The Lung and Large Intestine are exteriorly-interiorly related, and the anus is the gate of the Large Intestine. When Lung Qi is full, it is warm; warmth implies that internal vital energy is abundant and stored. When Lung Qi is deficient, it is cold; coldness implies that internal vital energy is discouraged and unable to retain, thus causing the rectum to prolapse. This condition often results from persistent dysentery with tenesmus (urgent need to defecate with a feeling of incomplete evacuation), straining efforts opening the anus, and exposure to external wind pathogens. It may also result from acute sudden diarrhea caused by latent summerheat, where the rectum cannot be controlled due to profuse watery stools. Alternatively, it may stem from congenital weakness, susceptibility to cold, and straining during crying or shouting, leading to deficiency-collapse of the Large Intestine. Generally, diarrhea and dysentery arise from wind, summerheat, dampness, and heat damaging the Spleen. Spleen deficiency leads to weakened Lung Qi and subsequently a deficient Large Intestine. Since Earth generates Metal (Spleen is Earth, Lung is Metal), maternal deficiency fails to nourish the child organ; thus, even slight exposure to wind-cold can cause rectal prolapse due to deficiency. Treatment should focus on tonifying the Spleen and warming the Stomach, allowing Lung Metal to benefit from its mother and ascend. Subsequently, administer intestinal-astringent formulas. Externally, use fumigation and powder application methods. If the prolapsed rectum has been out for a long time and becomes hard, first soften it with warm medicinal decoctions, then gradually reduce it. If the prolapsed tissue itches, it is mostly due to parasites generated by damp-heat in the Large Intestine eroding the anus. Ulcers on the upper lip indicate parasites eroding the viscera; ulcers on the lower lip indicate parasites eroding the anus. In chronic cases, gum roots lose color, the tongue becomes entirely white, limbs feel fatigued, hemoptysis resembles millet grains, and the patient experiences mental restlessness and vexation, constituting a critical condition. Initial treatment involves administering Huachuan Wan (Parasite-Dissolving Pill) externally using raw Mugwort, fumigating and washing with decoction of Melia Toosendan Root. Cases where parasites erode through the anus internally are incurable.
The anus serves as the envoy of the Large Intestine. Whether the Large Intestine is affected by heat or cold, both can lead to rectal prolapse. The Large Intestine is the official responsible for conduction, while the Kidney is the official responsible for strength and vigor. Excessive indulgence in alcohol and sexual activity leads to Kidney deficiency, which drains maternal qi (referring to the depletion of foundational essence affecting the whole system). Consequently, the Lung becomes deficient, and the Qi of the Large Intestine lacks mastery, resulting in prolapse. Children whose blood and qi are not yet robust, and the elderly whose blood and qi have declined, are both susceptible to this condition. Furthermore, according to Danxi's discussion, rectal prolapse is indeed common due to Qi deficiency and Blood deficiency, but there are also cases caused by Qi heat and Blood heat. These must be carefully examined via pulse diagnosis. For Qi deficiency, employ Qi-tonifying methods such as Ginseng, Atractylodes Macrocephala Rhizome, Licorice Root, combined with prepared Cimicifuga Rhizome for lifting. For Blood deficiency, use Siwu Tang (Four Substance Decoction). For Blood heat, use Liangxue Siwu Tang (Cooling Blood Four Substance Decoction) with fried Phellodendron Bark. For Qi heat, use Scutellaria Baicalensis Root and Cimicifuga Rhizome, all suitable for lifting therapies.
Anal itching and pain are mostly caused by parasites generated from damp-heat, potentially developing into hemorrhoids. One may mix Realgar with Mugwort and burn them to smoke-fumigate the area. Alternatively, steam one hu (a large ancient unit of volume) of Peach Leaves until extremely hot, place them in a small-mouthed bottle, and sit over the bottle to fumigate, which can cure the condition immediately.
