消阴翳 制阳光 热之不热,是无火也;益火之源,以消阴翳。寒之不寒,是无水也;
Malaria (Nüe) as a disease belongs to the Shaoyang (Lesser Yang) channel. The Shaoyang channel is located at the half-exterior and half-interior level, where pathogenic factors reside at this boundary. When pathogens enter and contend with Yin, chills occur; when they emerge and contend with Yang, fever occurs. Contention causes the disease to manifest, cessation of contention causes the disease to stop, but after stopping, the pathogenic factors still occupy the Shaoyang channel.
Chills and fever alternate like a turning back and forth; the chills and fever must arrive according to a specific periodicity.
If it occurs once daily, it does no great harm; shallow pathogens result in daily attacks, while deep pathogens result in bi-daily attacks.
If it occurs every three days, the condition becomes rampant; malaria occurring every three days is named "San-Yin Malaria" by contemporary physicians, lingering and difficult to cure.
The method of treatment centers on Xiao Chai Hu Tang (Minor Bupleurum Decoction). At the onset, folk custom forbids the use of Renshen (Ginseng Root); following custom, it is omitted, and Qingpi (Green Citrus Peel) is added, one qian.
If heat predominates, add cooling agents: To Xiao Chai Hu Tang, add Zhi Mu (Anemarrhena Rhizome), Hua Fen (Trichosanthes Root), Shi Gao (Gypsum Fibrosum), Huang Lian (Coptis Rhizome), etc., selected as appropriate.
If cold predominates, add Gui Zhi (Cinnamon Twig) and Gan Jiang (Dried Ginger): Add Gan Jiang and Gui Zhi; in severe cases, add Fu Zi (Aconite Lateral Root) and Rou Gui (Cinnamon Bark).
If pathogenic factors are exuberant, remove Renshen: For those with body heat, use Xiao Chai Hu Tang removing Renshen and adding one qian of Gui Zhi. After ingestion, consume hot porridge and cover warmly to induce slight sweating.
Chang Shan (Dichroa Root) enters [the formula], strength doubles: Add two or three qian of Chang Shan to Xiao Chai Hu Tang. Folk saying states that Chang Shan should not be used to intercept the pathogen before it is purified. This is unknown; Chang Shan is not an agent to intercept pathogens, but one to drive pathogens out. Zhang Zhongjing used its sprout, named Shu Qi (Dichroae Caulis).
For extreme deficiency, Du Shen Tang (Single Ginseng Decoction) is suitable: For deficient persons with chronic malaria unhealed, decoct one liang of Renshen and five qian of Sheng Jiang (Fresh Ginger) in water; taking it at Wu Geng (3-5 AM) yields excellent results. For the poor, Bai Zhu (Atractylodis Macrocephalae Rhizoma) one liang substitutes for Renshen; for those with more heat, Dang Gui (Angelica Sinensis Root) substitutes.
Single chills without heat constitute Pin Nüe (Yin Malaria), Li Zhong Tang (Regulate the Middle Decoction) supports it: Single chills without heat are named Pin Nüe; it is suitable to treat with Fu Zi Li Zhong Tang (Aconite Regulate the Middle Decoction) plus Chai Hu (Bupleuri Radix).
Single heat constitutes Dan Nüe (Sunny Malaria), Bai Hu Tang (White Tiger Decoction) details it: Single heat without chills is named Dan Nüe; or heat preceding chills is named Re Nüe (Heat Malaria); both are suitable for treatment with Bai Hu Tang plus Gui Zhi. Contemporary physicians treat it with Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) plus Chai Hu and Shao Yao (Paeoniae Radix Alba).
Methods beyond methods, distinguish the subtle: All above are established methods of previous physicians. There are methods beyond methods; one must not fail to distinguish and treat them.
Eliminate Yin shadows, control Yang sunlight: If warming does not produce warmth, it is Firelessness; benefit the source of Fire to eliminate Yin shadows. If cooling does not produce coolness, it is Waterlessness; strengthen the master of Water to control Yang sunlight.
Ta Pu's (Grand Steward's) annotations, do not forget: Wang Taipu's annotations on eliminating Yin and controlling Yang are immortal theories of eternity. Zhao Yangkui followed them, using Ba Wei Wan (Eight-Ingredient Pill) to benefit the source of Fire, and Liu Wei Wan (Six-Ingredient Pill) to strengthen the master of Water; chronic malaria often achieves success with these methods.
