In women, heel sores and swelling may result from damp-heat pouring downward in the Liver channel, or from emotional frustration injuring the Liver and Spleen. The manifestations include swelling and pain in the hypochondrium and lower abdomen, or swelling and pain of the Yumen (female external genitalia), aversion to cold with high fever, difficult and scanty urination, acute abdominal pain, or a sensation of fullness in the lower abdomen radiating upward to the hypochondrium; this indicates stagnation of damp-heat in the Liver channel, treated with Longdan Xiegan Tang (Gentian Liver-Draining Decoction). If only the Yumen is swollen, it indicates Liver fire with blood deficiency, treated with Jia Wei Xiaoyao San (Modified Free and Easy Wanderer Powder). Using indiscriminately blood-dispersing and detoxifying agents would be an error.
Swelling and pain in the heels and toes, along with heat in the soles of the feet, are caused by improper care during pregnancy, childbirth, or menstruation, leading to deficiency of the three Yin channels of the foot and resulting in deficient heat. If there is swelling, pain, or suppuration, Liuwei Wan (Six-Ingredient Rehmannia Pill) is used as the principal formula, assisted by Bazhen Tang (Eight-Treasure Decoction). If there is weak stomach qi with poor appetite, Liujunzi Tang (Six-Gentlemen Decoction) is added. If there are alternating chills and fever with internal heat, Xiaoyao San (Free and Easy Wanderer Powder) is added. If afternoon tidal fever worsens with cloudiness of the head and eyes, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) is added. All conditions involving fever, afternoon tidal fever, internal heat, spontaneous sweating, or night sweats belong to false heat due to yin deficiency. Thus, Zhu Danxi stated that "fire rising from the nine springs" refers to extreme yin deficiency. The heel is the origin of the Du vessel (Governor Vessel), the pathway of the Kidney channel, and the foundation bearing all bones. If one does not seek the root cause and indiscriminately uses cold and bitter drugs, many will suffer premature death. Men who overindulge in alcohol and sexual activity often suffer from this condition.
When women feel as if their ten toes are being scalded by hot oil, it is due to deficiency of Ying (Nutrient) and Wei (Defensive) qi, with damp-toxic qi stagnating in the meridians. If attacking upward to the Heart, it causes heart pain; if attacking downward to the feet, it causes foot pain, with toes burning like fire. This resembles Jiaoqi (Beriberi/foot disease), which the Classic describes as Re Jue (Heat Jue).
Sores on the female vulva may result from improper regulation after childbirth, injuring the Spleen and Stomach, or from worry, thought, depression, and anger, injuring the Liver and Spleen, leading to damp-heat pouring downward. The exterior belongs to the three Yang channels of the foot and is easy to treat; the interior belongs to the three Yin channels of the foot and is difficult to cure. If initially red, swollen, and painful, it is due to invasion by damp-heat toxin, treated with Renshen Baidu San (Ginseng Detoxification Powder). If diffusely swollen and painful, or neither swollen nor painful, it is due to Spleen deficiency with damp-heat pouring downward, treated with Buzhong Yiqi Tang or Bazhen Tang, adding Bibi (Dioscorea Hypoglauca Rhizome) and Jinyinhua (Lonicerae Japonicae Flos). If pus and fluid drip continuously with fatigue, poor appetite, internal heat, and dry mouth, it is due to Spleen qi deficiency, treated with Buzhong Tang (Tonify Middle Decoction) adding Fuling (Poria) and Jiushao (Wine-processed Paeoniae Radix Alba). If there is afternoon fever with fatigue, it is due to blood deficiency, adding Chuanxiong (Ligusticum Chuanxiong Rhizome) and Shudi (Rehmanniae Radix Preparata) to the previous formula, or using Liuwei Wan. If limbs feel cold with reduced desire for food, it is due to Spleen and Kidney deficiency-cold, treated with Shiquan Dabu Tang (Complete Great Tonifying Decoction) or Bawei Wan (Eight-Ingredient Pill). Red color indicates heat-toxin and is easy to treat; dark color indicates deficiency-cold and is difficult to treat.
