Amenorrhea (absence of menstruation) may result from miscarriage and multiple childbirths damaging the blood, chronic tidal fever consuming the blood, long-term night sweats depleting the blood, spleen and stomach disharmony with poor appetite leading to insufficient blood generation,
dysentery or wind injury causing blood loss, or emotional disturbances affecting the heart qi, causing stagnation and knotting, thus blocking blood flow. Treatment should focus on generating blood, nourishing blood, and regulating blood.
[Main Formula Principle]
When menstrual blood coagulates and qi stagnates, obstructing circulation, the upcoming menses may appear purple or pale red. Post-illness excessive sweating and overthinking damage the blood, or it may be due to
damp-heat combined with cold and wind. For stagnant and subtle qi, promote qi movement; for tight cold and blood stasis, unblock and circulate. Use Siwu Tang (Four Substance Decoction) added with Areca Seed (Binglang), Aconite (Fuzi), and Curcuma Zedoaria Rhizome (Ezhushu), Peach Kernel (Taoren), Achyranthes Root (Niuxi), Cinnamon Twig (Guizhi), and Blister Beetle (Mengchong). If the pulse is wiry and soggy indicating qi deficiency, use Ginseng (Renshen) and Atractylodes Macrocephala Rhizome (Baizhu);
Phellodendron Bark (Huangbai), Biancao, Moutan Cortex (Mudanpi), and Ligusticum Chuanxiong Rhizome (Chuanxiong). If there is stomach fire causing dryness and depletion, using Rhubarb (Jiangjun, Da Huang) assisted by wine is highly effective.
(Benglou / Metrorrhagia and Metrostaxis) all arise from labor-induced damage to qi and blood, injuring the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel). With both qi and blood deficient, they cannot restrain the menstrual blood, resulting in sudden profuse bleeding or gradual dripping. Generally, women suffer from unsettled minds or lack of sexual activity, leading to loss of essence among the nobility. Transitioning from wealth to poverty often results in flaming-up of heart fire; treatment should involve persuasion and counseling, along with herbs that settle and descend heart fire.
[Main Formula Principle]
Metrorrhagia and metrostaxis stem from qi and blood injury, presenting with a wiry, large, thin, and long pulse. Li Dongyuan stated that sinking does not imply heat, a point students must ponder carefully. Zhu Danxi often discussed deficiency and damp-heat, detailing that vigorous yin pulse with weak yang indicates pathology. If the pulse is wiry and rapid, clear heat; Jingjie (Schizonepeta Spike) and Scutellaria Root (Huangqin) are beneficial before medication takes full effect. Baimaoshuang (Soot from Chimney) powdered and combined with Siwu Tang (Four Substance Decoction); for severe pain, add Yanhusuo (Corydalis Rhizome) stir-fried black and Ginger (Shengjiang). If the pulse is soggy and qi is deficient, use Renshen (Ginseng) and Baizhu (Atractylodes Macrocephala Rhizome); Xuanduan (Dipsacus Root) and Huangqin (Scutellaria Root) have strong effects. For prolonged metrorrhagia with sinking, lift and raise; nourish blood, regulate spleen, and drain the two yang channels.
Red corresponds to blood, white corresponds to qi. Pathogenic damp-heat causes leukorrhea and discharge, which are essentially phlegm accumulations in the stomach flowing downward, seeping into the bladder, appearing thick and sticky. Another type resembling white soup is called Bai Zhuo (White Turbidity). Treatment prioritizes drying dampness and employing lifting methods; in severe cases, lift the qi. Dietary restrictions should include avoiding rich flavors.
[Main Formula Principle]
The root cause of leukorrhea is damp-heat requiring regulation; imbalance between Chi Rong (Nourishing Blood) and Bai Wei (Defensive Qi). Onset brings soreness and pain in the waist and knees; excessive discharge leads to bone dryness like dead wood. A wiry and slow pulse indicates wind injury; take Stir-fried Ginger Powder to cut off the source. Fulonggan San (Dragon's House Earth Powder) is highly effective; if the pulse is rapid, Chugen (Bignonia Root Bark) yields even better results.

Dragon's House Earth (Fulonggan)
Mugwort Leaf (Aiye)
Ligusticum Chuanxiong Rhizome (Chuanxiong) (each 1.5 qian)
Red Halloysite (Chishizhi) ()
Ophiopogon Tuber (Maidong, heart removed)
Ginger (Ganjiang, roasted)
Angelica Sinensis Root (Chuan Gui)
Cinnamon Bark (Rougui)
Licorice Root (Gancao)
Prepared Rehmannia Root (Shudi) (each 0.3 qian)
Decoct with dates and take orally, or grind into powder and take with rice wine.
