In cases where bowel movements are obstructed for a month following a prolonged illness, there is no need to be impatient. One should merely tonify the True Yin (Zhen Yin), so that sufficient essence naturally generates blood; when blood is abundant, it moistens the intestines, and bowel movements will occur spontaneously. It should not be regarded as a condition requiring urgent resolution to hasten recovery. The prescription is as follows: Shu Di Huang (Prepared Rehmanniae Radix, 1 liang), Yuan Shen (Scrophulariae Radix, 1 liang), Dang Gui (Angelicae Sinensis Radix, 1 liang), Chuan Xiong (Ligustici Chuanxiong Rhizoma, 5 qian), Tao Ren (Persicae Semen, 10 grains), Huo Ma Ren (Cannabis Fructus, 1 qian), Da Huang (Rhei Radix et Rhizoma, 1 qian), Hong Hua (Carthami Flos, 3 fen), Honey (half cup). Decoct in water and take orally.
The brilliance of this formula lies in using Shu Di Huang, Yuan Shen, and Dang Gui to generate Yin-blood, with small amounts of Huo Ma Ren and Da Huang added to moisten the intestines and promote downward movement. This addresses the root cause of the obstruction rather than hastily forcing evacuation.
Obstruction of urination is a disease of the Bladder (Pang Guang). If the Qi transformation (Qi Hua) of the Bladder fails, urine cannot be excreted. Although urinary retention involves the meridians of the Bladder, the treatment method does not lie primarily in treating the Bladder itself. The prescription is as follows: Ren Shen (Ginseng Radix, 3 qian), Lian Zi (Nelumbinis Semen, 3 qian), Fu Ling (Poria, 3 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma, 1 qian), Rou Gui (Cinnamomi Cortex, 1 qian), Bai Guo (Ginkgo Semen, 2 qian), Che Qian Zi (Plantaginis Semen, 1 qian), Wang Bu Liu Xing (Vaccariae Semen, 1 qian). Decoct in water and take orally.
The brilliance of this formula lies in the use of Ren Shen and Rou Gui. The Bladder requires Qi transformation to excrete urine. What is Qi transformation? It is the Qi of the Pericardium Collateral (Xin Bao Luo). The Bladder must receive the descending Qi of the Pericardium Collateral for the water passages to flow freely. Having used Ren Shen and Rou Gui, Qi transformation is activated. Particularly ingenious is the use of Bai Guo, an aspect often misunderstood. Bai Guo unblocks the Ren Mai (Conception Vessel) and Du Mai (Governor Vessel) and acts upon the Bladder (guiding the Qi of Ren Shen and Rou Gui directly into the Bladder). Moreover, since Che Qian Zi and Wang Bu Liu Xing are herbs that promote downward drainage, would they not facilitate smooth urination from the anterior orifice?
For obstruction of urination, another prescription is used: Shu Di Huang (Prepared Rehmanniae Radix, 1 liang), Shan Yu Rou (Corni Fructus, 4 qian), Dan Pi (Moutan Cortex, 1 qian), Shan Yao (Dioscoreae Rhizoma, 1 qian), Rou Gui (Cinnamomi Cortex, 1 qian), Che Qian Zi (Plantaginis Semen, 1 qian), Ze Xie (Alismatis Rhizoma, 1 qian), Fu Ling (Poria, 2 qian). This formula is essentially Liu Wei Di Huang Tang (Six-Ingredient Pill with Rehmannia) modified to Qi Wei Di Huang Tang (Seven-Ingredient Kidney-Nourishing Decoction). Its brilliance lies not in promoting diuresis but in specifically treating Kidney Water. When there is sufficient Water in the Kidneys, the Qi transformation of the Bladder functions naturally. The opening and closing of the Bladder are governed by the Kidneys.
