A female servant of Dairen (Zhang Congzheng) came from the road in winter, her face red as fire. Upon arrival, she suffered severe pain in the waist and hips, accompanied by abdominal urgency and tenesmus (a feeling of incomplete defecation with straining). When the pain occurred, she hallucinated, seeing ghosts and spirits. Dairen said, "This is a Shaoyang meridian disease; located on the side of the body, it belongs to Ministerial Fire (Xianghuo). Have him take Zhouche Wan (Boat and Cart Pill) and Jingsan (Meridian-Scattering Powder), resulting in several basins of diarrhea, yet the illness did not subside." People were puzzled, believing it was caused by malevolent spirits. Dairen angrily shouted, "It is an ass-spirit!" He then ordered the preparation of Tiaowei Chengqi Tang (Stomach-Regulating Purge-Decoction), two liang, mixed with one liang of the head-end powder of Qian Niu Zi (Pharbitis Seed), decocted together and administered. The patient had dozens of bowel movements, expelling approximately one or two cans of material, after which he could discard his walking stick. However, he felt thirsty. Dairen allowed him to drink freely and consume watermelon, pear, and persimmon. Dairen stated, "In treating Fire pathogenic factors, nothing compares to Ice. Water is the ultimate Yin substance of heaven and earth." After drinking about one or two buckets of water, slight pain persisted. Only then did Dairen needle Yanglingquan (GB34, Gallbladder 34), a point of the Foot-Shaoyang Gallbladder Meridian, to unblock stagnation. From then on, the patient calmed down. The servant later explained, "This illness requires purging five to seven times annually. Since I did not purge this year, the condition manifested thusly." Chang Zhongming understood her words; having dampness pathology himself, he experienced over ten bowel movements annually, after which his illness ceased. Such matters can be discussed with the wise, but are difficult to explain to the foolish.
An old man, aged sixty, suffering from heavy and harassing corvée labor, suddenly developed manic psychosis. He felt as if insects were crawling in his mouth and nose, scratching his hands incessantly for years without relief. Dairen palpated both wrists and found the pulses surging and large like ropes. He diagnosed, "The mouth is called Feimen (Flying Gate), and the stomach is called Benmen (Pyloric Gate)." He further explained, "The mouth is the upper source of the stomach. The nose is where the Foot-Yangming Stomach Meridian originates; it starts at the lateral aspect of the nose, intersects at the root of the nose, receives Qi from the Taiyang Bladder Meridian, descends along the nasal bridge, intersects at Renzhong (GV26), circles the lips, and intersects at Chengjiang (CV24). Hence, his symptoms manifest this way. Heavy and harassing labor generates Fire transformation. When Fire invades the Yangming Meridian, mania ensues. Thus, the *Neijing* states: 'Yangming diseases present with climbing high and singing, discarding clothes and running, cursing without distinguishing relatives or strangers.' Moreover, the Liver governs planning, and the Gallbladder governs decision-making. Under urgent pressure, resources may be insufficient, leading the Liver to plan repeatedly while the Gallbladder fails to decide. Frustration has no outlet, anger cannot be vented, causing Heart Fire to surge and oppress, subsequently invading the Yangming Meridian. Although the Stomach belongs to Earth, the Liver belongs to Wood, and the Gallbladder houses Ministerial Fire; Fire follows Wood Qi into the Stomach, resulting in sudden mania." Dairen instructed placing the patient in a warm room, inducing sweating through emesis therapy (Yongfa), repeating this three times. As the *Neijing* says, 'When Wood is stagnant, disperse it; when Fire is stagnant, vent it.' This precisely applies here. He also prescribed half a jin of Tiaowei Chengqi Tang (Stomach-Regulating Purge-Decoction), boiled in five sheng of water until half-boiled, divided into three doses. This resulted in massive purgation of twenty movements, expelling several sheng of blood, water, and stasis. Recovery followed. Finally, Tongsheng San (Universal Benefit Powder) was used for post-treatment regulation.
