Fu Yu (Oppression and Depression) patterns include both deficiency and excess types. One must carefully distinguish the symptoms and pulse. ("Fu" is pronounced as 'fu'. Dry stool accumulation indicates an excess pattern; improper use of emesis or purgation leading to righteous qi depletion indicates a deficiency pattern.) Dry stool accumulation is solely suitable for Chengqi Tang (Cinnamon Twig, Rhubarb, and Mirabilite Decoction). (Zhang Zhongjing stated: When a patient has difficulty urinating, bowel movements that are alternately difficult and easy, occasional slight fever, restlessness preventing sleep due to internal heat oppression, this is caused by dry stool. Chengqi Tang should be primarily used.) Improper use of emesis and purgation leads to extreme stomach deficiency and cold. (Zhang Zhongjing stated: After severe emesis or purgation in Shaoyang disease, the body becomes extremely deficient. If excessive sweating is induced subsequently, the exterior qi becomes oppressed. If water is then given to induce sweating, hiccups will result. This occurs because the stomach is cold, thus causing this condition.) Fire fumigation inducing sweat leads to yellowing of the eyes. (Zhang Zhongjing stated: At the Cun Kou pulse position, the Yang aspect floats while the Yin aspect is soft and weak. If the physician uses fire fumigation to force sweating, the pathogenic heat from the fire exacerbates, steaming the skin and muscles, resulting in jaundice of the body and eyes.) Two Yangs concurrent disease with persistent redness of the face. (Zhang Zhongjing stated: In concurrent disease of the two Yang channels, when Taiyang disease first appears, sweating should be induced initially. If the sweat is not thorough, the pathogen transfers to Yangming. Continued slight sweating occurs. If the face remains persistently red, it indicates Yang qi oppression at the exterior, which should be resolved through methods such as fumigation.) A surging and large pulse indicates abundant Ying Qi (Nutritive Qi). (Zhang Zhongjing stated: A surging and large pulse at the Cun Kou position indicates abundant Ying Qi. Abundant Ying Qi leads to Yang excess, causing oppression that prevents vocalization.) How can errors occur if treatment follows the channel pathways?
Why does Shanghan (Cold Damage) generate Jing Ti (Fright and Palpitations)? It arises from improper use of emesis, purgation, warm needle therapy, or fire stimulation. (Due to improper emesis/purgation, or improper warm needles, or improper fire coercion.) Purging leading to delirium can be treated with Muli Tang (Oyster Shell Decoction). (Zhang Zhongjing stated: After eight or nine days of Shanghan, if purgation is improperly used, resulting in chest fullness, mental restlessness and fright, difficulty urinating, delirium, whole-body pain, and inability to turn over, Chaihu Jia Longgu Muli Tang (Bupleurum, Dragon Bone, and Oyster Shell Decoction) should be used.) Indiscriminate use of warm needles contradicts medical principles. (Zhang Zhongjing stated: In Taiyang Shanghan, adding warm needle therapy will inevitably cause fright.) Feng Wen (Wind-Warmth) injured by fire often leads to convulsions. ("Yi" is pronounced as 'zi yong qie', also written as "Yi". Zhang Zhongjing stated: If Feng Wen is injured by fire, mild cases present with slight yellowing of the complexion; severe cases resemble epilepsy with intermittent convulsions, resembling fire fumigation. One error may prolong life, but a second error accelerates death.) Yangming disease injured by fire causes slight sweating on the forehead. (Zhang Zhongjing stated: In Yangming disease subjected to fire therapy, there is slight sweating on the forehead, fever, sweating without aversion to cold. If warm needles are added, there will inevitably be fear, fright, irritability, and insomnia.) A floating pulse subjected to fire coercion必然 leads to Wang Yang (Loss of Yang Qi). (Zhang Zhongjing stated: In Shanghan with a floating pulse, if the physician uses fire coercion to force sweating, leading to loss of Yang Qi, there will inevitably be fright, mania, restlessness in lying down and rising up. Guizhi Qu Shaoyao Jia Shuqi Longgu Muli Jiuni Tang (Cinnamon Twig Decoction without Peony, plus Chinese Dodder Herb, Dragon Bone, and Oyster Shell, for Saving from Emergency) should be used.) These three conditions differ but stem from similar iatrogenic errors. Shaoyang Wind Strike results in deafness. Improper emesis and purgation lead to palpitations and frequent fright. (Zhang Zhongjing stated: In Shaoyang Wind Strike, both ears cannot hear, eyes are red, chest is full but not vexed. Emesis and purgation are contraindicated. If improperly used, palpitations and fright will result.)
