Qian Zhongyang stated: Slow convulsion (Man Jing) occurs due to damage to the Spleen and Stomach following illness, vomiting, diarrhea, or improper medication. Symptoms include cold limbs, weak breath from the mouth and nose, twitching of the hands and feet, drowsiness with eyes half-open. This is wind generated by Spleen deficiency, a syndrome without Yang. The primary treatment is Wen Bai Wan (Warm White Pill). Generally, slow convulsion belongs to Yin syndromes; all involve organ pathology characterized by deficiency. Due to vomiting and diarrhea, both the Spleen and Lung become deficient, allowing Liver Wood to overact upon them, resulting in slight twitching. As Lou Quanshan said: "When Wood is deficient, there is twitching but without strength." The Classics state: "Insufficiency of Wood is called Wei He (Compliant Harmony), its disease manifests as shaking of the head" (referring to twitching of hands and feet, diarrhea, palpitations). Insufficiency of Fire leads to cold body and cold breath. Insufficiency of Earth leads to vomiting and diarrhea, and sleeping with eyes exposed. Treatment should focus on warming and tonifying the Spleen and Stomach, using formulas such as Liu Jun Zi Tang (Six Gentlemen Decoction) or Wu Wei Yi Gong San (Five-Ingredient Powder Enhancing Merit). Xu Yongcheng stated: Yi Wood belongs to Yin and is a disease of the Liver, hence it is slow and difficult to treat. Furthermore, cases may present with concurrent Heat, Food Retention, Phlegm, or symptoms resembling exterior contraction; in such cases, one should follow Qian's prescriptions as the main approach. *Bao Ying Ji* states: If acute convulsion (Ji Jing) recurs frequently and purgative drugs are used repeatedly, Spleen Yin will be further depleted, leading to many cases transforming into slow convulsion. Typically, symptoms include vomiting and diarrhea, phlegm rattle, asthma, open eyes with sluggish spirit, drowsiness with eyes exposed, startle jumps and convulsions, alternating between发作 and quietness, alternating hot and cold body sensations, pale cyan or white complexion, or cyan/red eyebrows and lips, with pulse being slow, deep, rapid, or moderate. Treatment should primarily warm and tonify Spleen Qi, assisted by calming the Heart and subduing the Liver. Li Dongyuan also stated: Slow convulsion arises from Spleen and Stomach deficiency. Spleen deficiency occurs when Fire pathogen invades the Earth position; excessive Fire strengthens Wood, and excessive Wood attacks Earth. One should use sweet-warm herbs to tonify the source of Earth via the Heart channel, and simultaneously clear Fire in the Spleen/Earth using sweet-cold herbs to tonify Metal, and sour-cool herbs to astringe, thereby making Metal strong and Fire weak within the Spleen/Earth, causing Wind-Wood to naturally become deficient. Individuals with congenital insufficiency, prolonged illness causing Spleen deficiency, or those who frequently take draining/depleting drugs are prone to this condition. If the cause is Earth deficiency failing to generate Metal, and Metal failing to control Wood, leading to Wood insulting Earth and causing the aforementioned symptoms, use Wu Wei Yi Gong San added with Dang Gui (Angelica Sinensis Root) and Suan Zao Ren (Ziziphus Seed), assisted by Gou Teng Yin (Uncaria Decoction) to tonify Earth and level Wood. If Spleen/Earth is deficient and cold, use Liu Jun Zi Tang added with Pao Jiang (Roasted Ginger) and Mu Xiang (Aucklandia Root); if ineffective, urgently add Fu Zi (Prepared Aconite Root) to restore Yang Qi. Generally, Yin Blood is born from Spleen/Earth; Si Jun Zi Tang (Four Gentlemen Decoction), Dang Gui, and Suan Zao Ren are suitable. For cases where Yuan Qi (Primal Qi) is depleted leading to coma, urgently moxa Bai Hui (GV20); if one waits until phlegm discharge fails before moxing, Yuan Qi will scatter and rescue will be impossible. This is because the transmission of Zang-Fu has reached its extreme, returning to deficiency; only the Spleen is affected, with no Wind to expel and no Convulsion to treat. Because Spleen deficiency cannot control saliva, fluids overflow excessively, appearing like Phlegm. One should follow the previous methods for effectiveness. If the cause is not examined and wind-dispelling/phlegm-resolving agents are used indiscriminately, Spleen Qi will be further injured, Yin Blood further damaged, pathogens stronger, leading to danger.
Clinical Cases:
The son of Scholar Yu Shizheng developed erysipelas due to food stagnation. After taking exterior-releasing agents, he experienced hand and foot convulsions. After taking Bao Long Wan (Treasure Dragon Pill), phlegm increased. I stated: This is Spleen and Stomach depletion transforming into slow convulsion; there is no Wind to dispel and no Phlegm to resolve; only warming and tonifying Stomach Qi is appropriate. Thus, I used Liu Jun Zi Tang plus Fu Zi (Prepared Aconite Root), which cured him in one dose.
A child had convulsions with spontaneous salivation. Someone used convulsion medicines, worsening the condition. Observing his yellowish-white complexion, I used Liu Jun Zi Tang and Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) to tonify Spleen and Lung, curing him.
A child had wind-cold cough with surging phlegm. Treated with Liu Jun Zi Tang, Jie Geng (Platycodon Root), Sang Bai Pi (Mulberry Root Bark), and Xing Ren (Apricot Seed), cured. Later, food stagnation caused diarrhea and abdominal distension; treated with Liu Jun Zi Tang added with Shan Zha (Hawthorn Fruit) and Hou Po (Magnolia Bark), stabilized. Again, food retention caused vomiting; someone used purgatives, worsening the cough. I stated: Spleen and Lung are both deficient, suitable for regulation and tonification. The parents thought it too slow and self-administered exterior-releasing/draining-stagnation drugs, worsening previous symptoms and causing head and neck tremors. I used Tian Nan Xing San (Arisaema Powder), doubling the amount of Gou Teng Gou (Uncaria Rhynchophyllis Hook) and Yi Gong San (Merit-Enhancing Powder), curing him.
A child developed excessive phlegm, teeth grinding, and convulsions upon fright, with head shaking and diarrhea. Strictly administering Nao She (Camphor and Musk) and Zhu Sha (Cinnabar) led to slow convulsion and death.
Shu Fu Tang (Atractylodes and Aconite Decoction) treats rheumatism and wind-dampness clashing, causing bodily pain and inability to turn, no vomiting or thirst, hard stool, and free urination, as well as wind affecting head and eyes.
Bai Zhu (Atractodis Rhizoma, 4 liang) Gan Cao (Licorice Root, stir-fried, 2 liang) Fu Zi (Prepared Aconite Root, peeled and de-navelled, 1 liang)
Grind into powder. Add Fu Zi, take 3 qian per dose, with 5 slices of ginger and 1 jujube, decoct in water and drink.
My Commentary: Fu Zi warms the middle and restores Yang; it is the sacred medicine for Man Pi Feng (Slow Spleen Wind). If Yuan Qi is not yet collapsed, it is invariably effective. One must use individual roots weighing 1–3 qian, upright, non-pointed, flat-bottomed, with petal-like surroundings. Otherwise, Chuan Wu (Sichuan Aconite) may be mistaken for it. Processing method: Cut off skin and tips, soak in child's urine. Autumn/Winter: 7 days; Spring/Summer: 5 days. Change daily. After soaking, cut into four pieces, wrap in several layers of wet grass paper, roast slowly over low fire for half a day. Take out, cut open; stop when no white spots remain. For urgent use, roast until cracked, then soak in child's urine for a long time, slice. If color is white, roast slightly again. For severe Qi collapse, raw usage is also effective.
