(16) Hoarseness in variola (smallpox/rubella-like rash) is mostly caused by lung and stomach heat pathogens failing to fully reach the exterior, with severe coughing damaging the throat. Treatment follows the aforementioned method. If hoarseness is accompanied by sore and swollen throat, remove Ophiopogonis Radix (Maidong) and Polygonati Odorati Rhizoma (Yuzhu), and add preparations such as Arisaematis Cum Bile (Shandougen) and Belamcandae Rhizoma (Shegan).
(17) Convulsions during variola are invariably accompanied by phlegm rattle in the throat, indicating a syndrome of phlegm-heat accumulating in the Pericardium. Use Qingre Touji Tang (Clear Heat and Unblock Muscle Decoction) plus Trichosanthis Semen (Gualouren) and Bambusae Folium (Zhuye). If there is no phlegm rattle, or if convulsions occur with biting, this indicates a fatal syndrome of righteous qi deficiency and pathogen sinking inward.
(18) Cracking of lips and tongue during variola is caused by fire of the Heart and Spleen rushing upward. The color must be deep crimson. If seen at the initial onset or when the rash is erupting, use Baihu Tang (White Tiger Decoction) plus Scutellariae Radix (Huangqin), Coptidis Rhizoma (Huanglian), Scrophulariae Radix (Xuanshen), Schizonepetae Herba (Jingjie), and Menthae Haplocalycis Herba (Bohe). If seen after the rash has subsided, it means both Heart and Spleen are attacked by toxins, which is a fatal syndrome.
(19) Teeth grinding during variola is caused by stomach heat. It is often accompanied by thirst, heat in hands and feet, and preference for cold water. Add herbs that clear stomach heat and nourish lung yin according to the syndrome. If hands and feet are not hot, or even cold, with preference for hot drinks and teeth grinding, this is likely a critical syndrome of excess yang repelling yin. Hematochezia, sore throat, and phlegm rattle will follow, leading to death. Even with Baihu Tang, rescue is difficult.
(20) Chest distension and rapid breathing during variola are caused by excessive lung heat leading to lung qi distension. This is a deteriorating syndrome of the Lung Meridian and is often incurable.
(21) Loss of appetite during variola is due to excessive evil heat in the stomach; when heat fills the space, food cannot be accommodated. Do not force feeding. Even if no food is taken for several days, as long as rice soup can be consumed in small amounts, there is no major harm. At the initial eruption stage, use Baihu Tang plus Jingjie. Appetite returns once the rash clears and heat dissipates. If anorexia persists after recovery, clear stomach heat; if yuan qi is depleted, tonify and supplement.
(22) Cold body during variola is a very ominous sign. Since variola belongs to yang pathogens, a yang syndrome presenting with yin signs is called a counter-flow syndrome. Use penetrating agents combined with warming and nourishing spleen and stomach herbs. If fever returns and the rash emerges after medication, it is treatable. However, if internal heat is excessive but yin signs appear externally, large doses of cold-cooling herbs are needed; careful differentiation is essential.
(23) People generally know that failure of the rash to emerge is dangerous, but do not realize that danger exists even after emergence. This is due to unresolved internal heat or damage to righteous qi after pathogen elimination. Therefore, neglect should be avoided when the rash is about to subside.
(24) In observing variola, spots usually first appear behind the ears, on the top of the head, or in the lumbar region. The spots are pointed but not elongated, small, and uniform. Abundance on the head/face and chest/back is a favorable prognosis (Shun); abundance on limbs with absence on head/face is unfavorable (Ni). Infants may have fewer spots; medication is not necessary.
Attached Formulas
I. External Treatments
(1) For variola not emerging with severe asthma: Soak Sesame seeds (Zhimah/Humazi) in boiling water, place in a wooden basin inside a tent, and steam the head and face while hot to induce eruption.
(2) For slow emergence of variola/pox: Chop Coriandri Fructus (Huxu) finely. Boil in two cups of good wine for one or two boils, add Coriander, boil briefly, let cool slightly. Inhale a large mouthful and spray gently from neck to foot evenly, avoiding the face.
(3) For variola hindered by fog/dew/cold-wind causing chest oppression and hidden eruption: Wrap dried elephant dung in rough paper and burn to smoke; or use dried pig manure.
(4) For slow emergence of variola: Boil millet with husk and steam with the hot vapor; highly effective.
(5) For variola not emerging: Burn Agallochum (Chenxiang), Aucklandiae Radix (Muxiang), and Santalum Album (Tánxiāng) in a large basin, hold the child over the smoke to induce eruption.
II. Internal Medications
(1) Prevention of variola: Sun-dry one loofah fruit. On New Year's Eve, sweep ash from new roof tiles, spread on ground to remove fire energy, grind to powder. Take with boiling water annually. After 3-4 courses, the child will never suffer from variola.
(2) For critical cases of variola not emerging: Seal four jin of cherries in a porcelain jar, bury in soil for 2-3 months until liquefied. Take one cup of this juice, slightly warmed, for critical cases; highly effective.
(3) For variola with deficient exterior and solid muscle texture preventing penetration: Decoct Congbai (Scallion White) strongly and administer frequently. Resolution occurs with slight sweating.
(4) For early variola: Decoct Imperatae Rhizoma (Maogen) or Phragmitis Rhizoma (Lugen) and drink regularly.
(5) Early Variola Formula (Self-made): Treats fever, oppression, dry mouth with tears.
