此发明伏邪之异于新邪。既无头痛身疼,则表邪甚微;
Fu Shu (Summerheat-Latent Disease) presents with slight aversion to cold, fever, vomiting, nausea, and diarrhea, along with a sensation of stuffiness in the wrists and a white tongue coating. This indicates the stirring up of latent pathogens within. Huoxiang Zhengqi San (Agastache Powder for Rectifying the Qi) should be used to disperse stagnation and promote diuresis to resolve dampness.
This passage elucidates the distinction between latent pathogens and newly acquired external pathogens. Since there is no headache or body ache, the exterior pathogenic factor is very slight; whereas the aversion to cold, fever, wrist stuffiness, vomiting, and diarrhea are manifestations of the stirring up of latent pathogens, clearly indicating that there is no new external pathogen inducing them.
If one does not rely on herbs such as Huoxiang (Patchouli Herb), Houpo (Magnolia Bark), Ziye (Perilla Leaf), Baizhi (Angelica Dahurica Root), Chenpi (Tangerine Peel), Fupi (Areca Peel), Banxia (Pinellia Tuber), and Shenqu (Medicated Leaven) to dispel turbidity and promote venting and draining, there is a risk that the pathogen will fail to be vented and discharged, leading to its spread and transmission.
In Fu Shu, if the heat fails to resolve, accompanied by cough, air rebelling upwards, desire to vomit, irritability, stuffiness, and profuse diarrhea, this indicates the latent pathogen permeating the Triple Burner (San Jiao). Su Zi Jiang Qi Tang (Perilla Seed Decoction for Descending Qi) combined with Liu Yi San (Six-to-One Powder), plus Tongcao (Akebia Stem) and Huangqin (Scutellaria Root), should be used to unblock and drain the Triple Burner.
This refers to the situation where heat fails to resolve, and the latent pathogen transmits and lingers in the Lung and Stomach, causing cough and desire to vomit; it lodges in the Middle Burner, causing wrist stuffiness and discomfort; it rushes downward, causing profuse diarrhea. Since the pathogen has permeated the Triple Burner, with conflict between the upper and lower parts, Su Zi Jiang Qi Tang is used to diffuse the Lung and Stomach, while Liu Yi San, Tongcao, and Huangqin separate and direct the water passages. Thus, the upper, middle, and lower Triple Burners can be simultaneously unblocked and drained, preventing the retention of pathogens and the occurrence of complications, which is the essence of the treatment.
In Fu Shu, if the heat gradually intensifies, accompanied by cough, air rebelling upwards, insomnia, stabbing pain in the chest and hypochondrium, excessive phlegm, and a white tongue coating, this indicates phlegm stagnation blocking the Lung collaterals and the failure of Lung Qi to descend. Xuanfuhua (Inula Flower), Xinjiang (Safflower/Red Dye - *Note: Text says 'Xinyi' but context implies Xinjiang/Xincang based on Jin Gui Yao Lue*, here translated as Xinjiang/Safflower for accuracy to likely intent or kept as transliteration if strict), Zhike (Bitter Orange Fruit), Jiegeng (Platycodon Root), Sangbaipi (Mulberry Root Bark), Yiren (Coix Seed), Suzi (Perilla Seed), Jiangxiang (Sandalwood), Pipaye (Loquat Leaf), Lugeng (Reed Rhizome), and Huashi (Talcum) should be used to descend Qi and unblock the collaterals.
The previous paragraph discussed heat failing to resolve with the pathogen spreading through the Triple Burner; this paragraph discusses heat failing to resolve with the pathogen coagulating with phlegm, blocking the Lung collaterals. If not regulated and unblocked, it may lead to chronic Wei Bi (flaccidity and obstruction) and pain. Therefore, Xuanfuhua Tang (Inula Flower Decoction) from the *Jin Gui Yao Lue* (Essential Prescriptions of the Golden Cabinet) is employed. By using Xinjiang (Safflower) and Qingcong (Green Onion Bulb), it first unblocks the Qi aspect and secondly unblocks the Blood collaterals. Additionally, Zhishi (Immature Bitter Orange), Jiegeng, Sangbaipi, Suzi, and Jiangxiang open the Lung and descend Qi, while Yiren, Pipaye, Lugeng, and Huashi, being sweet and bland in nature, clear heat and drain dampness.
