Postpartum chills and fever alternate, occurring daily at the same time. The symptoms resemble malaria (Nüe Ji), but it must not be treated as malaria. This is because deficiency of Qi and Blood causes alternating chills and fever, and deficiency of Yuan Qi allows external pathogens to invade. Symptoms may include severe cold, extreme heat, being lighter during the day and heavier at night, or chills and fever in the late afternoon. These conditions closely mimic malaria syndrome. Treatment should focus on nourishing Ying Blood and tonifying Yuan Qi to resolve the chills and fever. If sweating is present, it should be stopped immediately, possibly by adding herbs like Mahuang Gen (Ephedrae Caulis); if sweating is limited only to the head and does not extend to the feet, this indicates a critical condition of isolated Yang and exhausted Yin, requiring the addition of Dihuang (Rehmanniae Radix Preparata) and Danggui (Angelicae Sinensis Radix). If there are Yangming channel signs without aversion to cold, with headache and no sweating, Shenghua Tang (Life Return Decoction) should be administered, augmented with Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), and several stalks of Congbai (Allii Fistulosi Bulbus) to disperse the pathogen. Formulas such as Chaihu Qinggan Yin (Bupleurum Liver-Clearing Drink) and herbs like Changshan (Polygalae Radix) and Caoguo (Tsaoko Fructus) must not be used.
Zirong Yangqi Fuzheng Tang (Nourishing Blood and Qi to Support Righteousness Decoction): Treats postpartum chills and fever with sweating, occurring at the expected time in the afternoon.
Renshen (Ren Shen, Ginseng Root) (2 qian), Zhi Huangqi (Astragali Radix Preparata), Baizhu (Atractylodis Macrocephalae Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), Shudi (Rehmanniae Radix Preparata), Maidong (Ophiopogonis Radix), Mahuanggen (Ephedrae Caulis) (each 1 qian), Danggui (Dang Gui, Angelicae Sinensis Radix) (3 qian), Chenpi (Citri Reticulatae Pericarpium) (4 fen), Zhi Cao (Glycyrrhizae Radix et Rhizoma Preparatum) (5 fen), Jujubae Fructus (Jujube), decocted in water.
Verse: Postpartum chills and fever come frequently, arriving on schedule with sweat pouring like rain.
Though resembling malaria, do not treat it as such; the cause is dual deficiency of Qi and Blood.
Use Baizhu and Renshen to nourish blood and support Qi, along with Danggui, Chuanxiong, Shudi, and Chenpi.
Maidong and Zhi Cao with Jujubae Fructus serve as guides; add Mahuanggen if sweating occurs.
Jiajian Yangwei Tang (Modified Stomach-Nourishing Decoction): Treats postpartum alternating chills and fever, headache with no sweating, resembling malaria.
Zhi Cao (Prepared Licorice Root) (4 fen), Bai Fuling (Poria) (1 qian), Banxia (Pinelliae Rhizoma Preparatum) (8 fen, processed), Chuanxiong (1 qian), Chenpi (4 fen), Danggui (2 qian), Cangzhu (Atractylodis Rhizoma), Huoxiang (Agastaches Herba) (4 fen), Renshen (1 qian). Decoct with fresh ginger juice as a guide.
If phlegm is present, add Zhuli (Bambusae Succus), Jiangzhi (Zingiberis Recens Juice), Banxia, and Shenqu (Massa Medicata Fermentata); for weak patients, also administer Heche Wan (Placenta Hominis Pill). For chronic malaria that does not resolve, take Shen Zhu Gao (Ginseng and Atractylodes Paste) to assist the medicinal power.
Shen Zhu Gao (Ginseng and Atractylodes Paste):
Take one jin of Baizhu, soak in rice washing water overnight, chop and roast. Take one liang of Renshen, add six bowls of water, decoct to two bowls, decoct twice more, collecting a total of six bowls of liquid. Combine all juices, then further reduce to one bowl. Take half a wine cup dissolved in rice gruel on an empty stomach.
Verse: Postpartum headache with body devoid of sweat, alternating chills and fever is the source of illness.
Chuanxiong, Danggui, Fuling, Huoxiang, Cangzhu, and Banxia, decocted with Zhi Cao, Chenpi, and Renshen.
For phlegm, add Zhuli, Jiangzhi, Banxia, and Shenqu; weak patients also take Heche Wan.
For unresolved chronic malaria, take Shen Zhu Gao, dissolved in gruel on an empty stomach, half a wine cup.
