阳明之本宜解肌,审葛根白虎于寒热;
凡言宜汗,即前正方中,或发表,或解肌之剂选用;凡言宜吐,即前正方中,或探吐,或涌宣之剂选用;
无汗服桂枝而呕吐,失血下利者变用;
伤寒轻者,麻黄杏仁饮;

合病呕而不利,葛根加以半夏;

Taiyang disease with no sweating indicates cold pathogen damaging the Ying (Nutrient) level; in the deep winter, Mahuang Tang (Ephedra Decoction) is most appropriate. Taiyang disease with sweating indicates wind pathogen damaging the Wei (Defensive) level; in the deep winter, Guizhi Tang (Cinnamon Twig Decoction) can be used first. Li Dongyuan’s Chonghe Tang (Harmonizing Decoction, i.e., Jiwei Qianghuo Tang / Nine-Ingredient Notopterygia Decoction) treats wind-cold occurring in spring, summer, and autumn; Tao’s Chonghe Tang distinguishes Yin and Yang to rescue dual-transmission syndromes. This formula addresses two-channel cold damage (Lianggan), which is typically fatal; however, if the Yang channel is affected first and predominantly, this decoction can be used for exploration. If the medicine hits the target, recovery ensues. If not, medication should be adjusted based on the degree of excess or deficiency in the exterior, interior, Yin, and Yang. Therefore, when using Jiwei Qianghuo Tang, remove Cangzhu (Atractylodis Rhizoma), add Chaihu (Bupleuri Radix), Gan Ge (Puerariae Radix), Shigao (Gypsum Fibrosum), and Heidou (Sojae Semen Nigrum); these are all herbs for the three Yang channels.
The root of Yangming disease requires muscle-releasing methods; carefully distinguish between Gegen Tang (Kudzu Decoction) and Baihu Tang (White Tiger Decoction) based on cold and heat symptoms. For wind-strike with sweating and exterior deficiency, use Guizhi Tang. For cold damage with no sweating and exterior excess, use Gegen Tang. If there is aversion to cold replaced by aversion to heat, and bowel/bladder functions are unobstructed, use Baihu Tang.
The branch of Yangming disease requires Chengqi (Ordering the Qi) formulas; carefully assess Pi (stuffiness), Man (fullness), Zao (dryness), and Shi (excess) in relation to hard stools. When Pi, Man, Zao, and Shi are all present, use Dachengqi Tang (Major Order the Qi Decoction). When only Pi and Man are present without Zao and Shi, use Xiaochengqi Tang (Minor Order the Qi Decoction). When only Zao and Shi are present without Pi and Man, use Tiaoweichengqi Tang (Stomach-Harmonizing Order the Qi Decoction). Daxiehu Tang (Major Bupleurum Decoction) resolves constipation in Yangming involving the exterior; Xiaoxiehu Tang (Minor Bupleurum Decoction) excels in harmonizing Shaoyang disorders. If phlegm obstruction causes chest fullness, slowly drink Gouti San (Melon Seed Powder) or Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction). If there is no headache or fever, decoct Fuzi (Aconiti Lateralis Radix Preparata) and Ganjiang (Zingiberis Rhizoma) thoroughly. Ancient Jiangfu Tang: Ganjiang (1 liang), Shengfuzi (1 piece), water 3 cups, decoct to 1 cup, remove dregs and take in one dose. Treats true cold syndrome directly invading the Yin channels, such as cold limbs and absent pulse; double the dosage of Gancao (Licorice Root). Lizhong Tang (Regulate the Middle Decoction) and Xiao Jianzhong Tang (Minor Construct the Middle Decoction) treat Taoyin direct invasion of organ cold deficiency; Guizhi Jia Huangqin Tang (Cinnamon Twig plus Scutellaria Decoction) and Guiling Tang (Cinnamon and Poria Decoction) dispel heat from Taoyin transmission to the fu organs.
Guizhi Jia Dahuang Tang: Guizhi (3 qian), Shaoyao (4 qian), Gancao (2.5 qian), Dahuang (2 qian), Shengjiang (5 slices), Dazao (2 pieces), decoct in water and take warm. Treats Taoyin transmission heat syndrome with abdominal fullness and pain, dry throat with thirst, warm hands and feet, and deep forceful pulse. Tao’s modification adds Zhike (Aurantii Fructus) and Chaihu, adding Binglang (Arecae Semen) water before administration. If abdominal fullness occurs without aversion to cold but with wheezing, remove Gancao and add Dafupi (Tsaoko Pericarpium). Guizhi Jia Shaoyao Tang: Guizhi (3 qian), Shaoyao (4 qian), Gancao (1 qian), Shengjiang (4 slices), Dazao (1 piece), decoct in water and take warm. Treats severe Taoyin abdominal fullness and pain, or Taiyang disease that has shifted to Taoyin self-diarrhea after purgation. Tao’s Guiling Yin and Wuling San (Five-Poria Powder) are also suitable.
Shaoyin heat syndrome uses Xiaochengqi Tang; conversely, for cold syndrome, use Sini Tang (Recover the Prostrate Decoction) with doubled Gancao to cure Rejection of Yang syndrome. Jueyin heat syndrome uses Dachengqi Tang; conversely, for cold syndrome, use Danggui Sini Tang (Angelica Decoction for Cold Extremities) to resolve malaria-like symptoms. Although these are standard treatments for the Six Channels, they embody the true essence of treating hundred diseases.
General principles for Exterior, Interior, Yin, and Yang: For any mention of Exterior syndrome, distinguish sweating/no sweating and select from previous formulas according to the channel; for Interior syndrome, distinguish transmission vs. direct invasion and select from previous formulas; for Yin syndrome, select from previous warming/internal heat formulas; for Yang syndrome, select from previous interior-cooling formulas. These few formulas can replace hundreds; although attributed to Zhang Zhongjing, there remains regret over omissions. In my humble opinion, extending these categories to miscellaneous diseases makes the methods unsurpassable.
Sweating, Vomiting, Purging, and Warming methods are all included. For indications of sweating, select releasing-exterior or muscle-releasing agents from previous formulas; for vomiting, select exploratory-vomiting or涌宣 (surge-and-disperse) agents; for purging, select moist-purgative or surging-purgative agents; for warming, select internal-cold/yang-loss agents. These methods encompass hundreds, relying on appropriate application. Misuse of Mahuang leads to Yang loss and incessant sweating; soak patient's hair in water and dust with Nuomicu (Glutinous Rice Powder), Longgu (Fossilized Bones), and Muli (Oyster Shell). Misuse of Xiaoshi (Mirabilite) and Dahuang leads to unbearable diarrhea; stop it with Lizhong Tang added with fried glutinous rice, Wu Mei (Mume Fruit), and Zaoxin Tu (Kitchen Hearth Soil). Misuse of Ganjiang and Fuzi leads to bleeding and macules;解毒 (detoxify) with Radish juice, or mud juice, plus Huangqin (Scutellariae Radix), Gancao, and Xijiao (Rhinocerotis Cornu substitute). Otherwise, red spots indicate blood attempting to exit through orifices.
Furthermore: Wind-cold is generally called "sweat disease"; Mahuang and Guizhi must not be used indiscriminately. Whether wind-strike or cold-damage, resolution comes with sweat, but distinguish between profuse sweating and muscle-releasing.
No-sweat patients taking Guizhi Tang may vomit; those with blood loss or diarrhea require modified usage. Cold-damage with no sweat, or wine-drinkers mistakenly taking Guizhi Tang leading to vomiting, will likely vomit pus and blood, indicating Feiwei (Lung Atrophy). If sweating is indicated but coincides with blood loss or diarrhea, do not give Mahuang Tang; instead, give small doses of Guizhi Tang frequently until skin is moist for natural harmony.
Liver-fire patients taking Mahuang may become irritable with malaria-like chills and fever. Wind-strike not resolved by Guizhi Tang, with malaria-like symptoms occurring twice daily, indicates pathogen in Ying/Wei; use Guizhi Er Mahuang Yi Tang (Guizhi Twice, Mahuang Once Decoction). If not this syndrome, misuse of Mahuang causes irritability and epistaxis. Treat epistaxis and lung atrophy per miscellaneous diseases using blood-cooling agents like Xijiao Dihuang Tang (Rhino-Corn and Rehmannia Decoction) or Huangqin Tang plus Chaihu.
Mild cold-damage uses Mahuang Xingren Yin (Ephedra and Apricot Kernel Infusion): Mahuang, Jiegeng, Qianhu, Huangqin, Chenpi, Banxia (1 qian each), Xingren, Xixin (0.8 qian each), Fangfeng (0.7 qian), Gancao (0.4 qian), Shengjiang (3 slices), decoct and take warm. Treats Taiyang fever/aversion-to-cold, headache, no sweat, floating-tight pulse, cough. In summer, remove Mahuang, add Ziye (Perilla Leaf); for spontaneous sweat, remove Mahuang, add Guizhi, Shaoyao; for exterior heat, swap for Chaihu; for thirst, add Tianhuafen (Trichosanthis Radix); for chest fullness, add Zhike; for wheezing, add Gualou Ren (Trichosanthis Seed).
Mild wind-strike uses Chaihu Banxia Tang (Bupleurum and Pinellia Decoction): Chaihu, Banxia (1.5 qian each), Huangqin, Baizhu, Chenpi, Maidong (1 qian each), Gancao (0.5 qian), Shengjiang (3 slices), Dazao (2 pieces), decoct and take warm. Treats wind-strike fever/aversion-to-cold, headache, no sweat, cough, or heat-collateral diarrhea. Also treats phlegm syndromes resembling cold-damage: add Fuling (Poria) for difficult urination; winter/no sweat add Mahuang; spring/summer/autumn/no sweat add Ziye; winter/sweat add Guizhi; spring/summer/autumn/sweat add Fangfeng; sore throat add Jiegeng; wheezing/cough remove Baizhu, add Xingren, Sangbaipi (Morus Bark); alcohol-heat add Huanglian (Coptidis Rhizoma); food stagnation add Shanzha (Crataegi Fructus), Shenqu (Medicated Leaven); phlegm in hypochondrium pain add Baijiezi (Sinapis Semen); phlegm rattling in throat add Zhuli (Bamboo Sap), Jiangzhi (Ginger Juice); thick phlegm add Jinfeicao (Boehmeriae Herba), Qianhu (Peucedani Radix); chest/stomach stuffiness add Zhike.
Da Qianghuo Tang (Major Notopterygia Decoction) treats Yin-Yang dual infection: Fangfeng, Qianghuo, Duhuo, Fangji, Huangqin, Huanglian, Cangzhu, Baizhu, Gancao, Xixin (0.3 qian each), Zhimu, Chuanxiong, Dihuang (1 qian each), decoct hot and take. Repeat 3-4 doses if unresolved. Treats fever/aversion-to-cold/no sweat, or spontaneous sweat/headache/stiff neck, or cold-damage with wind-pulse/wind-strike with cold-pulse. Resolves dual-infection. Suitable for separated Yin/Yang with predominant Yang syndrome. Tao’s Chonghe Tang is for undifferentiated Yin/Yang.
Da Qinglong Tang (Major Green Dragon Decoction) excellently releases wind-cold dual injury: Mahuang (3 qian), Guizhi (2 qian), Xingren (1.5 qian), Shigao (4 qian), Gancao (1 qian), Shengjiang (3 slices), Dazao (2 pieces), decoct and take warm. The logic: Cold requires sweet release; sweetness of Mahuang, Xingren, Shigao, Dazao disperses Ying-cold. Wind requires pungent dispersion; pungency of Guizhi, Shengjiang disperses Wei-wind. Pungent-sweet combination disperses Ying-Wei wind-cold. Wind damages causing Wei-excess, Cold damages causing Ying-excess; both Ying/Wei excess with Taiyang signs, no sweat, irritability: suitable. If pulse is faint with sweat (Ying/Wei deficiency), misuse causes cold extremities, muscle twitching, and death.
Combined illness with vomiting but no diarrhea: Gegen Jia Banxia Tang (Kudzu plus Pinellia Decoction): Gegen (3 qian), Mahuang (2 qian), Guizhi (1 qian), Banxia, Shaoyao (1 qian each), Gancao (0.8 qian), Shengjiang (3 slices), Dazao (2 pieces), decoct and take. Treats Taiyang-Yangming combined illness with rebellious Qi, vomiting without diarrhea. Use Gegen Tang to disperse pathogen, add Banxia to descend rebellious Qi.
Combined illness with diarrhea but no vomiting: Huangqin Banxia Shengjiang Tang (Scutellaria, Pinellia, Ginger Decoction): Huangqin (3 qian), Shaoyao, Shengjiang (2 qian each), Banxia (1 qian), Gancao (0.5 qian), Shengjiang (5 slices), Dazao (2 pieces), decoct and take warm. Treats Taiyang-Shaoyang combined illness with self-diarrhea/vomiting, or intestinal mucus with heat-collateral diarrhea. Not suitable for sweating/purging; use this to harmonize and descend stomach Qi.
Taiyang combining with Yangming: Check if Mahuang, Gegen, Xiaoshi, Dahuang have been exhausted. If Taiyang signs persist, no sweat: Gegen Tang; with sweat: remove Mahuang. If Taiyang signs resolved, Yangming signs appear: Tiaoweichengqi Tang.
Taiyang combining with Shaoyang: Weigh Mahuang, Guizhi, Chaihu. No sweat: Xiaoxiehu Tang + Mahuang Tang; with sweat: Chaihu Guizhi Tang. Spring/Summer/Autumn/mild cases: Xiaoxiehu Tang main, replacing Mahuang/Guizhi with Qianghuo/Fangfeng is acceptable.
Spring/Summer/Autumn/Winter epidemics: Shengma Renhuang Ruyi (Cinnabar and Human-Gold Ruyi). Shengma Gegen Tang: Logic is four-qi disharmony,郁毒 (stagnant toxin) becomes epidemic. Shengma, Gegen, Gancao resolve hundred toxins; Shaoyao harmonizes middle. Better than Baidu San for epidemic toxin. Renzhonghuang Wan: Dahuang, Huanglian, Huangqin, Renshen, Jiegeng, Cangzhu, Fangfeng, Huashi, Xiangfu, Renzhonghuang equal parts, powder, Shenqu paste pills, size of wu zi (loquat seed), 70 pills/dose. Qi-deficiency: Si Junzi Tang; Blood-deficiency: Si Wu Tang; Phlegm: Er Chen Tang; Heat: add Tongbian (Child's Urine). If no Renzhonghuang, use dung pit wall ash, or coat with Zhusha (Cinnabar)/Xionghuang (Realgar). Ruyi Dan: Chuanwu (8 qian), Binglang, Renshen, Chaihu, Wuzhuyu, Chuanjiao, Baijiang, Baifuling, Huanglian, Ziwan, Houpo, Rougui, Danggui, Jiegeng, Zaojiao, Shichangpu (5 qian each), Badou (2.5 qian). Select auspicious day, silent room, pure ingredients, honey pills, cinnabar coat, 3/5/7 pills. Treats warm-epidemic, ghosts, latent corpses, TB, mania, mountain miasma. Date soup or plain water. Wind-epidemic/big wind, numbness, no tears, restlessness, face crawling, beard/hair loss, lip/tooth rot, hemi-paralysis, knee swelling: Jingjie decoction. Cold-epidemic/small intestine qi pain: Xiaohui or Wuzhuyu decoction. Summer-epidemic/five stranguries: Dengxin (Juncus) decoction. Severe heat: Dahuang decoction. Dry-epidemic: Shengdi or Humaoren decoction or cold water. Damp-epidema/edema: Cheqianzi or Mutong decoction. Ten types of water qi: Gansui, Daji decoction. Goiter/gu poison: Gansui decoction. Bladder hernia/swelling: Radish decoction. Five hemorrhoids: Baifan decoction. Five epilepsy types: Rushiang decoction. Kidney accumulation/grinding teeth/saliva/lower back pain: Salt water. Five malaria types: Taozhi (Peach twig) decoction. Heart lost evil: Liuzhi Taozhi decoction. Yin-Yang two poisons/wind-cold cough: Bohe decoction. Five malnutrition/eight dysenteries/intestinal wind/visceral toxin: Old rice decoction. Various coughs: Ginger decoction. Children twelve convulsions: Bohe decoction. Danliu, abscess, scrofula, sores, salivation/wheezing, diabetes, bowel blockage/diarrhea, alcohol toxicity/red urine, throat bi, double cheek, swallowed metal/stones/drugs, acclimatization issues: Warm water. Red dysentery: Huanglian decoction. Women cold uterus/blood-leukorrhea/infertility/blood qi issues: Aiye (Mugwort) decoction. This formula suits mountain people unused to medicine; success depends on skillful use.
Wind-Cold-Damp-Heat deviation: Huoxiang Zhengqi prevention. Buhuanjin Zhengqi San: Houpo, Chenpi, Huoxiang, Banxia, Cangzhu (1 qian each), Gancao (0.5 qian), Shengjiang (3 slices), Dazao (2 pieces), decoct and take warm. Treats four-season cold-damage, seasonal epidemic, mountain miasma, limb stiffness, abdominal fullness, indigestion, vomiting/diarrhea/aversion-to-cold without fever. Regulates Stomach Qi for prevention. Also treats cholera vomiting/diarrhea, bloody-white dysentery, acclimatization issues. If cold-damage adds headache/fever/chest fullness: Huoxiang Zhengqi San. If body pain adds: Renshen Baidou San, Jiwei Qianghuo Tang. Huoxiang Zhengqi San: Huoxiang, Zisu, Baizhi, Dafupi, Fuling (0.6 qian each), Houpo, Baizhu, Chenpi, Jiegeng, Banxia Qu (0.4 qian each), Gancao (0.2 qian), ginger/date decoction. Treats internal Spleen/Stomach injury, external cold, chills/fever/stiffness, headache/vomiting, chest fullness, food/cold/damp injury, summerheat, cholera, miasma, acclimatization, malaria-like chills/fever. Modify as needed. Not for true cold-damage. If Taiyang headache/fever/joint pain, this formula is irrelevant; misuse weakens righteous Qi and reverses meridians. Contraindicated in Qi-deficiency and Yin-clasping cold-damage.
Spring-Warmth: Tongsheng Shuangjie. Fangfeng Tongsheng San: Fangfeng, Chuanxiong, Danggui, Chishao, Dahuang, Mahuang, Bohe, Lianqiao, Mangxiao (0.25 qian each), Shigao, Huangqin, Jiegeng (0.5 qian each), Huashi (1.5 qian), Gancao (1 qian); Jingjie, Baizhu, Shanzhi (0.15 qian each), Shengjiang (3 slices), decoct and take warm. Treats spring/summer warmth resembling cold-damage, exterior-interior involvement. Also internal wind-cold/dry-heat, alcohol stroke, constipation/diarrhea, wind-heat blocking speech. ENT/Mouth/Nose/Throat wind-heat/phlegm. Surgical abscesses/sores/spots/trauma. Children convulsions/malnutrition/heat, wind spasms, slow eruption. Women diseases + Si Wu Tang. Treats all wind-heat. Diarrhea: remove Mangxiao/Dahuang; Sweat: remove Mahuang = Tongsheng San. Shuangjie San: Fangfeng Tongsheng San + Yiyuan San equal parts, ginger/scallion/fermented soybean decoction. Vomiting: induce; Purging: purge; Sweating: sweat. Hence "Double Release". Treats wind-cold-damp, hunger/fullness/labor, internal/external evils causing Qi/Blood stagnation, accumulated heat, sweat disease, alternating chills/fever, epilepsy/spasms/panic. Excellent for children rash. Diarrhea: remove Mangxiao/Dahuang; Sweat: remove Mahuang.
Summer-Heat: Yiyuan Single Formula. Yiyuan San, aka Liuyi San (Six-One Powder). Meaning: Heaven generates Water (1), Earth completes Six (6). Huashi (6 liang), Gancao (1 liang), powder, 3 qian with honey, boiling water mix. Heat: cold water mix. Pregnant/miscarry-prone: avoid. Cold-damage persistent heat: add Cangzhu powder (3 qian), Congchi Tang mix, several doses until sweat. Post-sweat/vomit/purge residual heat: excellent. Deficient irritability/insomnia: add Chen Sha (Cinnabar). Fever/cholera/vomiting/tetany: Dig pit, fill with fresh water, stir/settle = Dijiangshui (Earth Slurry Water). Well water bowl + oil spoon = floats. Sprinkle medicine on oil, sinks. Remove clear water, use earth slurry to mix = Youzhui San (Oil-Sinking Powder). Wind-heat blocking throat: add Qingdai (Indigo Naturalis), Bohe, honey pill suck. Cold diarrhea/vomiting: add Ganjiang (5 qian) = Wenliu Wan (Warm Six Pill). Severe cold: add Shihuang (Sulfur), ginger juice steamed bread pill. Liver evil suppression. Summer-thirst: new well water. Field-burn-thirst: muddy rice straw decoction. Zhizi Shengma Tang combines Autumn late-onset: Shengdi (3 qian, Sweet-Cold), Zhizi, Shengma, Chaihu (1 qian each, Bitter/Cold), Shigao (2 qian, Sweet-Cold), Shengjiang (3 slices), decoct warm. Treats late-onset cold-damage. Taiyang signs present, do not sweat heavily; use this to clear muscle/heat. Also warm-heat/deficient irritability. Sweat: add Guizhi; No sweat: add Ziye, Gan Ge; Deficient irritability: add Zhimu, Maidong; Thirst: add Tianhuafen; Cough: add Xingren. Moistens dryness, fits autumn late-onset. Classic: "Dryness invades interior, treat with bitter-warm, assist with sweet-pungent." Fits this formula. Zhang Hezi’s Liutong Shuangjie San (Cangzhu, Shigao, Huashi, Huangqin, Mahuang, Gancao, ginger/scallion) treats March late-onset, less suitable. Doubt: Winter cold damages, Spring warms; late-onset is delayed warmth. Summer heat damages, Autumn malaria; can Autumn disease be late-onset? Malaria once daily/heat > cold: shallow pathogen, summer heat. Every three days/cold > heat: deep pathogen, malaria mass, not just winter cold stagnation + summer heat reactivation. Summer pathogen cannot form masses. Thus Autumn diseases exceed other seasons; Dysentery/Jaundice similar.
Baishao Shengdi restrains Winter aberrant Yang: Jiajian Tiaozhong Tang: Baishao (1.5 qian), Fuling, Baizhu (0.8 qian each), Maidong (0.4 qian), Shengdi (0.5 qian), Chenpi (0.3 qian), Jiegeng, Wumei, Gancao (0.2 qian each), decoct warm. Treats Winter-Warmth/Spring sudden warmth, irritability/sleep/food disturbance, or wind-strike-like state. Full constitution: add Huangqin; Phlegm: add Beimu.

Deficient irritability: add Maidong. Treats movement-induced heat damaging Qi, fever,洪 (Surging) or Chi (Slow) pulse, irritability/thirst/dry mouth, post-urination chill/goosebumps, open mouth/black front teeth. Also treats macules, hence named Hua...
