一小儿腿腕间患此,已半载,肿硬色白,形气俱虚。余先用五味异功散,加当归三十余剂,却佐以八珍汤二十余剂;
胃虚寒之恶症也。以异功散,内用人参一两、干姜一钱半,灌之尽剂而苏。又以托里散,内用人参五钱,数剂始能动履;

Pediatric Liuzhu (Flowing Abscess) is a condition caused by the stagnation of Qi flow and the coagulation of blood stasis, belonging to syndromes of insufficient Yuan Qi (Original Qi). Etiologies may include sprains or falls, hyperactivity of Liver Fire leading to rebellious Qi, internal invasion of the Six Exogenous Pathogens (Liu Yin), Spleen deficiency with food retention, or congenital endowment insufficiency. The pathogen knots in the limbs and joints, manifesting in the chest, abdomen, waist, and buttocks, presenting as nodules, diffuse swelling without distinct borders, or pain. Treatment should universally employ the Scallion Moxibustion Method (Cong Yi Fa), with the primary principle being the consolidation of Yuan Qi.
For cases due to sprains or falls, use Hexue Dingtong Wan (Blood Harmonizing and Pain-Relieving Pill). For cases due to Liver Fire, use Jiwei Lujian Wan (Nine-Ingredient Aloe Pill). For cases due to food retention, use Siwei Fei'er Wan (Four-Ingredient Child-Nourishing Pill). If the medication corresponds to the syndrome, un-suppurated lesions will resolve spontaneously, and suppurated lesions will rupture spontaneously. If pus has formed but fails to rupture, it is due to deficient Yuan Qi; one must first tonify Yuan Qi before puncturing, so that the toxic pathogens do not attack internally and Qi and Blood do not collapse. If pain persists after pus drainage, it is due to Qi and Blood deficiency; use Bazhen Tang (Eight-Treasure Decoction). Vomiting and anorexia indicate Stomach Qi deficiency; use Sijunzi Tang (Four-Gentlemen Decoction). Nausea with refusal to eat, or abdominal distension, indicates Spleen Qi deficiency; use Liujunzi Tang (Six-Gentlemen Decoction). Trismus and convulsions indicate extreme deficiency of Qi and Blood resulting in transformational syndromes; use Shiquan Dabu Tang (Complete Great Tonifying Decoction). Internal heat or afternoon tidal fever indicates Yin Blood deficiency; use Siwu Tang (Four-Substance Decoction) plus Renshen (Ginseng), Baizhu (Atractylodes Macrocephala Rhizome), and Fuling (Poria). Exterior heat with aversion to cold indicates Yang Qi deficiency; use Shiquan Dabu Tang (Complete Great Tonifying Decoction). Heat that comes and goes, or appears by day and hides by night, or hides by day and appears by night, constitutes Deficient Heat; one should greatly tonify Yuan Qi. If the lesion is red, swollen, and the pus is thick, it is treatable. If it is not red, hard, and the pus is thin, or if the pulse is洪大 (Huge) accompanied by alternating chills and fever, thirst, and intolerance to tonification, it is untreatable.
The son of Yang Honglu, aged twelve, suffered from this condition under his left hypochondrium. After taking medications such as Liuqi Yin (Flowing Qi Decoction) and Shixuan San (Ten-Aperture Powder), his Yuan Qi became further depleted, and the wound failed to heal for over a year. His Zuo Chi (Left Cuneiform) pulse was rapid and forceless, and his Zuo Guan (Left Crotch) pulse was wiry and short. This is a Liver Meridian syndrome, caused by congenital insufficiency of Kidney Water failing to nourish Liver Wood, resulting in dry Blood and blazing Fire. He was treated with Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to nourish Kidney Water and Jiwei Lujian Wan (Nine-Ingredient Aloe Pill) to clear Liver Fire, resulting in recovery.
A nine-year-old child suffered from this condition with long-term non-healing, occasional cough, and alternating chills and fever. All were treated with Qingqi Huatan drugs (Clear Qi and Resolve Phlegm), exacerbating previous symptoms. At night, he wheezed with mouth open, slept through the night, and had cold hands and feet. Medication induced vomiting upon ingestion. I reasoned that all were due to extreme Spleen Qi deficiency. Initially, I used five qian of Renshen (Ginseng) and Baizhu (Atractylodes Macrocephala Rhizome), and five fen of Paojiang (Prepared Ginger), decocted in rice soup, administered in small spoonfuls periodically. The next day, he could drink one cup; the following day, another dose. Symptoms gradually improved. After more than ten doses, he took Buzhong Yiqi Tang (Middle-Boosting and Qi-Invigorating Decoction) in the morning and Yigong San (Extraordinary Skill Powder) in the evening, leading to recovery.
The son of Li Tongfu, aged sixteen, suffered from this on his lower back for three years without healing, presenting with dark, depressed coloration. I stated: "This is a Kidney Meridian syndrome. It is suitable to use Liuwei Wan (Six-Ingredient Pill) to nourish the source of transformation to generate Kidney Water, and simultaneously apply Rusheng Bing (Sage Cake) externally to disperse cold pathogens and reconnect Yang Qi." He did not believe me and used miscellaneous drugs, causing further depletion of Yuan Qi and the onset of Seven Evil Syndromes. Only then did he accept my words, using the previous medicines to recover.
The son of Chen Zhou Shou suffered from swelling and pain in the right wrist due to a sprain. After using Liuqi (Flowing Qi) drugs, he developed fever and chills, reduced appetite, dry mouth and tongue, intensified swelling and pain, and increased fatigue. I focused on strengthening the Stomach and boosting Qi, assisted by external treatment methods, resulting in recovery.
The son of Huang Diguan suffered from this on his leg, with swelling, pain, and fever. Treated as Damp-Toxin, all deficiency syndromes appeared. I stated this was due to weak Yuan Qi invaded by external pathogens. He recovered after more than ten doses of Dabu Tang (Great Tonifying Decoction) and Rusheng Bing (Sage Cake).
A child had swelling and hardness in the arm and elbow. After using Liuqi Yin (Flowing Qi Decoction), the skin color remained unchanged, and appetite decreased. I stated: "This is a Liver and Spleen deficiency syndrome." He was treated with Liujunzi Tang (Six-Gentlemen Decoction) plus Jiegeng (Platycodon Root), Bohe (Mint), Guizhi (Cinnamon Twig), Fuling (Poria), Banxia (Pinellia Tuber), and Rusheng Bing (Sage Cake), leading to resolution.
A child suffered from this between the leg and ankle for half a year, with swelling, hardness, white color, and deficiency of both form and Qi. I initially used Wuyi Yigong San (Five-Agent Extraordinary Skill Powder) with Danggui (Angelica Sinensis) for more than thirty doses, followed by Bazhen Tang (Eight-Treasure Decoction) for more than twenty doses as an adjunct. Then, the Cong Yi Fa (Scallion Moxibustion Method) was applied, gradually reducing the swelling, though a central lump remained. This indicated impending suppuration. Blood and Qi should be tonified, primarily using Tuoli San (Interior-Supporting Powder) with Yigong San (Extraordinary Skill Powder) as an adjunct, along with the Cong Yi Fa (Scallion Moxibustion Method). After about a month, thick pus was drained via puncture. Using the previous two medicines and Douchi Bing (Fermented Soybean Cake) for over three months led to recovery.
A child suffered from this on the leg, with swelling, hardness, white color, aversion to cold, and reluctance to eat. This is due to deficiency of Spleen and Stomach Yang Qi, unable to form pus; it cannot be treated with poultices that defeat toxins. Thus, Tuoli San (Interior-Supporting Powder) and the Cong Yi Fa (Scallion Moxibustion Method) were used. After more than a month, the lesion became painful, swollen, hot, and pus formed; it was punctured, and peace returned upon pus drainage. Tuoli San (Interior-Supporting Powder) was continued, and the limbs gradually strengthened. Due to dietary injury causing diarrhea, sudden trismus, closed eyes, spontaneous sweating, and cold hands occurred. This is an evil syndrome of Spleen and Stomach deficiency-cold. Yigong San (Extraordinary Skill Powder) with one liang of Renshen (Ginseng) and one qian and half of Ganjiang (Dried Ginger) was administered, reviving him after one dose. Then, Tuoli San (Interior-Supporting Powder) with five qian of Renshen (Ginseng) was given for several doses until movement resumed. Finally, Tuoli San (Interior-Supporting Powder), Dabu Tang (Great Tonifying Decoction), the Cong Yi Fa (Scallion Moxibustion Method), and Douchi Bing (Fermented Soybean Cake) were used, leading to recovery after half a year.
A fifteen-year-old child, having lost his natural vitality early, experienced afternoon fever, whole-body pain, or softness and soreness in the four limbs, expectoration of phlegm, and dizziness. After taking drugs to dispel dampness and resolve phlegm, the muscles inside and outside the legs became swollen, hard, and unchanged in color. Because he took drugs to attack toxins, deficiency syndromes arose. Note: Chu Shi said, "When a man's essence is not full, yet he engages in intercourse with women to drain his essence, the five zang organs have unfulfilled aspects, and later there will be difficult-to-describe diseases." This perfectly matches this theory. He was treated with Buzhong Yiqi Tang (Middle-Boosting and Qi-Invigorating Decoction) and Dihuang Wan (Rehmannia Pill), recovering after half a year. Such syndromes, mistakenly identified as Excess-Heat and treated with toxin-defeating drugs, will inevitably lead to death.
The son of Jia Gelao, aged sixteen, had suffered from this for two years. The pulse was huge and rapid, with pus, heat, thirst, reduced food intake, body fatigue, night sweats, and morning aversion to cold. I stated: "This is a syndrome of insufficient True Qi and excess pathogenic Qi; treatment will be ineffective." He pleaded for treatment. In the morning, he was reluctantly given Sijunzi Tang (Four-Gentlemen Decoction) plus Xiong (Ligusticum Chuanxiong), Gui (Angelica Sinensis), and Zhi Cao (Honey-fried Licorice); in the afternoon, Sijunzi Tang (Four-Gentlemen Decoction) plus Maidong (Ophiopogon Tuber) and Wuweizi (Schisandra Fruit). After a month, symptoms gradually decreased. Someone used diuretic agents, promising complete recovery. He believed and took them, becoming emaciated, and died shortly thereafter.
A child had severe swelling at the right ankle, with a timid physique. I intended to focus on tonifying Qi and Blood, assisting with moving and dispersing agents. He did not believe me, instead taking Liuqi Yin (Flowing Qi Decoction) internally and applying cold-cooling drugs externally, worsening fever and chills, and increasing emaciation. He then sought my treatment. I stated: "Aversion to cold and fever, with a huge pulse, are evil syndromes of exhausted Qi and Blood; they are untreatable." Later, it ulcerated and failed to heal, draining all Qi and Blood until death.
The son of Zhangjiao Gu Dongfan, in his teens, suffered from vague pulling pain in the waist and legs in autumn, with a complexion green mixed with black. I stated: "Green indicates Liver deficiency, black indicates Kidney deficiency. One must urgently regulate and tonify Spleen and Kidney, otherwise, Liuzhu (Flowing Abscess) will occur in spring." He did not believe me and used Qi-moving and Blood-breaking drugs. By summer, there was diffuse swelling of about five cun between the waist and buttocks. He came again for treatment. The pulse was rapid and slippery, feeling empty upon pressure. This indicates extreme Yuan Qi deficiency with internal rupture of pus that cannot rise. I declined to treat. He subsequently died.

Shenxiao Cong Yi Fa (Miraculous Scallion Moxibustion Method) treats Liuzhu nodules, or bone abscesses, Crane's Knee, etc. First apply Garlic Partition Moxibustion. If residual swelling remains, use this method to moxa, aiding Qi flow and Blood circulation, dissipating stagnation, with very rapid effects. It is also an excellent method for treating contusions and injuries, stopping pain, dispersing blood, and reducing swelling. The method involves finely chopping scallions, pounding them into a paste, stir-frying until hot, and frequently moxa-ing the affected area. Replace when cool. For Crane's Knee Wind, concurrently take Dafangfeng Tang (Great Saposhnikovia Decoction) for recovery.
Garlic Partition Moxibustion Method treats Liuzhu, carbuncles, sores, Crane's Knee Wind, etc. Moxa 20-30 cones daily. For pain, moxa until pain ceases; for no pain, moxa until pain arises. The toxin disperses with the fire. Since fire promotes free flow, drawing out stagnant toxins, this is a contrary treatment method with life-saving power. The method involves peeling garlic, slicing it to the thickness of three cash coins, placing it on the lesion, and moxa-ing three cones on top of the garlic. Replace garlic and continue moxa-ing. Unsuppurated lesions will resolve; suppurated ones will kill the toxin. For large sores, pound garlic into a paste, spread on the lesion, place moxa on top and burn. Replace garlic when it spoils. For lesions that are painless, suppurate, fail to rise, or belong to Yin syndromes, especially many moxa-cones are needed. Generally, sores that are painless, do not suppurate, or do not rise are due to Qi and Blood deficiency and are mostly untreatable. Only those on the head and face should not be heavily moxa-ed. Infant moxa methods are discussed in the Carbuncle section.
