Cheng Menxue's commentary: This clause is indispensable. Asasada Sohaku (Qian Tian Zongbo) did not cover this point, so I have supplemented it. Exogenous cold and endogenous heat do not go beyond these two categories, but the first clause should be regarded as the major syndrome.
"Deep jue indicates deep heat; jue should be treated with purgation. If sweating is induced in error, the mouth will surely suffer from sores and redness." This refers to the heat jue syndrome. "In all yin reversals, the pulse arrives following the trend, but upon pressing, it beats vigorously and strongly." This discusses the pulse of heat jue.
False cold signs include: cold reversal of the hands and feet (i.e., jue), aversion to cold, or shivering. This occurs because heat is extreme internally while cold manifests externally; extreme heat transforms into a cold-like appearance.
True heat evidence includes: a deep, hidden, and slippery pulse; sweat on the head; although the hands are cold, the fingernails occasionally become warm (according to Zhu Gong); a deep-slippery forceful pulse, or a slippery-solid forceful pulse; dry tongue coating (according to Hata Yuiwa).
Symptoms: Constipation, reddish urine, or black stool (according to Zhu Gong); loud voice and coarse breath, strong physical constitution, scorched tongue coating, dry tongue surface with prickles, sedge-color, thirst craving for cold drinks, foul breath, irritability, painful and reddish urination, constipation, or purging of clear water that is greenish-black, yet dry feces remain in the abdomen accompanied by extremely foul flatus (according to Hata Yuiwa).
Treatment: If there is no internal excess, presenting with abdominal fullness, heaviness of the body, delirium, urinary incontinence, dry mouth, dry tongue, and severe restlessness and thirst, use Baihu Tang (White Tiger Decoction).
If there is internal excess with dry feces, presenting with abdominal fullness and wheezing, delirium as if seeing ghosts, or purging of pure clear water with fullness and pain in the epigastrium, use Dachengqi Tang (Major Order the Qi Decoction). For milder cases, Xiaochengqi Tang (Minor Order the Qi Decoction) or Tiaoweichengqi Tang (Regulate the Stomach and Order the Qi Decoction) may be selected. If there are alternating chills and fever, use Daichaihu Tang (Major Bupleurum Decoction) to purge.
Cheng Menxue's commentary: It should be stated as Xiao Chaihu Tang (Minor Bupleurum Decoction) plus Rhubarb.
