This is Shengjiang Xiexin Decoction (Ginger Bitter-Heart Decoction) with Ren Shen (Codonopsis Root) and Sheng Jiang (Fresh Ginger Rhizome) removed, and Gan Cao (Licorice Root) added in the amount of one liang.
Gan Cao (4 liang, honey-fried), Huang Qin (Scutellaria Root), Gan Jiang (Dried Ginger Rhizome) (each 3 liang), Ban Xia (Pinellia Tuber) (half sheng, washed), Huang Lian (Coptis Rhizome) (1 liang), Da Zao (Jujube Fruit) (12 pieces, split). The above six herbs,
Boiled in ten dou of water to obtain six sheng of decoction, filter out the dregs, then boil again to obtain three sheng. Take one sheng warm, three times a day.
In cases of Shanghan (Cold Damage) or Zhongfeng (Wind Strike), if the physician erroneously applies purgation, the patient may suffer from diarrhea dozens of times a day, undigested food, borborygmus (rumbling in the abdomen), epigastric fullness and hardness, dry heaving, and irritability with inability to find peace.
If the physician sees the epigastric fullness and assumes the pathogen remains, applying purgation again will exacerbate the fullness. This is not due to accumulated heat, but rather deficiency in the Stomach. Due to two erroneous purgations, Gan Cao (Licorice Root) is used to tonify the Stomach Qi, thereby resolving the fullness naturally. Empirical physicians consider Gan Cao (Licorice Root) as obstructing the middle jiao and thus contraindicated for fullness and vomiting; this stems from a lack of understanding regarding the principles of excess and deficiency. When rebellious Qi ascends, it causes hardness; Gancao Xiexin Decoction (Licorice Bitter-Heart Decoction) should be administered.
Fanwei (Regurgitation/Reverse Vomiting) can be treated by modifying this formula.
Ban Xia (Pinellia Tuber) (half sheng, washed), Huang Qin (Scutellaria Root), Gan Jiang (Dried Ginger Rhizome), Gan Cao (Licorice Root) (honey-fried), Ren Shen (Codonopsis Root) (each 3 liang), Huang Lian (Coptis Rhizome) (1 liang), Da Zao (Jujube Fruit) (12 pieces, split). The above seven herbs,
Boiled in ten dou of water to obtain six sheng of decoction, filter out the dregs, then boil again to obtain three sheng. Take one sheng warm, three times a day.
After five or six days of Shanghan (Cold Damage), if there is vomiting accompanied by fever, the Zheng (syndrome) of Chaihu Tang (Bupleurum Decoction) is present. If other drugs were erroneously used for purgation, and the Zheng (syndrome) of Chaihu Tang (Bupleurum Decoction) persists, administer Chaihu Tang (Bupleurum Decoction) again. Although purgation has been applied, it does not constitute a complication (Ni). The patient will inevitably experience shaking and chills, followed by fever and sweating, leading to resolution. If the original symptoms persist, use the original formula immediately. If there is epigastric fullness without pain, this is termed Pi (Glomus/Fullness). The emphasis on "without pain" precisely defines the Pi syndrome. In this case, Chaihu Tang (Bupleurum Decoction) is not indicated; Banxia Xiexin Decoction (Pinellia Bitter-Heart Decoction) is appropriate. The herbs in the above three Xiexin Decoctions are largely derived from Chaihu Tang (Bupleurum Decoction); therefore, the syndromes they treat are mostly similar to those of Chaihu Tang (Bupleurum Decoction), with the addition of herbs that address deficiency and resolve Pi (Glomus/Fullness).
