一二日即能致命,故名走马,言其骤也,此症有五不治∶(一)舌根烂不能食者不治;(二)黑腐如筋者不治;
The area under the tongue of a child, near the root of the tongue, swells in a shape resembling the tongue itself; hence it is named *Chong She* (Double Tongue, a swelling under the tongue resembling a second tongue). This condition also arises from heat in the Heart and Spleen. Ancient methods primarily advocated internal administration of *Qing Re Yin* (Heat-Clearing Decoction, composed of Huanglian, Shengdi, Mutong, Sheng Gancao, Lianqiao, Lianzi, and Dan Zhuye as guides), decocted in water and administered frequently into the mouth. Externally, *Liang Xin San* (Cool-the-Heart Powder, composed of Qingdai, Pengsha, Huangbai, Huanglian, Renzhongbai, Fenghua Xiao, and Bingpian) was ground into an extremely fine powder and applied to the sublingual area, which proved effective.
[True's Commentary] *Chong She* results from heat in the Heart and Spleen. However, the great collateral of the Spleen emerges from under the tongue; when heat is present, both Qi and Blood become abundant, causing a "second tongue" to regenerate. This belongs to excess heat in the Upper Jiao. Internally, one may decoct Huanglian, Sheng Gancao, Dan Zhuye, Niubangzi, and Dengxincao for ingestion. Externally, take one black date, remove the pit leaving only the flesh, insert one qian of Qingfan into the flesh, wrap it in paper, and roast until hot. Discard the date flesh; use only two fen of Qingfan mixed with water, applying it several times with a brush until it subsides.
For a child with a swollen tongue, simply apply Meihua Bingpian (Plum-blossom Borneol) or dip paper in Castor Oil (Ricini Oleum), ignite it, and fumigate with the smoke, after which the swelling will immediately subside.
If a child suffers from continuous bleeding from the middle of the tongue, mix equal parts of Octopus Bone (Haima Gu / Sepiae Endoconcha) and Pollen Typhae (Puhuang), grind them into a fine powder, and apply it to the tongue to cure.
For a child with ulcerated tongue making eating difficult, first grind Evodiae Fructus (Wuzhuyu) into powder, mix with good vinegar, and apply to the soles of both feet, changing it after one shichen (two hours). Then, mix Renzhongbai, Ercha, Qingdai, Pengsha, Bohe, Xuanmingfen, Maibo (Ma Bo), and Bingpian into an extremely fine powder, and sprinkle it on the lesion for immediate effect.
For a child with a stiff tongue covered by a white membrane (*She Sheng Yi*), use real Rhinoceros Horn (Xiniu Huang), Cinnabar (Zhusha), and Yuanjingshi, grind them into a fine powder, and frequently apply to the tip of the tongue to heal.
For red blisters appearing on a child's tongue, this condition differs from throat diseases. Use one liang of Cnidii Fructus (Shechuangzi), burn it in a jar, and have the child inhale the smoke into the throat, which will naturally dissipate.
When a child's gums initially emit a foul odor, followed by blackening of the teeth, and eventually rotting of the gum tissue with bleeding, it is named *Ya Gan* (Gum Gangrene). This is caused by Kidney heat rushing directly to the Upper Jiao, hence metaphorically referred to as "Running Horse" due to its rapid progression. Ancient methods primarily advocated internal administration of *Hangxie Dan* (see formula above) and external application of *Rusheng San* (Like-Saint Powder, composed of Renzhongbai, Tonglv, and Shexiang ground into fine powder). First, wash the area clean with rice water infused with Wax Leaf (Laye), then apply this medicine.
[True's Commentary] Children often suffer from this after febrile diseases or smallpox/rubella, where fire-toxins flow into the Stomach meridian, causing this affliction. The condition is critical; severely, it leads to tooth loss, cheek perforation, and nasal penetration, potentially fatal within one or two days. Hence it is called "Running Horse," indicating its swiftness. There are five untreatable signs: (1) Ulceration at the tongue root preventing eating; (2) Blackish decay resembling sinews; (3) Floating white flesh indicating stomach rot; (4) Tooth loss, cheek perforation, and unbearable nasal odor; (5) Red spots appearing on the Shan Gen (root of the nose). One must act quickly at the onset: use three qian of raw Dahuang, ten Dingxiang, and three qian of Lüdou, grind into powder, and apply with hot vinegar to the soles of both feet. Then, prepare a mixture of greenish-green Lvfan, Renzhongbai, Xionghuangjing, real Shexiang, and top-grade Meihua Bingpian, grinding them into an extremely fine powder. First, use a silver needle to puncture and scrape away necrotic tissue and purple blood, then apply the powder. Only after toxic blood is expelled can the patient be cured.
Furthermore, if this condition is accompanied by leg swellings resembling cloud patterns, varying in color (red or blue), size, pain, hardness, and difficulty walking, the swollen areas on the legs must be pricked with a magnetic edge to bleed, followed by applying slices of beef, changed several times daily, which yields significant efficacy.
