Carbuncles and sores caused by heat-toxin arise from consuming rich, greasy, and strong-flavored foods, or from the fire generated by emotional stagnation in the nursing mother. If swelling involves deep bone pain, it indicates stagnation of Qi and blood; use Xianfang Huoming Yin (Immortal Life Elixir). If there is thirst and constipation, it indicates internal accumulation of heat-toxin; use Sishun Qingliang Yin (Four-Compliance Cooling Clearing Decoction), assisted with Rusheng Bing (Like-Sage Cake). If swelling is hard and red, it indicates condensation of heat-toxin; use Huoming Yin, assisted with Garlic-Partitioned Moxibustion. If swelling does not subside, it indicates impending suppuration; use Tuoli Xiaodu San (Interior-Supporting Toxin-Expelling Powder). If pus fails to form, or forms but does not ulcerate, it indicates deficiency of Qi and blood; use Bazhen Tang (Eight-Treasure Decoction). If after ulceration the flesh is red and fails to heal, it indicates deficiency of Spleen blood; use Siwu Tang (Four-Substance Decoction) plus Ginseng and Atractylodes. If the flesh is white and fails to heal, it indicates weakness of Spleen Qi; use Sijunzi Tang (Four-Gentleman Decoction) plus Chuanxiong and Angelica. If appetite is poor, the body feels fatigued, and the wound fails to heal, it indicates deficiency of Spleen Qi; use Liujunzi Tang (Six-Gentleman Decoction) plus Danggui and Shengma. When the medicine matches the syndrome, recovery is inevitable. However, if one recklessly uses toxic-expelling agents, depleting Yuan Qi (Original Qi), the condition may transform into a malignant syndrome that is difficult to treat. Generally, carbuncles belonging to the Fu organs are easy to treat; those where Yuan Qi is not depleted will heal spontaneously even without treatment. Carbuncles belonging to the Zang organs are difficult to treat; if Yuan Qi is already depleted, they transform into malignant syndromes. If one mistakenly uses purgative or attacking methods, further depleting Yuan Qi, treatment becomes especially difficult. Therefore, harsh and drastic medicines must absolutely be avoided. Observing Li Dongyuan and Zhu Danxi: whenever swelling and pain are seen, if pulse and symptoms indicate deficiency, one should provide tonification. If Blood and Qi are not depleted, auspicious outcomes can be guaranteed. Using expulsive and detoxifying agents inevitably leads to the disaster of "making the deficient more deficient."
A child suffered from this condition. After applying and taking detoxifying herbs, the swelling worsened, accompanied by vomiting. The pulse at the Yin-Guan position was green and red, indicating heat-toxin of wind in the Liver channel, compounded by injury to Middle Qi. Treatment with Wuweiyigong San (Five-Flavor Extra-Effort Powder) plus Chaihu and Shengma, followed by Buzhong Yiqi Tang (Middle-Warmer-Boosting Qi-Supplementing Decoction) plus Baizhi and Jiegeng, resulted in recovery.
A girl over ten years old had red swelling extending from below the ear to the neck, accompanied by chills, fever, and headache. Strict application of Tiegu San (Iron-Hoop Powder) was ineffective. This was an internal flare-up of Fire in the Shaoyang Gallbladder channel, which Tiegu San could not cure. I used Zhizi Qinggan San (Gardenia-Liver-Clearing Powder) and achieved a cure.
A child presented with similar symptoms. The pulse at the Right Guan position was rapid, becoming wiry upon pressure, accompanied by vomiting and reluctance to eat. This was caused by Liver Wood overacting on Spleen Earth. Treatment with Xiao Chaihu Tang (Minor Bupleurum Decoction) minus Huangqin, plus Fuling and Shaoyao, resulted in recovery.
A child suffered from this condition. I attributed it to innate Liver and Spleen Qi stagnation, but the parents did not believe me. They used Tiegu San and Xijiao Wan (Rhinoceros Horn Pill), resulting in vomiting, reduced food intake, and cold hands and feet. This indicated re-injury to the Spleen and Stomach. I treated the child with Wuweiyigong San plus Muxiang; the mother took Jiawei Xiaoyao San (Modified Free Wanderer Powder) and Jiawei Guipi Tang (Modified Heart-Supplementing Decoction), leading to resolution.
A child suffered from this condition at the Keyuan acupoint area. After external application and internal administration of detoxifying drugs, fever and painful swelling occurred. Palpation revealed softness, indicating internal suppuration. Fortunately, the right cheek was white and the left yellow, indicating mutual generation between Spleen and Stomach, suggesting an easy recovery. I punctured to drain the pus and used Tuoli San (Interior-Supporting Powder) for a cure.
A child suffered from this condition on the head and face. After taking Stomach-clearing herbs, swelling and pain worsened. I diagnosed intense toxin and unresolved blood stasis, prescribing Xianfang Huoming Yin. Recovery occurred after two doses. Later, due to food stagnation, the child experienced morning chills and evening fevers, with recurrence on the head and face. Taking fire-descending herbs led to red swollen mouth and tongue, and cold hands and feet. I diagnosed re-injury to Stomach Qi resulting in deficiency-cold, treating with Wuweiyigong San for recovery.
A child had many boils on the head, with chills, fever, and pain. It was late summer, indicating both physical constitution and pathogen were excess. First, Renshen Baidu San (Ginseng-Toxin-Expelling Powder) plus Huanglian and Xiangru was given for one dose, stopping pain immediately. Next, three doses of Xianfang Huoming Yin powder were administered; larger boils suppurated, smaller ones resolved spontaneously. Recurrence after eating rich flavors was cured with one dose each of Qingwei San (Stomach-Clearing Powder) and Huoming Yin.
A child who frequently ate roasted foods suffered intermittently from this condition, caused by accumulation of heat from rich foods. Qingwei San and Xiehuang Er San (Two Draining-Yellow Powders) brought improvement. Subsequent food stagnation and administration of Radix Crotonis Fructus (Badou) herbs led to red, eroded mouth and tongue, and facial sores. This indicated re-injured Stomach Qi and internal heat, treated with Renshen Anwei San (Ginseng-Stomach-Anchoring Powder) for recovery.
A child suffered intermittently from this condition, accompanied by tubercles on the cheeks. This was a symptom of Liver Gan (malnutrition/parasite syndrome). First, Longdan Xiegan Tang (Gentian-Draining-Liver Decoction) was given for two doses to clear Liver Fire. Then, Siwei Feier Wan (Four-Ingredient Intestine-Nourishing Pills) and Wuweiyigong San plus Shengma and Chaihu were used to resolve Gan and strengthen the Spleen, resulting in recovery.
A child with chronic Liver and Spleen issues suffered many boils. Two doses of Xianfang Huoming Yin quickly reduced swelling and pain. Then, Siwei Feier Wan and Wuweiyigong San plus Chaihu and Shengma were used for recovery. At that time, others with the same condition used Xijiao Wan and Huadu Dan (Toxin-Transforming Pellet), injuring their Spleen and Stomach, all of whom died.
A child suffered on the head and face with severe swelling and pain, attributed to Heat-Toxin of the Stomach channel. First, Xianfang Huoming Yin powder was used, followed by Qingwei San for recovery. Later, mouth ulcers appeared after applying topical powders, leading to heavy abdominal pain, vomiting, anorexia, and cold fingers. I diagnosed Spleen-Stomach deficiency-cold and treated with Yigong San plus Shengma for recovery.
A twelve-year-old child suffered on the chest with painful swelling. After applying Tiegu San and taking Xijiao Wan, cold abdominal pain occurred. Verification showed pus formation. First, Wuweiyigong San was used, then Tuoli Xiaodu San, allowing natural drainage. Finally, Tuoli San was used for recovery.
A child suffered toxicity on the arm with diffuse swelling and slight pain. After applying Tiegu San, frequent vomiting, chest/abdominal fullness, and cold limbs occurred. This indicated Spleen Qi deficiency-cold, a half-Yin half-Yang syndrome. Tiegu San, being purely Yang, was unsuitable. Instead, Chonghe Gao (Harmony Paste) was applied, and Liujunzi Tang plus Ganjiang (Dried Ginger) was taken for resolution. Children's Yuan Qi easily shifts between excess and deficiency; using cold applications forces toxins into the Zang organs, often leading to untreatable conditions.
Xianfang Huoming Yin (Immortal Life Elixir):

Grind into powder, form pills with rice porridge, size of wheat grains. Apply one pill to the sore head, cover with plaster for half a shichen (one hour). Pus will discharge naturally. If the sore opens but no pus comes out, or pain increases, or fever occurs, it indicates Qi and blood deficiency; use Tuoli San. If vomiting phlegm, poor appetite, and fatigue occur, it indicates Spleen Qi deficiency; use Liujunzi Tang.
Wufu Huadu Dan (Five-Blessings Toxin-Transforming Pellet): Treats accumulated heat-toxin, red dry throat, mouth/tongue ulcers, or head/face boils, with delirious restlessness. (Formula see Section on Neonatal Toxin Carbuncles).
Note: The preceding formulas generate and cool blood, clearing cold-damaging toxins. They are effective for excess syndromes of both form and pathogen. Stop after 1-2 pills; do not overdose, as excessive use damages Yuan Qi and causes unpredictable complications.
Tianwu San (Heavenly Black Powder):
Tiannanxing (Arisaematis Rhizoma), Chixiaodou (Vigna Angularis Semen), Caowu (Aconiti Radix Lateralis Preparata), Huangbai (Phellodendri Cortex) (equal parts).
Grind into powder, mix with ginger juice and rice vinegar, apply to affected area.

Neonatal Toxin Scrofula (Tai Du Luo Li) arises from inherited depression-fire and Qi stagnation in the Liver and Gallbladder channels. These channels run along the sides of the body. If illness arises from Liver Fire, it manifests in these areas; treat according to the cause. If caused by maternal anger or Blood deficiency with internal heat, treat the mother based on the cause. Do not use harsh drugs to avoid damaging Yuan Qi.
A newborn had five nodules on the neck. The mother had a history of anger, chills/fever, breast pain, and slight hypochondriac swelling, all indicating Liver/Gallbladder channel issues. First, Xiao Chaihu Tang plus Danggui and Shaoyao cleared the chills/fever immediately. Then, Jiawei Xiaoyao San was taken by the mother for over two months, curing the child.
A child suffered around the ears and neck because the mother had Liver channel heat. Treating the mother with Jiawei Xiaoyao San cured the child spontaneously.
A three-year-old had nodules on the neck and below the ears. Long-term use of detoxifying herbs worsened the condition and depleted Yuan Qi. The mother’s pulse indicated Liver/Spleen Blood deficiency with heat due to chronic anger. Primary treatment was Jiawei Guipi Tang, assisted by Jiawei Xiaoyao San. Maternal heat subsided; the child drank several spoonfuls daily, recovering in two months.
A child had four nodules on the neck since birth, progressing to pearl-like strands around the ears by age five. Severe Yuan Qi deficiency, alternating chills/fever, and incomplete feeding indicated inherited Liver/Gallbladder Qi stagnation. The mother took Bazhen Tang and Xiaoyao San. After six months of breastfeeding, the child’s physique improved, nodules diminished, and final recovery occurred with Dihuang Wan and Xiaoyao San.
A child had neck nodules with intermittent chills/fever, blue left cheek, and red forehead, indicating inherited Liver/Heart channel issues. The mother took Jiawei Xiaoyao San plus Loulu. The child drank half a clam-shell amount daily. Nodules diminished over two months. Recurrence due to maternal anger/fever was treated with Jiawei Xiao Chaihu Tang plus Loulu, then Jiawei Xiaoyao San plus Loulu for the mother. Both recovered after two months.
A child had one nodule each front and back on the neck. At one year, maternal anger turned them red. Jiawei Xiaoyao San plus 0.5 fen Loulu for over ten doses nearly cured it. Later, severe maternal anger caused alternating chills/fever and limb spasms in both mother and child. Jiawei Xiaoyao San plus Loulu and Goutenggou (Uncariae Ramulus cum Uncis) were taken by both, ensuring safety.
A newborn had three scrofula nodules on the neck. As delivery approached, medication spread to the pre-auricular area. I diagnosed inherited Liver/Gallbladder issues; the mother’s Liver/Gallbladder pulse was surging and rapid. Since mother and child are one entity, treating the mother would cure the child. Disbelieving, they used Bixiao San (Sure-Efficacy Powder) once, causing vomiting/diarrhea, and death occurred overnight.
