The cun (distal) and chi (proximal) pulses are both floating, indicating disease in the Taiyang (Greater Yang) channel. Within one to two days, the illness presents as usual. The meridian connects to the Fengfu acupoint, causing pain in the head and neck, and stiffness in the waist and spine. (Zhang Zhongjing stated: When the cun and chi pulses are both floating, it is disease of the Taiyang channel, which should manifest within one or two days. Because it connects superiorly to the Fengfu point, headache, stiff neck, and stiffness of the lumbar spine occur.)
A long pulse indicates disease in the Yangming (Bright Yang) channel. Manifesting within two to three days, this follows the expected progression. The meridian runs alongside the nose and connects to the eyes, resulting in eye pain, nasal dryness, and restless sleep. (Zhang Zhongjing stated: When the cun and chi pulses are both long, it is disease of the Yangming channel, which should manifest within two or three days. Because the vessel runs alongside the nose and connects to the eyes, fever, eye pain, nasal dryness, and inability to lie down occur.)
The Shaoyang (Lesser Yang) channel passes through the ear; the pulse is wiry, accompanied by hypochondriac pain and deafness. Before the fourth day, the pathogen remains in the Fu organs; inducing sweating will resolve the condition easily. (Zhang Zhongjing stated: When the cun and chi pulses are both wiry, it is disease of the Shaoyang channel, which should manifest within three or four days. Because the vessel courses along the hypochondrium and connects to the ear, chest and hypochondriac pain and deafness occur. Since these three channels are diseased but have not entered the Zang organs, sweating therapy is applicable.)
By the fourth or fifth day, disease reaches the Taiyin (Greater Yin) channel. The Taiyin pulse is fine and deep. The meridian distributes in the stomach and connects to the throat, causing abdominal fullness, dry throat, and unbearable abdominal pain due to Spleen dysfunction. (Zhang Zhongjing stated: When the cun and chi pulses are both fine and deep, it is disease of the Taiyin channel, which should manifest within four or five days. Because the vessel distributes in the stomach and connects to the throat, abdominal fullness and dry throat occur.)
Transmission to the Shaoyin (Lesser Yin) channel presents with a deep and tight pulse. The meridian penetrates the Kidney, connects to the Lung system and the root of the tongue, causing dry mouth, dry tongue, and unceasing thirst. By the fifth or sixth day, the disease pattern is established. (When the cun and chi pulses are both deep, it is disease of the Shaoyin channel, which should manifest within five or six days. Because the vessel penetrates the Kidney, connects to the Lung, and ties to the root of the tongue, dry mouth, dry tongue, and thirst occur.)
By the seventh or eighth day, disease reaches the Jueyin (Terminal Yin) channel, characterized by vexation, fullness, and retraction of the testicles, causing worry and alarm. Disease in the Three Yin channels has entered the Zang organs; purgative therapy is appropriate to restore harmony. (Zhang Zhongjing stated: When the cun and chi pulses are both slightly slow and relaxed, it is disease of the Jueyin channel, which should manifest within six or seven days. Because the vessel courses along the genitalia and connects to the Liver, vexation, fullness, and retraction of the testicles occur. Once the Three Yin channels have entered the Zang organs, purgation can cure the disease.)
After the transmission through the Six Channels is complete and pervasive, the Earth element no longer receives pathogenic attack, and the pulse becomes slow. Water and Fire interact, Qi harmonizes, resembling rain from clouds; recovery occurs through profuse sweating. (If transmission reaches Jueyin and the pulse is slow, the Spleen Earth is no longer overcontrolled, allowing Water to ascend and Fire to descend. Qi harmonizes, leading to resolution through profuse sweating.)
