Carbuncles and scabies caused by heat-toxin arise from nursing too early, or indulging in sweet and fatty foods, leading to accumulated heat in the zang-fu organs; or it may be due to the mother consuming rich and greasy food (rich and fine food), or internal fire generated by the seven emotions. One should differentiate the causes according to the affected zang-fu organs, distinguish between deficiency and excess of the disease, regulate qi and blood, and level out the excessive pathogenic factors. For example, for excess heat in the Liver channel, use Chaihu Qinggan San (Bupleurum Decoction to Clear the Liver); for deficient heat, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). For excess heat in the Heart channel, use Daochi San (Guide Out Redness Powder); for deficient heat, use Buxin Tang (Heart-Nourishing Decoction). For excess heat in the Spleen channel, use Xiehuang San (Drain the Yellow Powder); for deficient heat, use Buzhong Tang (Middle-Supplementing Decoction). For excess heat in the Lung channel, use Xiebai San (Drain the White Powder); for deficient heat, use Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder). For heat in the Kidney channel, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill). Generally, cold hands and feet indicate deficiency-cold; hot hands and feet indicate excess-heat. A pulse that is deep, rapid, and forceful, accompanied by thirst with a desire for cold drinks and dry, hard stools, indicates that the pathogen is in the interior, and internal purgation/drainage is appropriate. If the pulse is floating, rapid, and forceful, with thirst and a desire for cold drinks, this indicates that the pathogen is in the exterior, and dispersing/sweating methods are appropriate. If the pulse is floating and large but weak upon heavy pressure, or if there is thirst with a desire for warm drinks, reduced appetite for milk and food, this indicates fever due to deficiency of true qi; regulating the Spleen and Stomach will lead to spontaneous recovery. Cold-natured medicines must absolutely not be used, as they would further damage the true qi. Treatment for infants should primarily focus on regulating the nursing mother.
A child suffered from this condition in the hypochondriac region, experiencing alternating chills and fever resembling malaria, with frequent urination. This was caused by inherited Liver fire. Initially treated with Chaihu Qinggan San (Bupleurum Decoction to Clear the Liver), followed by Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder), resulting in recovery. Later, it recurred because the nursing mother's Liver fire became active. The mother and child both took Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) plus Bazhen Tang (Eight-Treasure Decoction) added with Mudanpi (Moutan Cortex) and Shanzhi (Gardeniae Fructus), and both recovered.
Another child suffered from this condition in the abdominal region, presenting with fever, hematochezia, sallow complexion, poor appetite, occasional vomiting, and diarrhea, with intermittent coldness in the hands and feet. The pulse at the right Guan position was wiry and rapid. This indicated weakness of Spleen Earth and pathology caused by Liver fire. Initially, Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) was used with Shengma (Cimicifugae Rhizoma), Chaihu (Bupleuri Radix), and Shanzhi (Gardeniae Fructus) to tonify Spleen qi and clear Liver fire; subsequently, Dihuang Wan (Rehmannia Pill) was used to nourish Kidney water to generate Liver blood, leading to recovery.
Another child suffered from this condition on the inner thigh, with redness persisting without healing, fever, and a complexion that was sometimes red and sometimes cyanotic. This indicated inherited insufficiency of Kidney yin, with flourishing fire and water imbalance. Initially, Chaihu Zhizi San (Bupleurum and Gardenia Powder) was used to clear the Liver and Heart; subsequently, Dihuang Wan (Rehmannia Pill) was used to tonify the Liver and Kidneys, resulting in recovery.
Another child suffered from this condition in the elbow region, with thirst and a desire for cold drinks. The pulse at the right Cun and Guan positions was rapid but weak. This indicated accumulated heat in the Stomach channel transmitting to the Lung channel. Initially, Xiehuang San (Drain the Yellow Powder) and Xiebai San (Drain the White Powder) were used gradually until recovery; subsequently, Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) and Siwei Fei'er Wan (Four-Ingredient Child-Nourishing Pill) were used, leading to recovery.
Another child, indulging in rich and sweet foods, first developed lesions on the shoulder blades, which later spread over the entire body with oozing. This was caused by dietary heat injuring Spleen blood. Initially, Qingwei San (Stomach-Clearing Powder) and Xiehuang San (Drain the Yellow Powder) were used, resulting in recovery. However, due to weak physical constitution and qi, Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) was administered, restoring primordial qi.
