After heatstroke with excessive sweating, the limbs are cold and the pulse is faint; respiration is shallow and spirit is fatigued, resembling a state of collapse. The urine is yellow and red, indicating dampness retention leading to Yang Qi depletion. It is appropriate to use Guizhi Jianzhong Tang (Cinnamon Twig Decoction for Building the Middle), adding Chifuling (Poria Cocos with Red Skin), Cheqianzi (Plantago Seed), Huashi (Talcum), and Bixie (Dioscorea Hypoglauca) among other ingredients, to protect Yang Qi and promote diuresis to eliminate dampness.
Excessive sweating with cold limbs, faint pulse, and spiritual fatigue indicate that the signs of Qi depletion are prominent. However, the yellow-red urine clearly reveals the pattern of internal accumulation of damp-heat. Therefore, Guizhi Jianzhong Tang is used to protect Yang Qi, combined with Cheqianzi, Chifuling, Huashi, and Bixie to clear damp-heat. This achieves the subtlety of clearing both pathogenic factors and healthy Qi simultaneously. If one merely supports and tonifies throughout, damp-heat will become more vigorous; thus, sedation is required.
If after treating heatstroke with sweating-inducing and cooling methods, there is no resolution after ten days, it gradually leads to mental confusion, muteness, loss of appetite, pale complexion, cold limbs, a pulse fine as a thread, and a stiff body resembling death. This indicates the pathogen has entered the Jueyin Liver Channel and is mixed with stagnant blood. It is appropriate to use Wu Keke's Sanjia San (Three Shells Powder) added with Chaihu (Bupleurum Root), Jiangcan (Bombyx Batryticatus), Guangyujin (Curcuma Aromatica Rhizome), Xian Changpu (Fresh Acorus Tatarinowii Rhizome), Liaoqiao Xin (Heart of Forsythia Fruit), Xuanshen Xin (Heart of Scrophularia Root), and Zixue Dan (Purple Snow Pill) among other ingredients, to vent the pathogen from the blood level to the exterior.
This is a case where pathogenic heat enters the collaterals and mixes with blood, which cannot be resolved by simple cooling or purgative methods. Therefore, it is appropriate to use Tu Chao Shan Jia (Earth-fried Pangolin Scales), Cu Chao Bie Jia (Vinegar-fried Turtle Shell), and Jiu Jin Di Bie Chong (Wine-soaked Ground Beetle), allowing them to penetrate deeply into the collaterals to attack latent pathogens. Combined with Chaihu, Jiangcan, and Zixue Dan, they soothe, vent, and unblock, guiding the pathogen from the interior to the exterior. Proper application can save lives in desperate cases; otherwise, there is hardly any remedy.
Heatstroke causing sudden collapse, unconsciousness, drooling at the mouth corners, closed eyes, and relaxed hands indicates that heat has clouded the Heart Spirit and Yin fluids fail to ascend. It is appropriate to crush several cloves of garlic, extract the juice, mix it with vinegar, and administer it orally, while moving the patient to a cool place, upon which they will regain consciousness. Subsequently, it is appropriate to use Yangshen (American Ginseng), Maidong (Ophiopogon Tuber), Lianzi (Lotus Seed), Zhuye (Bamboo Leaf), Fresh Acorus Tatarinowii Rhizome, Yuanzhi (Polygala Root), Huanglian (Coptis Rhizome), and Yiyuan San (Benefit Origin Powder) among other ingredients to clear the Heart and calm the Spirit.
During summer when heat steams and rises, if one engages in heavy labor or travels long distances under the scorching midday sun, sudden collapse, unconsciousness, closed eyes, and relaxed hands may occur. In such cases, one must not immediately irrigate with cold water, lest the heat attacks the Heart and causes death. It is appropriate to crush several cloves of garlic, extract the juice, mix it with vinegar, and administer it orally. Then move the patient to a cool place, scoop up hot earth from the road with hands, surround the navel with it, and have someone urinate into the navel; the patient will then regain consciousness. After regaining consciousness, it is appropriate to clear the Heart and calm the Spirit by administering Yangshen, Maidong, Huanglian, Acorus Tatarinowii Rhizome, Yuanzhi, Huanglian, Zhuye, and Yiyuan San. Upon taking these medicines, when the spirit clears, recovery is achieved.
