foam, add other herbs, and decoct to 2.5 sheng. Strain. Drink one zhan warm, three times daily, nursing care as with Guizhi Tang (Cinnamon Twig Decoction).
In combined Taiyang and Yangming disease with spontaneous diarrhea, Gegen Tang (Kudzu Decoction) is indicated.
In combined Taiyang and Yangming disease without diarrhea but with vomiting, Gegen Jia Banxia Tang (Kudzu Decoction with Added Pinellia) is indicated. Add 1.1 liang of Banxia (Rhizoma Pinelliae) to the previous Gegen Tang (Kudzu Decoction), wash ten times with decoction, and cut each piece into four.
Pang Anshi states: With a floating-tight pulse and no sweating, if the decoction fails to resolve the disease, the patient becomes irritable and eyes dim; severely, epistaxis (nosebleed) occurs. If slight epistaxis occurs and the pulse remains floating, Mahuang Tang (Ephedra Decoction) is suitable; if the pulse becomes faint after epistaxis, do not re-administer. In cases of floating pulse with spontaneous sweating, if the decoction fails and Guizhi Tang (Cinnamon Twig Decoction) syndrome persists, severe headache leading to epistaxis occurs. If slight epistaxis occurs and the pulse remains floating, re-administer Guizhi Tang (Cinnamon Twig Decoction); if the pulse becomes faint after epistaxis, do not re-administer.
Three days after Shang Han (Cold Damage), if various decoctions fail to improve the condition, and the pulse rate increases, with Yang Qi still in the meridians and not yet in the viscera, Guizhi Shigao Tang (Guizhi Decoction with Gypsum) is indicated. This formula can replace Guizhi Tang (Cinnamon Twig Decoction) after the Summer Solstice; if 1 liang of Mahuang (Herba Ephedrae) is added, it can replace Mahuang Tang (Ephedra Decoction) and Da Qing Long Tang (Major Blue Green Dragon Decoction).
Ingredients: Shigao (Gypsum Fibrosum) 3 liang; Zhizi (Fructus Gardeniae) 24 pieces; Shengjiang (Rhizoma Zingiberis Recens) 1.5 liang; Guizhi (Ramulus Cinnamomi), Huangqin (Radix Scutellariae), Gancao (Radix Glycyrrhizae) each 1 liang; Shengma (Rhizoma Cimicifugae), Gegen (Radix Puerariae) each 1.5 liang.
Crush, add 5 sheng of water, decoct to 2.5 sheng, strain. Drink one zhan warm, repeat shortly after eating. If sweating occurs, stop subsequent doses.
Pang Anshi states: Diaphoresis primarily uses pungent and sweet herbs. Why add bitter herbs? Pungent and sweet herbs reduce Yin Qi and assist Yang Qi. Since heat is abundant on the exterior, bitter herbs are added to disperse it. The *Su Wen* states: "When heat floods internally, disperse it with bitter herbs."
Gegen Longdan Tang (Kudzu and Gentian Decoction) treats illnesses lasting four to five days without resolution, characterized by toxic heat in the body and red face. It also treats Yang Du (Yang Toxin) and Feng Wen (Wind Warmth).
Ingredients: Gegen (Radix Puerariae) 4 liang fresh (or 2 liang dried); Shengjiang (Rhizoma Zingiberis Recens), Shengma (Rhizoma Cimicifugae), Daqing (Folium Isatidis), Longdan (Radix Gentianae), Guizhi (Ramulus Cinnamomi), Gancao (Radix Glycyrrhizae), Mahuang (Herba Ephedrae), Shaoyao (Radix Paeoniae Rubra) each 0.5 liang; Weirui (Rhizoma Polygonati Odorati) 1 liang; Shigao (Gypsum Fibrosum) 1.5 liang.
Crush, add 4.5 sheng of water, add Mahuang (Herba Ephedrae), boil briefly, skim foam, add other herbs, decoct to 2 sheng, strain. Drink one zhan warm, three times daily and twice nightly. (For Gegen (Radix Puerariae), choose garden-grown varieties with sweet taste and high starch content. Mistakingly using bitter wild Ge Gen often causes vomiting and worsens the condition.)
In epidemic febrile diseases, if six to seven days pass without sweating, with a surging or rapid pulse, red face and eyes, high body heat, irritability, delirious speech, restlessness, and extreme thirst, or if unresolved after five to six days with heat in the Stomach, locked jaw, and inability to speak, this is Huai Shang Han (Damaged Cold Damage), incurable by physicians. If the patient appears dead or spirit is exhausted but the epigastrium is still warm, open the mouth, pour medicine down, and life may be restored. This also treats Yang Du (Yang Toxin) and Maitu Wan (Wheat Slave Pill).
Ingredients: Mahuang (Herba Ephedrae) 3 fen; Fudi Mei (Carbon from Pot Bottom), Huangqin (Radix Scutellariae), Zaodi Mo (Carbon from Hearth Bottom), Liangshang Chen (Dust from Beam), Xiaomai Nu (Wheat Slave), Zaohuang Tu (Yellow Earth from Hearth) each 1 fen; Mangxiao (Natrii Sulfas), Dahuang (Radix et Rhizoma Rhei) each 0.5 liang.
Grind to fine powder, form pills the size of mustard seeds with honey. Dissolve one pill in 3 he of fresh well water and take. For thirst, allow free drinking of cold water. Soon, chills will occur, followed by sweating and recovery. If no sweating occurs after the time it takes to walk five chi, administer another pill per previous method. Stop when recovered; mild diarrhea is expected. Xiaomai Nu (Wheat Slave) is the black powder formed by rubbing unripe black wheat spikes; if unavailable, stir-fry wheat until blackened, remove fire toxicity, and use. This medicine is for patients with extreme thirst and intense restlessness; forcing it on those with mild thirst is harmful. Strong individuals take half an egg-sized pill. (Also treats Wen Nue [Warm Malaria].)
In Shang Han (Cold Damage), if seven or eight doses of diaphoretic decoctions are taken without sweating, death ensues. Use the Wind Strike method of steaming: create external warm air to induce sweating, which has been effective throughout history. Burn firewood on the floor, let it burn for a long time, remove fire, sweep the floor, and sprinkle with water. Use Can Sha (Silkworm Excrement), Tao Ye (Peach Leaves), Bai Ye (Cypress Leaves), Jing Ye (Vitex Leaves), Bran, and Wheat Chaff. Mix and spread on the heated floor, thickness allowing hand insertion. Place a mat on top, have the patient lie on it, and cover warmly. In hot months, use a quilt. Sweating will emerge immediately. Once sweating reaches the soles of the feet, apply Wen Fen (Warm Powder) to stop sweating, then move the patient to bed for recovery. If Can Sha (Silkworm Excrement) is unavailable, bran/chaff suffices. (Wen Fen Recipe: Baizhu (Rhizoma Atractylodis Macrocephalae), Ben Cao (Radix Ligustici Chuanxiong), Baizhi (Radix Angelicae Dahuricae) each 2 liang, powdered, mixed with 12 liang of rice flour. Substitute millet flour if rice flour is unavailable.)
After recovery from Shang Han (Cold Damage), lingering unclear symptoms may persist for ten days to twenty days, with constant mental fog, loss of spirit, erroneous speech, or no chills/fever resembling ghost possession, or morning/evening tidal fever with red cheeks, or chills/fever resembling malaria. This is caused by incomplete diaphoresis leaving residual toxic vapors in the Pericardium Collateral. Zhimu Mahuang Tang (Anemarrhena and Ephedra Decoction) is indicated.
Ingredients: Zhimu (Rhizoma Anemarrhenae) 1.5 liang; Mahuang (Herba Ephedrae) 1 liang; Shaoyao (Radix Paeoniae Rubra), Huangqin (Radix Scutellariae), Gancao (Radix Glycyrrhizae), Guizhi (Ramulus Cinnamomi) each 0.5 liang.
Crush, add 2.5 sheng of water, boil Mahuang (Herba Ephedrae) briefly, skim foam, add other herbs, decoct to 1.3 sheng, strain. Drink one large zhan warm, three doses consecutively within half a day, cover warmly for mild sweating. If irritable and thirsty, give small amounts of water until the Stomach harmonizes and recovery occurs. Finish the prescription if no sweating occurs.
In Taiyang disease, after purgation, if Qi rushes upward and the pulse is floating, diaphoresis may be administered according to the syndrome. Failure to sweat will result in Jie Xiong (Chest Binding). Generally, if the pulse is floating and large, even with constipation and scant urine, diaphoresis can resolve the disease. Failure to sweat when indicated leads to closure of all pores, suffocation, and death.
