People only know that the three Yang meridians ascend along the head, causing headache (Tóutòng), but they do not know that the Jueyin Meridian of Foot-Shaoyang (Zu Shaoyin) and the Du Meridian intersect at the vertex, nor that the marrow of the Shaoyin Meridian connects to the brain. People only know that wind-cold (Fēnghán), fire-heat (Huǒrè) in the head cause headache, but they do not know that the syndromes of the six meridians of the foot can ascend reversely to the head, resulting in "Jue Headache" (Jué Tóutòng). When the qi of the six foot meridians rebels upward to the head causing headache, one must also understand that cold pathogen invading the brain can transmit to the Jueyin and Shaoyin meridians, manifesting as a Yin syndrome (Yīnzhèng). A true severe head disease involves intense headache with pain throughout the brain, cold hands and feet extending to the joints; this is a fatal condition and cannot be treated.
It is commonly believed that heart pain (Xīntòng) refers to pain in the epigastrium directly behind the heart, but people are unaware of "Jue Heart Pain" (Jué Xīntòng). Jue Heart Pain is caused by the rebellious qi of the four zang organs invading the area below the heart, leading to pain. In heart pain that controls or radiates to the back, accompanied by frequent belching: if it feels as if the heart is being touched from behind, and the patient assumes a stooped posture, this is Kidney Heart Pain (Shèn Xīntòng); if the heart pain feels like being pierced by an awl or needle, and is severe, this is Spleen Heart Pain (Pí Xīntòng); if the complexion is dark greenish-gray resembling a dying state, and the patient cannot take deep sighs all day, this is Liver Heart Pain (Gān Xīntòng); if lying down or resting relieves the pain, but movement exacerbates it, with no change in complexion, this is Lung Heart Pain (Fèi Xīntòng). If the pain is in the epigastrium behind the heart, radiating upward to support both hypochondria, with obstruction in the diaphragm and throat, and inability to eat, these conditions should be carefully examined via pulse and syndrome differentiation. Treatment should follow the meridian pathways, distinguishing between cold, heat, deficiency, and excess, ensuring efficacy. However, True Heart Pain (Zhēn Xīntòng) presents with cyanosis of hands and feet extending to the joints, severe heart pain, where onset in the morning leads to death in the evening, and onset in the evening leads to death in the morning.
The abdomen is the domain of the Taiyin (Spleen) and the outer wall of the intestines and stomach. Syndromes of fullness and pain have distinctions between affecting the fu and zang organs, and differences in severity and urgency. For instance, sudden severe colic with a blue-green face and cold limbs indicates pathogen directly invading the yin aspect; urgent needling to drain the pathogen is required. Sudden vomiting and diarrhea indicate pathogen damaging the Yangming Meridian, termed Huoluan (Cholera-like disorder). If fever and headache appear, it indicates recovery. If there is cold extremities and absent pulse, Lizhong Wan (Regulate the Middle Pill) should be administered urgently, with Fuzi (Prepared Aconite Root) added for severe cases. If the pulse returns and hands/feet become warm within a short time, the patient survives; otherwise, death occurs due to extreme pathogen and exhaustion of Stomach Qi. Abdominal pain with diarrhea but ability to eat indicates pathogen entering the intestines; cessation of diarrhea signifies recovery. These are direct invasions of the interior with varying degrees of zang-fu involvement. Additionally, pathogen transmitting to the five zang organs causes abdominal pain. Initially at the skin and interstices, when righteous qi fails to resist, the pathogen diffuses into the blood vessels, transmits inward, and large pathogen enters the zang organs, causing abdominal pain and profuse discharge. This is a syndrome of pathogenic overflow and deterioration, leading to death within twenty days; even if Bian Que were reborn, he could not save them. Such cases are often neglected due to carelessness, leading to resentment from patients' families.
