Postpartum obstruction of both urination and defecation occurs because the intestines and stomach originally harbored heat pathogens. After childbirth, fluids and blood descend together, leading to exhaustion of body fluids, dryness and astringency of the intestines and stomach, and accumulation of heat causing dryness and impaction, thus resulting in obstruction. In some cases where postpartum patients suffer from this condition, drinking human or cow's milk relieves the symptoms; therefore, treatment is best achieved by tonifying the Kidney, as the Kidney governs the five fluids and opens into the two lower orifices (urethra and anus).
Postpartum urinary incontinence is caused by insecurity of Kidney Qi. It should be treated with Wuweizi Wan (Schisandra Berry Pill). If Spleen and Kidney are weak, Buzhong Yiqi Tang (Decoction for Boosting the Middle and Augmenting Qi) should be administered to deliver Huanshao Dan (Return to Youth Pill). If Spleen and Kidney are deficient and cold, Bawei Wan (Eight-Ingredient Pill) and Sishen Wan (Four Spirits Pill) should be used as adjuvants.
Postpartum hematuria (blood in urine) occurs due to deficiency of Qi and Blood allowing heat pathogens to invade. When blood is affected by heat, it flows and seeps into the uterus, causing blood to exit with urine. Some parturient women present with hematuria, sallow complexion, hypochondriac distension, and poor appetite; this indicates Liver Wood overacting on Spleen Earth. They can be cured by concurrently administering Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Buzhong Yiqi Tang. Postpartum hematochezia (blood in stool) may result from improper diet and lifestyle, or from excessive exposure to the Six Excesses (Wind, Cold, Summerheat, Dampness, Dryness, Fire) and Seven Emotions (Joy, Anger, Worry, Pensiveness, Sadness, Fear, Fright), leading to depletion of Yuan Qi (Original Qi) and injury to Yin collaterals. If caused by accumulation of heat from rich food and alcohol damp-toxins, clearing heat is indicated. If caused by anger stirring Liver Fire, stagnation injuring the Spleen, or overthinking injuring the Heart, harmonizing the Liver and regulating the Heart and Spleen is indicated. For Wind-Fire and Blood-Heat in the Large Intestine, cooling Blood and dispelling Wind is indicated. For weakness of the intestines and stomach with sinking of Yuan Qi, major tonification combined with lifting is indicated. Moreover, after the severe deficiency of Qi and Blood following childbirth, if one suffers again from collateral injury and blood loss, how can one not urgently consolidate the Middle Qi of the Spleen and Stomach to serve the function of controlling and retaining blood?
Around half a month after new childbirth, sudden carbuncles and swellings appearing on the limbs, chest, and abdomen indicate that lochia (decayed blood) is incomplete, flowing and stagnating in the collaterals, or that Qi and Blood are weak, causing Ying Qi (Nutritive Qi) to fail to follow its course and reverse into the muscle tissue. If caused by lochia stasis, symptoms include swelling, redness, pain, and a wiry, surging, and forceful pulse; treatment should focus on nourishing and moving Blood, assisted by guiding out stasis and unblocking Qi. If caused by Qi and Blood deficiency with stagnant Ying Qi and reversed Wei Qi (Defensive Qi), symptoms include flat, diffuse swellings and a faint, minute, and forceless pulse; treatment should primarily involve major tonification of Qi and Blood, using formulas such as Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction) or Bazhen Tang (Eight Treasure Decoction) to solidify the root and support Stomach Qi. When Qi is strong and Blood is harmonious, the toxin resolves spontaneously. If toxic diseases are treated with cooling and detoxifying herbs, it will inevitably lead to failure to suppurate or ulcerate, resulting in a disastrous complication.
