一小儿喉痹，因膏粱积热，或禀赋有热，或乳母七情之火，饮食之毒，当分其邪蓄表里，与症之轻重，经之所主而
治之。若左腮色青赤者，肝胆经风热也，用柴胡栀子散。右腮色赤者，肺经有热也，用泻白散。额间色赤者，心与小肠
经热也，用导赤散。若兼青色，风热相搏也，用加味逍遥散。鼻间色黄，脾胃经有热也，用泻黄散。若兼青色，木乘土
位也，用加味逍遥散。兼赤色心传土位也，用柴胡栀子散。颏间色赤，肾经有热也，用地黄丸。凡此积热内蕴，二便不
通者，当疏利之；风邪外客而发寒热者，当发散之；外感风邪，大便闭结，烦渴痰盛者，当内疏外解。若因乳母膏粱积
热者，母服东垣清胃散。若因乳母恚怒肝火者，母服加味逍遥散。禀赋阴虚者，儿服地黄丸。大概当用轻和之剂，以治
其本。切不可用峻利之药，以伤真气也。
一小儿喉间肿痛，惊悸饮水，服惊风降火之药益甚，仍欲攻风痰。余曰∶惊悸饮水，心经虚症也。盖胃为五脏之本，
先用五味异功散以补胃，加桔梗、甘草以消毒，诸症顿退，后用牛蒡子汤加柴胡而愈。
一小儿喉间肿痛，左腮色青赤，此心肝二经之热也，用柴胡清肝散而愈。后因惊，服至宝丹，吐痰发搐，手足指
冷，此肝木虚而肺金乘之，用补中益气汤以补脾肺，六味地黄丸以滋肝肾而愈。
一小儿发热饮冷，大便黄色，手足并热，不能吮乳，视口内无患，扪其喉间则哭，此喉内作痛，乃脾胃实热也，用
泻黄、清胃二散各一剂，母子并服而愈。后因乳母饮酒，儿躁不安，口内流涎，仍用前二散而愈。
一小儿喉间肿痛，口角流涎，手足并热，用泻黄、清胃二散，母子服之而愈。后因母大怒，儿憎寒发热，仍复流涎，
用柴胡清肝散加漏芦，母子服之而愈。
一小儿喉间肿痛，发热咳嗽，大便秘结，此肺与大肠有热也，先用牛蒡子汤加硝黄一服，大便随通，乃去硝黄，再
剂顿愈。审其母有肝火发热，用柴胡清肝散，母子并服而愈。
一小儿嗜膏粱之味，喉间肿痛，痰涎壅盛，服巴豆丸，前症益甚，口鼻出血，唇舌生疮，大便不实，余用犀角地黄
汤，解膏粱之热，用东垣安胃散，解巴豆之毒，又用甘桔汤而愈。
一小儿喉肿作渴，大便干实，右腮赤色，此肺与大肠经实热也，用柴胡饮子，一服而愈。后因饮食停滞，服峻厉之
药，喉间仍肿，腹中胀痛，此脾气复伤也，用异功散加升麻、当归而痊。
一小儿因母忿怒患前症，兼切牙呵欠，余谓肝经虚热之症，子用桔梗汤加柴胡、山栀、牛蒡子，母服加味逍遥散而
愈。
一小儿肌体瘦弱，嗜土炭煤灰，后鼻间不利，恪服清热之剂，肌体愈瘦，食少热甚，善惊善怒，小便良久变白，鼻
中出息肉二寸许，耳下颈间结小核隐于筋肉之间，余谓肝脾虚羸之变症，不信，乃内清肺火，外用腐蚀，喉间亦腐，余
先用五味异功散加升麻、柴胡、芜荑为主；更用四味肥儿丸
为佐，脾气渐健，夕用九味芦荟丸为主，以五味异功散为佐而愈。
一女子六岁，喉间肿痛，鼻中息肉，寒热往来，小便频数，良久变白，此肝疳之症，先用加味逍遥散加炒黑焦龙胆
草，热痒渐退，乃去龙胆草，佐以四味肥儿丸而愈。
一女子七岁，鼻生息肉，搽攻毒之药，成疮肿痛，外用黄连、甘草、黄柏末敷之，以解热毒，更以加味逍遥散清肝
火，佐以四味肥儿丸而愈。
一女子鼻中及下部，常出息肉，屡用毒药蚀之，各挺出一条三寸许，先与龙胆草汤为主，以加味逍遥散为佐而愈。
一小儿额间赤，足心热，喉中常痛，服清胃败毒之药，余谓禀肾水不足，而心火炽甚也，当用地黄丸，壮水之主以
制阳光。不悟。口舌赤烈，小便如淋而殁。
一女子十四岁，患前症，杂用清热败毒等药，前症益甚，患阴挺。详见下疳疮。
牛蒡子汤 治风热上壅，咽喉肿痛，或生乳蛾。
牛蒡子（炒杵） 玄参 升麻 桔梗（炒） 犀角（镑） 黄芩 木通 甘草（各等分）
上每服一二钱，水煎服。
拔萃桔梗汤 治热肿喉痹。
桔梗（炒） 甘草（炒） 连翘 栀子（炒） 薄荷 黄芩（各等分）
上为末，每服一二钱，水煎服。
柴胡饮 解肌热、积热，或汗后余热，脉洪实弦数，大便坚实者。
黄芩（七分） 甘草（四分） 大黄（八分） 芍药（炒七分） 柴胡 人参（各五分） 当归（一钱）
上每服一钱，姜水煎。
东垣人参安胃散 治脾胃虚热，口舌生疮，或伤热乳食，呕吐泻痢。
人参（一钱） 黄 （炒，二钱） 生甘草 炙甘草（各五分） 白芍药（酒炒，七分） 白茯苓（四分）
陈皮（三分） 黄连（炒，二分）
上为末，每服二钱，水煎。
三因玉钥匙 治风热喉闭，及缠喉风。
焰硝（一两半） 鹏硝（半两） 片脑（一字） 白僵蚕（一钱）
上研匀，用半钱吹入喉中，立愈。
九味芦荟丸 治肝经积热，咽喉口舌生疮；或牙龈蚀烂，两目生翳，耳中出水；或肝积瘰 ，下疳阴肿；或茎
出白津，拗中结核；或小水良久变白，大便不调，肢体消瘦等症。
胡黄连 宣黄连 芦荟 木香 白芜荑（炒） 青皮 白雷丸 鹤虱草（各一两） 麝香（三钱）
上各另为末，米糊丸，麻子大。每服半钱，空心米汤下，仍量儿大小用。
甘桔汤 治风热上攻，咽喉疼痛，及喉痹妨闷。
苦梗（一两） 甘草（炒，二两）
上每服二钱，水煎。
济生犀角地黄汤 治膏粱积毒，脾胃有热，咽喉肿痛，或口舌生疮。
犀角 牡丹皮（各一两） 生地黄（八钱） 赤芍药（七钱）
上每服一二钱，水煎。
五味异功散（方见败毒之药）
六味地黄丸（方见作渴不止）

（附：五脏虚损羸弱传变至喉间导致内部溃烂，或鼻中垂出息肉，或鼻外患疮）
有一个小儿患喉痹，是因为过食肥甘厚味导致积热，或者先天禀赋有热，或者是乳母因七情郁结产生的火气、饮食中的毒素所致。治疗时应当分辨邪气蓄积在表还是在里，根据症状的轻重，依据经络的主治原则进行治疗。如果左腮颜色青赤，是肝胆经的风热，用柴胡栀子散。右腮颜色赤红，是肺经有热，用泻白散。额间颜色赤红，是心与小肠经有热，用导赤散。如果兼见青色，是风热相搏结，用加味逍遥散。鼻间颜色黄，是脾胃经有热，用泻黄散。如果兼见青色，是肝木乘克脾土，用加味逍遥散。如果兼见赤色，是心火传于脾土，用柴胡栀子散。下巴颏间颜色赤红，是肾经有热，用地黄丸。凡是这种积热内蕴，大小便不通畅的，应当疏通利导；如果是风邪外袭引发寒热的，应当发散解表；如果是外感风邪，大便闭结，烦躁口渴痰多者，应当内疏外解。如果是因为乳母过食肥甘厚味导致积热，母亲服用东垣清胃散。如果是因为乳母愤怒导致肝火旺盛，母亲服用加味逍遥散。如果是先天禀赋阴虚的，孩子服用地黄丸。大概应当使用轻缓平和的药剂来治本。切不可使用峻猛攻利的药物，以免损伤真气。
另一个小儿喉间肿痛，惊悸想喝水，服用惊风降火的药物后病情反而加重，仍然想要攻逐风痰。我说：惊悸想喝水，是心经虚证的表现。因为胃是五脏的根本，先用五味异功散来补益胃气，加入桔梗、甘草来解毒，各种症状顿时消退，后来再用牛蒡子汤加入柴胡而痊愈。
又一个小儿喉间肿痛，左腮颜色青赤，这是心肝二经有热，用柴胡清肝散而痊愈。后来因为受惊，服用了至宝丹，出现呕吐痰涎、抽搐、手足指发冷的症状，这是肝木虚弱而肺金乘虚克制它，用补中益气汤来补益脾肺，六味地黄丸来滋养肝肾而痊愈。
又一个小儿发热喜欢喝冷水，大便黄色，手足都热，不能吮吸乳汁，查看口腔内部没有病变，触摸其喉间则哭闹，这是喉内作痛，属于脾胃实热，用泻黄散、清胃散各一剂，母子同时服用而痊愈。后来因为乳母饮酒，孩子躁动不安，口内流涎，仍然使用前两种散剂而痊愈。
又一个小儿喉间肿痛，口角流涎，手足都热，用泻黄散、清胃散，母子同时服用而痊愈。后来因为母亲大怒，孩子憎恶寒冷并发热，再次流涎，用柴胡清肝散加入漏芦，母子同时服用而痊愈。
又一个小儿喉间肿痛，发热咳嗽，大便秘结，这是肺与大肠有热，先用牛蒡子汤加入芒硝、大黄服用一次，大便随即通畅，然后去掉芒硝、大黄，再服两剂顿时痊愈。诊察发现其母亲有肝火发热，用柴胡清肝散，母子同时服用而痊愈。
又一个小儿嗜好肥甘厚味，喉间肿痛，痰涎壅盛，服用了巴豆丸，前面的症状更加严重，口鼻出血，嘴唇舌头生疮，大便不成形，我用犀角地黄汤，解除肥甘厚味之热，用东垣安胃散，解除巴豆之毒，又用甘桔汤而痊愈。
又一个小儿喉肿口渴，大便干硬结实，右腮赤红，这是肺与大肠经实热，用柴胡饮子，服用一次而痊愈。后来因为饮食停滞，服用了峻猛的药物，喉间仍然肿胀，腹中胀痛，这是脾气再次受伤，用异功散加入升麻、当归而痊愈。
又一个小儿因为母亲愤怒患上前面的病症，兼有咬牙呵欠，我认为这是肝经虚热的症状，孩子用桔梗汤加入柴胡、山栀、牛蒡子，母亲服用加味逍遥散而痊愈。
又一个小儿肌体瘦弱，嗜吃泥土、炭灰、煤灰，后来鼻间不利，严格服用清热药剂，肌体更加消瘦，吃得少且发热严重，容易受惊易怒，小便很久才变白，鼻中长出约两寸长的息肉，耳下颈间结有小核隐藏在筋肉之间，我认为这是肝脾虚损羸弱的变症，家属不信，于是内服清肺火药，外用腐蚀药，喉间也腐烂了。我先用五味异功散加入升麻、柴胡、芜荑为主药；更用四味肥儿丸作为佐药，脾气逐渐强健，晚上用九味芦荟丸为主药，以五味异功散为佐药而痊愈。
一个六岁女子，喉间肿痛，鼻中息肉，寒热往来，小便频数，很久才变白，这是肝疳的症状，先用加味逍遥散加入炒黑焦龙胆草，热痒渐渐消退，然后去掉龙胆草，佐以四味肥儿丸而痊愈。
一个七岁女子，鼻生息肉，搽抹攻毒药物，形成疮疡肿痛，外用黄连、甘草、黄柏末敷贴，以解热毒，更用加味逍遥散清肝火，佐以四味肥儿丸而痊愈。
一个女子鼻中及下部，经常长出息肉，屡次用毒药腐蚀，各挺出一条约三寸长，先用龙胆草汤为主药，以加味逍遥散为佐药而痊愈。
一个小儿额间赤红，足心热，喉中常痛，服用清胃败毒的药物，我认为这是禀赋肾水不足，而心火炽盛，应当用地黄丸，壮水之主以制约阳光（亢盛的阳热）。家属不听从。结果口舌赤红灼烈，小便如淋病状而死。
一个十四岁女子，患前面的病症，杂用清热败毒等药物，前面的症状更加严重，患上了阴挺（子宫脱垂）。详见下疳疮章节。
牛蒡子汤 治疗风热上壅，咽喉肿痛，或生乳蛾（扁桃体肿大）。

以上每服一二钱，水煎服。

(Attached: Transformation of deficiency and emaciation of the five zang organs leading to internal ulceration in the throat, or polyps protruding from the nose, or sores occurring outside the nose.)
A child suffered from throat obstruction (Hou Bi), caused by accumulation of heat due to excessive consumption of rich and sweet foods, or innate constitutional heat, or fire generated by the wet nurse's emotional disturbances and toxins from diet. Treatment should distinguish whether the pathogenic factor is accumulated in the exterior or interior, and treat according to the severity of symptoms and the governing principles of the meridians. If the left cheek is blue-red, it indicates wind-heat in the Liver and Gallbladder meridians; use Chaihu Zhizi San (Bupleurum and Gardenia Powder). If the right cheek is red, it indicates heat in the Lung meridian; use Xiebai San (Drain White Powder). If the forehead is red, it indicates heat in the Heart and Small Intestine meridians; use Daochi San (Direct Redness Powder). If accompanied by a bluish color, it indicates wind-heat entangling; use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder). If the nose area is yellow, it indicates heat in the Spleen and Stomach meridians; use Xiehuang San (Drain Yellow Powder). If accompanied by a bluish color, it indicates Wood overacting on Earth (Liver attacking Spleen); use Jiawei Xiaoyao San. If accompanied by a red color, it indicates Heart Fire transmitting to Earth (Spleen); use Chaihu Zhizi San. If the chin is red, it indicates heat in the Kidney meridian; use Dihuang Wan (Rehmannia Pill). In cases where accumulation of heat is internally stored with unsmooth urination and defecation, one should promote drainage and purgation; if external wind pathogen causes chills and fever, one should disperse and release the exterior; if external wind pathogen causes constipation, irritability, thirst, and profuse phlegm, one should internally purge and externally resolve. If caused by the wet nurse's accumulation of heat from rich foods, the mother should take Dongyuan Qingwei San (Dongyuan's Stomach-Clearing Powder). If caused by the wet nurse's anger and Liver Fire, the mother should take Jiawei Xiaoyao San. For children with innate Yin deficiency, Dihuang Wan should be administered. Generally, mild and harmonizing formulas should be used to treat the root cause. Harsh purgative drugs must not be used to avoid damaging true Qi.
Another child had swelling and pain in the throat, palpitations, and desire to drink water. After taking drugs for convulsions and clearing fire, the condition worsened, and there was still a desire to attack wind-phlegm. I said: Palpitations and desire to drink water are signs of Heart meridian deficiency. Since the Stomach is the foundation of the five zang organs, first use Wuwei Yigong San (Five-Ingredient Extraordinary Efficacy Powder) to tonify the Stomach, add Jiegeng (Platycodon Root) and Gancao (Licorice Root) to detoxify, after which all symptoms subsided. Later, Niubangzi Tang (Arctium Decoction) with added Chaihu (Bupleurum Root) cured the case.
Yet another child had swelling and pain in the throat, with a blue-red left cheek, indicating heat in the Heart and Liver meridians. It was cured using Chaihu Qinggan San (Bupleurum Liver-Clearing Powder). Later, due to fright, he took Zhibao Dan (Precious Treasure Pill), resulting in vomiting phlegm, convulsions, and cold fingers/toes. This indicated deficient Liver Wood being overcome by Lung Metal. He was cured using Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) to tonify Spleen and Lung, and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to nourish Liver and Kidney.
Another child had fever, preference for cold drinks, yellow stool, and hot hands and feet, unable to suckle milk. Inspection showed no lesions in the oral cavity, but crying upon touching the throat indicated internal throat pain due to excess heat in the Spleen and Stomach. One dose each of Xiehuang San and Qingwei San was given to both mother and child, resulting in recovery. Later, after the wet nurse drank alcohol, the child became restless and salivated excessively. The same two powders were used again, resulting in recovery.
Another child had throat swelling and pain, drooling at the corners of the mouth, and hot hands and feet. Xiehuang San and Qingwei San were given to both mother and child, resulting in recovery. Later, after the mother became very angry, the child developed aversion to cold and fever, with recurrent drooling. Chaihu Qinggan San with added Loulu (Euphorbia Fissuralis Root) was given to both mother and child, resulting in recovery.
Another child had throat swelling and pain, fever, cough, and constipation, indicating heat in the Lung and Large Intestine. First, Niubangzi Tang with Mangxiao (Mirabilite) and Dahuang (Rhubarb Root) was taken once, leading to immediate bowel movement. Mangxiao and Dahuang were then removed, and two more doses resulted in complete recovery. Examination revealed the mother had Liver Fire causing fever. Chaihu Qinggan San was given to both mother and child, resulting in recovery.
Another child who indulged in rich foods had throat swelling and pain, with abundant phlegm and saliva. After taking Badou Wan (Croton Seed Pill), previous symptoms worsened, with bleeding from mouth and nose, sores on lips and tongue, and loose stools. I used Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) to clear heat from rich foods, Dongyuan Anwei San (Dongyuan's Stomach-Anchoring Powder) to detoxify Croton seeds, and Ganjie Tang (Licorice and Platycodon Decoction), resulting in recovery.
Another child had throat swelling, thirst, dry and hard stool, and a red right cheek, indicating excess heat in the Lung and Large Intestine meridians. Chaihu Yin (Bupleurum Drink) was taken once, resulting in recovery. Later, due to food stagnation and harsh medication, throat swelling persisted with abdominal distension and pain, indicating renewed injury to Spleen Qi. Yigong San with Shengma (Cimicifuga Rhizome) and Danggui (Angelica Sinensis Root) was used, resulting in recovery.
Another child, whose mother was angry, suffered from the aforementioned symptoms, along with teeth grinding and yawning. I diagnosed this as deficient heat in the Liver meridian. The child was treated with Jiegeng Tang plus Chaihu, Shanzhi (Gardenia Fruit), and Niubangzi, while the mother took Jiawei Xiaoyao San, resulting in recovery.
Another child was emaciated and craved eating soil, charcoal, and coal ash. Later, nasal passage became obstructed. Strict use of heat-clearing agents made the child thinner, with poor appetite, severe fever, susceptibility to fright, irritability, and urine turning white only after a long time. Polyps about two cun long grew from the nose, and small nodules appeared under the ears and neck hidden in muscles and tendons. I diagnosed this as a transformation symptom of Liver and Spleen deficiency and emaciation. The family did not believe me and instead used internal lung-fire clearing drugs and external corrosive agents, causing throat ulceration. I first used Wuwei Yigong San with Shengma, Chaihu, and Wuyi (Chrysanthemum Indicum Seed) as the main formula; supplemented with Siwei Feier Wan (Four-Ingredient Child-Nourishing Pill). As Spleen Qi gradually strengthened, Jiwei Luhui Wan (Nine-Ingredient Aloe Pill) was used at night as the main formula, assisted by Wuwei Yigong San, resulting in recovery.
A six-year-old girl had throat swelling and pain, nasal polyps, alternating chills and fever, frequent urination that turned white after a long time. This was diagnosed as Liver Cantharis (Liver Parasitic Disease). First, Jiawei Xiaoyao San with stir-fried black Longdancao (Gentian Root) was used. Heat and itching gradually subsided. Longdancao was then removed, and Siwei Feier Wan was added as an assistant, resulting in recovery.
A seven-year-old girl had nasal polyps. After applying toxic drugs, she developed sores with swelling and pain. External application of Huanglian (Coptis Rhizome), Gancao, and Huangbai (Phellodendron Bark) powder was used to clear heat and detoxify. Jiawei Xiaoyao San was used to clear Liver Fire, assisted by Siwei Feier Wan, resulting in recovery.
A girl frequently had polyps growing in her nose and lower body. Repeated use of toxic cauterization caused three-inch-long projections to emerge. Longdancao Tang (Gentian Decoction) was used as the main formula, assisted by Jiawei Xiaoyao San, resulting in recovery.
A child had a red forehead, hot soles, and chronic throat pain. After taking stomach-clearing and toxin-resolving drugs, I diagnosed innate insufficiency of Kidney Water with blazing Heart Fire. Dihuang Wan should have been used to strengthen the source of Water to control Yang. The family did not comply. The child died with red and burning mouth/tongue and dysuria-like symptoms.
A fourteen-year-old girl suffered from the aforementioned symptoms. Mixed use of heat-clearing and toxin-resolving drugs worsened the condition, leading to vaginal prolapse (Yin Ting). See section on Lower Chancre Sores for details.
Niubangzi Tang (Arctium Decoction) treats wind-heat congestion upward, causing throat swelling and pain, or mumps (Ru'e).
Niubangzi (Stir-fried and pounded) Xuan Shen Scrophulariae Radix Shengma Cimicifugae Rhizoma Jiegeng Platycodonis Radix (Stir-fried) Xijiao Rhinocerotis Cornu Huangqin Scutellariae Radix Mutong Akebiae Caulis Gancao Glycyrrhizae Radix et Rhizoma (Equal parts)
Take 1-2 qian per dose, decoct in water and drink.
Bacui Jiegeng Tang (Excellence Jiegeng Decoction) treats heat-swelling throat obstruction.

Grind into powder. Take 1-2 qian per dose, decoct in water and drink.

Grind into powder. Take 2 qian per dose, decoct in water and drink.
Sanyin Yuyaochi (Three Causes Jade Key) treats wind-heat throat closure and winding throat wind.
Yan Xiao Niter Peng Xiao Mirabilite Pian Nao Camphor Bai Jiangcan Bombyx Batryticatus (1.5 liang, 0.5 liang, 1 zi, 1 qian respectively)
Grind uniformly. Blow 0.5 qian into the throat for immediate cure.
Jiuwei Luhui Wan (Nine-Ingredient Aloe Pill) treats liver meridian accumulation heat causing sores in throat, mouth, and tongue; or gum erosion, eye nebula, ear discharge; or liver accumulation scrofula, lower chancre genital swelling; or white fluid from penis, tubercles in abdomen; or urine turning white after a long time, irregular stool, emaciation, etc.

Grind each separately into powder. Make pills with rice paste, size of hemp seed. Take half a qian per dose on an empty stomach with rice soup, adjusting dosage according to child's age.
