Mania refers to a condition characterized by mental confusion and lack of normal stability. It manifests as shouting, running wildly, and being difficult for others to subdue. In severe cases, patients may climb high places to sing, strip naked and run, jump over walls or climb onto roofs, and curse people without distinguishing between relatives and strangers. Although this syndrome is attributed to phlegm, it is mostly due to excessive phlegm and fire. Treatment should prioritize clearing heat (Qing Re) as the principal method, with resolving phlegm (Hua Tan) as an auxiliary method. The Qinghuo Huatan Tang (Clear Fire Resolve Phlegm Decoction) is suitable to greatly resolve heat in the Heart and Stomach. If the stool is dry and constipated, the Tansu Wan (Phlegm-Draining Pill) can be used to purge downwards. After clearing heat, if pathogenic heat remains uncleaned, Chaihu Shaoyao Tang (Bupleurum and Peony Decoction) is suitable. If the pulse is deep and fine, Liujun Jianpi Tang (Six-Gentleman Spleen-Strengthening Decoction) is suitable. Mania originates from excess-heat syndromes; the pulse should be surging, large, and forceful. A deep and fine pulse indicates a critical condition. Dietary restrictions are required in treatment, and it must not be treated identically to dementia (Dian Syndrome).
Dementia (Dian Bing) manifests as delirious speech and incessant laughter. This arises from emotional depression and frustration. The pulse should be deep, small, and weak, and should not be surging and large. Treatment uses Liujun Jianpi Tang (Six-Gentleman Spleen-Strengthening Decoction). This disease is often caused by food stagnation generating phlegm; taking herbs such as Tianma (Gastrodia Rhizome) and Danshenxing (Arisaema cum Bile) is ineffective. Qi flow must be smoothed and phlegm eliminated. Additionally, Bawei Dihuang Wan (Eight-Ingredient Rehmannia Pill) is suitable to greatly tonify primordial Qi. This is an unchangeable therapeutic principle. ("The Classic" states: Heavy Yin leads to Dian, Heavy Yang leads to Kuang; this is the infallible method for differentiating symptoms.)
Qinghuo Huatan Tang (Clear Fire Resolve Phlegm Decoction): Huangqin (Scutellariae Radix), Huanglian (Coptidis Rhizoma), Shanzhi (Gardeniae Fructus), Beimu (Fritillariae Bulbus), Gualou Shuang (Pepo Fructus Pulvis), Zhishi (Aurantii Fructus Immaturus), Suzi (Perillae Fructus), Jiegeng (Platycodonis Radix), Chishao (Paeoniae Radix Rubra), Maidong (Ophiopogonis Radix).
Tansu Wan (Phlegm-Draining Pill): Dahuang (Rhei Radix et Rhizoma) (8 liang), Huangqin (Scutellariae Radix) (8 liang), Chenxiang (Aquilariae Lignum Resinatum) (5 qian), Mengshi (Calamina) (1 liang).
This formula is usable only for robust individuals; mistaken use in deficient persons will cause immediate death.
Liujun Jianpi Tang (Six-Gentleman Spleen-Strengthening Decoction): Renshen (Ginseng Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Baifu (Poria), Gancao (Glycyrrhizae Radix et Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), Zhike (Aurantii Fructus Immaturus), Houpo (Magnoliae Officinalis Cortex), Xingren (Armeniacae Semen Amarum), Zexie (Alismatis Rhizoma), Paojiang (Zingiberis Rhizoma Preparatum).
Epilepsy (Xian Bing) begins with sudden collapse to the ground, chest oppression, confusion, loss of consciousness, biting of the tongue, foaming at the mouth, opisthotonos, and convulsions of the hands and feet. Sometimes sounds resembling those of six domestic animals are produced. Ancient texts distinguished pig, sheep, cow, horse, and chicken epilepsy to correspond to the Five Viscera, but this distinction is unnecessary. Wind-phlegm agitates the orifices, causing natural changes in Qi, similar to a flute player where different blockages of the six holes produce distinct musical notes. If the pulse is surging and rapid, the syndrome belongs to Yang. Shuzhong Erchen Tang (Soothe Middle Two-Chen Decoction) is suitable, followed by adjustment using Qingxian Erchen Tang (Clear Epilepsy Two-Chen Decoction) with modifications. If the pulse is fine and weak, the syndrome belongs to Yin; treatment is difficult to cure. Liujun Jianpi Tang (Six-Gentleman Spleen-Strengthening Decoction) and Bawei Dihuang Wan (Eight-Ingredient Rehmannia Pill) are both essential. This disease involves phlegm latent in the Pericardium; it entirely depends on the clear and empty state of Stomach Qi to function normally. Daily diet should be taken in small amounts; rich, sweet, and heavy flavors should not be tasted frequently.
(Note: Rich, sweet, and meaty foods contain toxic substances. Even healthy individuals who consume them often develop chronic Spleen and Stomach diseases, let alone epilepsy patients. It is crucial that Stomach Qi remains clear and empty to avoid the risk of assisting phlegm generation. Physicians must warn patients of this in advance.)
Shuzhong Erchen Tang (Soothe Middle Two-Chen Decoction): Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), Baifu (Poria), Gancao (Glycyrrhizae Radix et Rhizoma), Xingren (Armeniacae Semen Amarum), Zhike (Aurantii Fructus Immaturus), Houpo (Magnoliae Officinalis Cortex), Shanzhi (Gardeniae Fructus), Huangqin (Scutellariae Radix), Xuanmingfen (Mirabilite).
Regarding epilepsy, people generally blame Liver Wind and frequently use herbs like Tianma (Gastrodiae Rhizoma) and Danshenxing (Arisaema cum Bile). They fail to realize that food stagnation generates phlegm, which suppresses Shaoyang Qi, leading to limb spasms and convulsions, and clouding the Spirit. Treating the food stagnation cures the disease. This is a method I have tested repeatedly with consistent success. Qingxian Erchen Tang (Clear Epilepsy Two-Chen Decoction) is recorded for reference (This formula has strong phlegm-resolving effects and is particularly effective for severe phlegm; however, for chronic illness with deficiency of Zheng Qi, dosage should be adjusted cautiously).
Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), Baifu (Poria), Gancao (Glycyrrhizae Radix et Rhizoma), Tianma (Gastrodiae Rhizoma), Danshenxing (Arisaema cum Bile), Gualou Shuang (Pepo Fructus Pulvis), Zhishi (Aurantii Fructus Immaturus), Shichangpu (Acori Tatarinowii Rhizoma), Jiegeng (Platycodonis Radix), Maidong (Ophiopogonis Radix), Huanglian (Coptidis Rhizoma), Shanzhi (Gardeniae Fructus).
