The Qi of all Yang meridians in the human body converges at the head and gathers on the face. Lesions such as sores and ulcers on the face occur due to disharmony of the Zang-Fu organs, causing stagnation of Qi and Blood in the Yang meridians. Etiologies may include congenital Kidney Yin deficiency with Liver Fire hyperactivity, maternal fetal toxin (Tai Du), maternal exposure to the Six Evils (Liu Yin) or Seven Emotions (Qi Qing), maternal consumption of rich/greasy foods and alcohol, or the infant's own intake of sweet, fatty, and strong-flavored foods. Since the causes differ, one must differentiate the affected meridian, examine the specific etiology, and treat accordingly. Sores appearing at the outer canthus, anterior to the ear, extending up the cheek to the nose and inner canthus belong to the Small Intestine Meridian; those at the vertex, temporal region, lower cheek, posterior to the ear, and sides of the brain belong to the Gallbladder Meridian; those at the anterior cheek, nostrils, and around the Philtrum belong to the Large Intestine Meridian; those at the sides of the nostrils, lower lip, corners of the mouth, Chengjiang point, post-mental region, Jiache point, anterior to the ear, and hairline/forehead belong to the Stomach Meridian; those at the inner canthus extending upward to the forehead tip and down to the occiput/nape belong to the Bladder Meridian. Having identified the meridian, treatment should be differentiated by meridian. For cases due to congenital Kidney Fire (Yin deficiency with empty heat), Liuwei Dihuang Wan is used; for fetal toxin, Xijiao Xiaodu Wan; for food accumulation and malnutrition (Gan), Siwei Fei'er Wan; for mothers consuming rich foods, Dongyuan Qingwei San; for diseases affecting the Fu organs, which inevitably involve the Zang organs: lesions on the forehead belong to the Heart Meridian—fever with desire for cold drinks indicates Excess Heat, treated with Daochi San; fever with desire for warm drinks indicates Deficiency Heat, treated with Yangxin Tang. The left cheek belongs to the Liver Meridian—neck stiffness indicates Excess Heat, treated with Chaihu Qinggan San; teeth grinding and irritability indicate Deficiency Heat, treated with Liuwei Dihuang Wan. The right cheek belongs to the Lung Meridian—wheezing/cough with desire for cold drinks indicates Excess Heat, treated with Xiebai San; fever with cough indicates Fire insulting Metal (Heart Fire insulting Lung Metal), treated with Renshen Pingfei San. The nose belongs to the Stomach Meridian—fever with desire for cold drinks and yellow/hard stool indicates Excess Heat, treated with Xiehuang San; fever with desire for warm drinks and green/white stool indicates Deficiency Heat, treated with Yigong San. Lesions on the chin and earlobes belong to the Bladder Meridian. The Kidney has no Excess patterns; only Dihuang Wan is used. If sores have ulcerated and fail to heal for a long time, one must assess the balance of Zang organ Qi and the transmission of pathogenic factors, focusing primarily on regulating and tonifying the Spleen and Stomach. If caused by residual heat from the wet nurse, treating the mother first will cure the infant.
A child had a sore on the temple, red, swollen, and painful. I diagnosed it as Wind-Heat in the Hand and Foot Shaoyang meridians and prescribed Chaihu Zhizi San. It was ineffective. Upon examining the mother’s pulse at the left Cun and Guan positions, it was wiry, surging, and rapid. I prescribed the same formula for the mother, and the child recovered.
A thirteen-year-old boy had swelling and pain in the right cheek, desiring cold drinks. The pulse was deep and solid, becoming rapid upon heavy pressure, indicating accumulated heat in the Zang organs. Internal purgation was indicated. The family disbelieved this and administered various miscellaneous medicines externally applied cooling agents. The swelling became hard and spread downward into the neck, where it ulcerated. Upon return, the pulse was forceless. I first administered Tuoli San for two doses and performed acupuncture/lancing. After another two doses, pus emerged. Symptoms included aversion to cold (slightly relieved), irritability, fever, thirst, and phlegm-wheezing. These were post-ulceration complications due to Qi and Blood deficiency. I first used Danggui Buxue Tang; after two doses, symptoms ceased. Then I used Yigong San with Zhizi, restoring Stomach Qi. Finally, Tuoli Xiaodu San was used, leading to healing.
A child had red swelling on the right cheek. I diagnosed Stomach Meridian Heat, initially prescribing Xiehuang San for two doses, followed by Qingwei San, resulting in recovery. Upon recurrence, application of calcined stone powder damaged the blood vessels, preventing ulceration. I first used Huoming Yin, followed by Tuoli Xiaodu San, leading to recovery.
A child had nodules behind the ear that were painful, with a cyan-red left cheek. This was caused by Liver Gan accumulation and heat. I prescribed Siwei Fei'er Wan, Chaihu Qinggan San, and Wuyi Gong San with Chaihu and Shengma, leading to resolution.
A child had red, swollen, painful ears with alternating chills and fever. This was Liver Meridian Heat-Toxin. I prescribed Jiawei Xiao Chai Hu Tang; chills and fever subsided. Then Chaihu Qinggan San was used, resolving the redness and swelling.
A child had redness and thirst on the chin, excessive white in the eyes, frequent facial sores, and sleeping with eyes open. My father diagnosed congenital Father-side Yin deficiency and prescribed Dihuang Wan and Buzhong Yiqi Tang, resulting in recovery. Later, when smallpox appeared, there was no danger. Had we not pre-conditioned the Kidney Qi, could the danger of smallpox have been guaranteed against?
A four-year-old child had itching at the Taiyang area connecting to the eyebrows, sometimes developing small sores. This was diagnosed as Gallbladder Meridian Wind-Heat. I first used Dihuang Wan, then Chaihu Zhizi San, and finally exclusively Dihuang Wan, leading to recovery.
A child frequently developed facial sores with red swelling on the left cheek and occasional convulsions during sleep. Investigation revealed the mother had chronic emotional depression/anger. I prescribed Jiawei Xiaoyao San and Jiawei Guipi Tang for both mother and child, resulting in recovery.
A child had a pale complexion that occasionally turned red, with small sores and hot feet. My father diagnosed congenital Deficiency Heat in the Three Foot-Yang meridients. The family disbelieved this and exclusively used cooling medicines. Later, when smallpox appeared, it turned black and depressed, leading to death.
A child suffered from the aforementioned condition with submandibular lymphadenopathy. I stated this was Liver-Spleen Gan toxin. As it did not heal for a long time, I first used Wuyi Gong San with Chaihu Zhizi San to clear Liver Fire. Subsequently, Siwei Fei'er Wan was used to treat Gan, leading to recovery.
A child suffered from the aforementioned condition with retroauricular lymphadenopathy, sour-smelling stool, and reduced appetite. I diagnosed Spleen Gan. I primarily used Wuyi Gong San, assisted by Yigong San, leading to recovery.
A child suffered from the aforementioned condition with shiny/tight scalp, fever, thirst, and frequent urination. I diagnosed Kidney-Liver Gan. I primarily used Dihuang Wan, administering Buzhong Yiqi Tang in the morning and Wuyi Gong San in the evening, leading to recovery.
A child suffered from the aforementioned condition with a yellow nasal tip, cyan left cheek, poor appetite, and diarrhea. After taking Xijiao Wan, the child became emaciated, thirsty, and desired warm drinks. I used Buzhong Yiqi Tang to strengthen Spleen Qi; assisted by Siwei Fei'er Wan to eliminate Fu toxin, leading to recovery.
A child suffered from the aforementioned condition with variable pain/itching, red right cheek and nasal tip, indicating Stomach Meridian Heat. Investigation confirmed it was caused by the mother drinking alcohol. I first used Qingwei San, then Jiawei Xiaoyao San to treat the mother's heat, while applying Dafeng Gao to the child, leading to recovery.
A child suffered from the aforementioned condition due to maternal anger, accompanied by convulsions, with redness in the nose and left cheek. I first used Jiawei Xiao Chai Hu Tang for two doses, then Jiawei Guipi Tang for four doses to treat the mother's heat; the child also improved with minimal intake.
A child suffered from head/facial sores due to the mother eating rich foods, with redness on the right cheek persisting for a long time. I used Qingwei San to treat the mother and Niuhuang San to treat the child, leading to recovery within ten days.
A child had facial sores with sudden red swelling of the head, locked jaw, and inability to speak. This was intense Stomach Meridian Heat. I used Huoming Yin mixed with wine; two doses provided slight relief. One more dose of wine-decoction reduced swelling. Finally, Jiedu San led to complete recovery.
A child had red, painful facial sores with fever and desire for cold drinks. The Spleen and Lung pulses were rapid and forceful. I used Xianfang Huoming Yin for two doses. Fluid exuded from the sores, spreading over the body, resembling leprosy. I continued with the previous decoction and Qingre Xiaodu San, leading to recovery.
My cousin Ling Yunxiao, aged fifteen, presented in summer of Renyin year with a red, slightly dry face and emaciation. I said, "Your disease will progress." In winter of Gui Mao year, I saw him again and said, "Your disease will deepen." By spring of Jiachen year, the foot Yangming meridian areas were cyan, indicating Wood invading Earth. I first used Liujun Zi Tang, Chaihu, Shaoyao, Zhizi, Wuyi, and stir-fried black Huanglian for several doses to stabilize. Then I used Siwei Fei'er Wan, Liuwei Dihuang Wan, and a decoction of Renshen, Fuling, Danggui, Baizhu, Shaoyao, Gancao, Zhizi, Chaihu, and Rougui for over thirty doses, combined with Jiawei Bawei Wan, leading to recovery.
A child first had itching between the eyebrows, scratching led to sores spreading to the head and face, treated with Lixiao San. Later, due to maternal anger, itching and pain returned. This was Heat in the Liver and Gallbladder meridians. The child used Niuhuang Jiedu Wan, and the mother used Jiawei Xiaoyao San, leading to recovery.
A child first had eroded eyebrows spreading over the body like leprosy, unhealed for a long time, with hot hands/feet and constant red face. This was due to maternal Stomach Fire. The son took Xiehuang San; the mother took Zhuye Shigao Tang and Jiawei Xiaoyao San. The son also used Niuhuang San and Badu San; both mother and son recovered.
A child had head sores resembling leprosy, with itching/scabbing or dripping pus for two years, accompanied by thirst for cold drinks, fever, and red face. This was inherited Heat in the Heart and Small Intestine (Interior-Exterior relationship). I first used Daochi San for two doses, then Huoming Yin and Badu San gradually improved the condition. The son took Niuhuang San, and the mother took Xiaoyao San, leading to recovery. Later, recurrence due to maternal rich food diet was treated with Qingwei San for both; the son additionally took Huoming Yin, leading to recovery.
A child had facial sores, thirst for warm drinks. After taking Baidu San, vomiting/diarrhea occurred, loss of appetite, and cold extremities. I diagnosed Spleen-Stomach Qi deficiency with secondary injury, transforming into Deficiency-Cold. I first used Yihuang San to reverse the adverse symptoms, then Yigong San to heal the sores.
