草。〔向〕前搐者,升麻、白芷、防风、独活、甘草。两傍搐者,柴胡、防风、甘草;
Inability to move the hands and feet indicates *Zhongfu* (Stroke affecting the Fu organs), where the pathology is at the exterior level. Treatment should initially employ sweating methods. Use Qianghuo, Fangfeng, Shengma, Chaihu, and Gancao each 2 qian, prepared as a single dose to induce sweating. Thereafter, employ methods to unblock the meridians and nourish the blood, using Danggui, Qinjiao, Gancao, and Duhuo each 1 liang, adjusted according to the specific meridian involved.
Deafness, visual impairment, and deviation of the mouth and eye indicate that pathogenic factors have affected the Zang organs, where the pathology is at the interior level. Treatment should initially promote bowel movement, followed by methods to unblock the meridians. Use Baizhi, Chaihu, Fangfeng, and Duhuo each 1 liang, plus Chuanxiong 0.5 liang and Bohe 0.5 liang.
Grind the above ingredients into powder, mix with refined honey to form pills the size of a bullet. Take one pill per dose, chew thoroughly, and swallow with warm wine; clear tea decoction may also be used.
A floating pulse indicates an exterior condition, which requires sweating; a deep pulse indicates an interior condition, which requires purging. For spasms/twitching of the back, use Qianghuo, Fangfeng, Duhuo, and Gancao. For forward twitching, use Shengma, Baizhi, Fangfeng, Duhuo, and Gancao. For lateral twitching, use Chaihu, Fangfeng, and Gancao; for right-sided twitching, add Baizhi.
The Classics state: Whenever sores involve intense heat and stagnation, preventing the free circulation of Ying and Wei, white scales often appear. At this time, the sore orifice is closed, Qi cannot vent, and extreme heat generates wind. The "Treatment Methods" states: In *Poshang Zhongfeng* (Wind-stroke following trauma), if wind-heat dryness is severe, constrained at the exterior while the interior Qi remains relatively calm, symptoms include frequent stretching and yawning, tendon and vessel stiffness/spasm, occasional aversion to cold with twitching, and a floating, rapid, and wiry pulse. Treat with acrid-hot wind-dispelling herbs to open and flush out stagnation, restoring the free flow of Ying and Wei for recovery. When using acrid-hot herbs, it is particularly advisable to assist them with cold-cooling herbs to prevent mismatched treatment and exacerbation of wind. If *Poshang Zhongfeng* fails to resolve at the exterior and gradually penetrates inward, the disease severity increases. If the interior condition is not yet severe, but the pulse reflects pathology in the muscle layer, cool herbs that dispel wind-heat and resolve stagnation should be used for regulation. Alternatively, slightly adding acrid-hot wind-treating herbs may be considered, but dosage must be carefully judged according to the situation and never applied recklessly. *Zhibao Dan* (Precious Treasure Pill) is also a cooling formula. If interior heat is extremely severe, purge it using Dachengqi Tang (Major Order the Qi Decoction).
