The lips and mouth are the orifices governing speech and eating. If the lip is injured, broken, or missing, it should first be sutured with oiled thread, then covered with Zhixue Shengji San (Hemostatic and Flesh-Regenerating Powder), followed by applying Yuhong Gao (Jade Red Ointment). Internally, Jianpi (Strengthening the Spleen) herbs should be taken, and medications applied to the tongue frequently changed. The patient should lie supine, with thin gruel poured into the throat, ensuring the tongue remains immobile, which facilitates healing. There is also a point called Yutang, located in the upper part of the oral cavity, also known as Shanghan; its orifice corresponds to the Hangsang (nasopharynx). If the left or right sides are pierced and injured, only swelling occurs. If the central orifice is injured, connecting upwards to the brain, it inevitably damages the curled fleshy structures within the nasal cavity (turbinates), or invades the hollow cavities, leading to unceasing bleeding, causing both nose and eyes to swell, the face to turn dark purple, accompanied by mental fatigue, dizziness, and pain radiating to the brain marrow. If the epiglottis and the upper transverse bone are injured, severe cases may result in inability to speak, pain extending to the chest diaphragm, coma, and heaviness, indicating a critical condition. Immediately, Nifen (Calomel) and Bingpian (Borneol) should be finely ground and insufflated onto the affected area to stop bleeding. Jiegu Zijin Dan (Bone-Knitting Purple Gold Pill) should be administered. Additionally, decoctions made from Xiehuang (Crab Yellow) and Xuexie (Dragon's Blood) should be used for gargling two or thirty times daily. If swallowing food and drink is difficult, items such as Shishuang (Persimmon Frost), Yulu Shuang (Jade Dew Frost), milk, cream, or flour-based gruels can be used. With proper cooling and moistening care, recovery is possible.
The ear is the orifice governing hearing. The opening of the ear is called Bi, and the earlobe is called Guo. It may be severed or knocked off by cuts or falls, or it may detach downwards while adhering upwards, or detach upwards while adhering downwards; these require binding and fixing Lingming (a term referring to the ear structure or related acupoint in this context). If the injury involves swelling, pain, and bleeding, Jiegu Zijin Dan (Bone-Knitting Purple Gold Pill) should be taken. Externally, Baxian Xiaoyao Tang (Eight Immortals Free-and-Easy Decoction) should be used for fumigation and washing, and Hunyuan Gao (Primordial Unity Ointment) should be applied. If the injury is severe, affecting the brain marrow internally, and damaging Lingming, resulting in lethargy and unconsciousness, especially if the patient has always suffered from Qi and Blood deficiency, it is untreatable. The Dige bone, also known as the Ké (chin), commonly called the lower jaw bone, is where the two Che bones (mandibular angles) intersect, and it supports the teeth above. Injury here leads to inability to eat, closed eyes, confusion, heart heat, and mental disorder. Dashen Xiao Huo Luo Dan (Great Efficacy Meridian-Activating Pill) should be taken, followed by dissolving Renshen Zijin Dan (Ginseng Purple Gold Pill) in the mouth, applying Guchi San (Tooth-Firming Powder), and externally sticking Wanling Gao (Myriad Spirits Ointment). Wind and cold foods should be avoided. If the teeth are loosened, roots of Tribulus terrestris (Jili) should be burnt until carbonized, ground into powder, and rubbed on them; alternatively, applying Guchi San (Tooth-Firming Powder) is also effective.
