The diseases of children all originate from heat. Why is this? It is because boys often have Kidney Fire, and girls often have Liver Fire. When the Kidney has fire, the essence becomes hot; when the Liver has fire, the blood becomes hot. Children are formed in the womb by inheriting the essence and blood of their parents; therefore, the diseases of children all stem from fetal heat (Tai Re).
The Classics state: "Heat generates wind." It also states: "Heat generates phlegm." Fetal heat combined with wind rising upward manifests as head sores, ear disorders, dental canker (Chi Gan), and night blindness (Que Mu); dispersing outward, it manifests as erysipelas (Dan Du), urticaria (Yin Zhen), and generalized fetal edema. When fetal heat combines with wind and phlegm to flow and accumulate in the head, neck, chest, back, axillae, hypochondrium, and joints, it manifests as scrofula (Luo Li), goiter (Ying Liu), rapid-progressing dental gangrene (Zou Ma), sudden fatal illnesses (Fei Qiong), phlegm syncope (Tan Jue), numbness, hemiplegia, turtle chest (Gui Xiong), and turtle back (Gui Bei). Wind, phlegm, and fire take advantage of deficiency (when Qi is deficient, pathogens gather) and aggregate within the five zang organs, manifesting as acute convulsion (Ji Jing - excess heat), chronic convulsion (Man Jing - deficiency heat), mania (Kuang - excess heat), depression/epilepsy-like syndrome (Dian - deficiency heat), epilepsy (Xian), fishing-worm convulsion (Diao), flaccid spasm (Rou Jing), rigid spasm (Jing), hernia (Shan), strangury (Lin), edema (Fu), and distension (Zhang). Thus, fetal heat transforms into six syndromes (wind, heat, phlegm, fire, dryness, dampness); these six syndromes develop into a hundred diseases, and tracing their root causes reveals they are nothing but fetal heat (regarding heat generating phlegm: excessive heat boils down body fluids to form phlegm).
Generally, children's innate qi has deficiency or excess. If qi is excess, heat is excess; if qi is deficiency, heat is deficiency. When deficiency heat is severe with much phlegm and little wind, it manifests as chronic convulsion (deficiency of qi allows heat to increase its momentum, and severe heat increases the momentum of phlegm); when excess heat is severe with little phlegm and much wind, it manifests as acute convulsion. In treating acute convulsion, clearing heat is prioritized; in treating chronic convulsion, tonifying qi and resolving phlegm is prioritized.
Convulsion presents in three forms: acute convulsion, chronic convulsion, and chronic spleen wind (eyes half-open and half-closed indicate impending spleen wind. Recurrent convulsions occurring three times transform into epilepsy).
Convulsion presents with eight signs: 1. Twitching (Chù - extension and flexion of arms and elbows); 2. Grasping (Nuo - closing of all ten fingers); 3. Pulling (Chè - motion like pulling); 4. Trembling (Chàn - trembling of head and body); 5. Arching (Fǎn - backward arching of body and head); 6. Drawing (Yǐn - hands drawn back like drawing a bow); 7. Gazing upward (Cuàn - eyes staring straight up); 8. Glancing (Shì - eyes showing whites and looking obliquely).
Twitching presents in five forms: Body heat with strong force indicates acute convulsion; body cold with weak force indicates chronic convulsion; falling down making sounds and foaming at the mouth upon waking indicates epilepsy; head looking upward indicates heavenly suspension (Tian Diao); rigid arching backward indicates spasm (Jing).
Spleen wind presents in five forms: Head is hot, but eyes are blue-white and feet are cold; head is hot, or abdomen is distended and feet are cold; head is hot, or diarrhea occurs and feet are cold; head is hot, or vomiting occurs and feet are cold; head is hot, or thirst occurs and feet are cold. These are all signs of spleen wind. (Whenever the head is hot and feet are cold, it is caused by insufficiency of true yuan, leading to rising deficiency heat).
Convulsions without sound are favorable (shun); those with sound are unfavorable (ni). Males belong to yang and wood; thus, during convulsions, looking left toward the water position (Metal/Lung position in some contexts, though traditionally Water/Kidney, here text says Lung/Metal position for sound/no sound logic based on Metal-Wood interaction) results in no sound, while looking right toward the metal position results in sound. Females are the opposite. This is due to the clash between metal and wood.
The Heart governs fright. Excess manifests as crying, screaming, fright, palpitations, shaking of hands and feet, fever, and desire to drink water. Heat in heart qi manifests as sleeping face-down. Heat-excess manifests as supine sleeping with eyes rolling up, stiff tongue unable to speak. Deficiency manifests as lethargic lying down, fright, palpitations, and restlessness.
The Liver governs wind. Excess manifests as fixed gaze, loud screaming, yawning, neck stiffness, and irritability. Severe cases present with tightness in both hypochondria, inability to move eyeballs, fright with teeth grinding, hand wandering touching clothes and objects aimlessly, and rigid body with backward arching.
The Spleen governs stagnation/lethargy. Excess manifests as drowsiness, body heat, desire to drink, red-yellow diarrhea, loss of appetite, and sleeping with eyes open. Deficiency manifests as white vomiting and diarrhea, sleeping with eyes open, or presence of phlegm.
The Lungs govern panting. Excess manifests as chest oppression, confusion, rapid panting, or thirst. Deficiency manifests in long sighing, short exhalation, and white lips.
The Kidneys are described as having no deficiency in this specific context listing symptoms, but manifestations include: Excess manifests as lack of luster in eyes, photophobia, heavy bones/joints. Deficiency involves depletion of spirit and qi, excessive whites in eyes, fontanelles remaining open or splitting, and pale complexion. Kidney deficiency manifests as downward sinking of essence, or cold shins with hot feet, and dislike of covering with clothes.
Transmission among the five zang organs is always caused by phlegm. Phlegm is the sprout of wind. When fire is quiet, it hides in the Spleen; when fire moves, it obstructs the Spleen. The intersection of phlegm and fire produces convulsion; stagnation and knotting produce epilepsy and fishing-worm convulsion; coming and going produce green diarrhea. All these arise from spleen dampness. (Bao Yuan Tang, added with Bai Shao Yao [Radix Paeoniae Alba] to nourish blood, serves as the guiding herb. This is a nourishing formula for chronic convulsion.)
