Moxibustion for symptoms such as hemiplegia caused by wind striking the fu organs:
Baihui (GV 20) one point (located in the center of the nape, in the depression within the whorl of hair, large enough to hold a bean).
Quchi (LI 11) one point (located in the depression at the end of the transverse crease, between the lateral epicondyle of the humerus and the olecranon of the ulna when the elbow is flexed; take the point with arms crossed over the chest).
Jianyu (LI 15) two points (located in the depression between the acromion and the greater tubercle of the humerus at the shoulder tip; raise the arm to locate).
Fengshi (GB 31) two points (located in the depression midway between the two tendons on the lateral aspect of the knee; stand upright, relax the hand, and where the fingertip touches the thigh is the location).
Zusanli (ST 36) two points (located three cun below the kneecap, between the two tendons on the lateral side of the tibia).
Yanglingquan (GB 34), referred to here as Juegu (Ex-LE 5) two points (located in the arterial pulse three cun above the lateral malleolus).
For wind striking the zang organs, presenting with cold qi, tidal phlegm, aphasia, coma, and critical condition, moxibustion yields immediate effect:
Baihui (GV 20) one point.
Dazhui (GV 14) one point (also known as Bailao, located in the depression above the spinous process of the first thoracic vertebra at the back of the neck).
Fengchi (GB 20) two points (located in the depressions at the hairline behind the temporal region).
Quchi (LI 11) two points.
Jianshi (PC 5) two points (located three cun proximal to the wrist crease, in the depression between the two tendons).
Zusanli (ST 36) two points.
For wind striking the vessels, causing facial paralysis (deviation of mouth and eyes):
Tinghui (GB 2) two points (located in the depression posterior to the ear; open the mouth to locate, palpable arterial pulse).
Jiache (ST 6) two points (located eight fen below the earlobe).
Dicang (ST 4) two points (located four fen lateral to the corner of the mouth, near the lower lip, where a slight pulse can be felt).
If the mouth deviates to the right, it indicates wind-stroke of the left vessel with flaccidity; moxa fourteen burns (two sevens) on the depression of the left side. If the mouth deviates to the left, it indicates wind-stroke of the right vessel with flaccidity; moxa fourteen burns on the depression of the right side. Use cone-shaped moxa the size of a wheat grain, apply frequently until the wind pathogen is expelled and the mouth and eyes return to normal.
For sudden syncope or critical conditions due to stroke:
Shenque (CV 8) [navel]: Fill the navel with dry, clean salt, cover with a thick slice of ginger, and moxa 100 to 500 cones. More is better. Replace the ginger if it burns.
Moxa Dantian (CV 4, three cun below the umbilicus) and Qihai (CV 6, 1.5 cun below the umbilicus). These two points connect to Mingmen (GV 4), serving as the sea of vital energy and the root of the meridians; moxibustion here is highly effective.
General principles of moxibustion: The moxa cone should be the size of a cocklebur seed, must be compact, and the mugwort must generate strong heat. Upon contracting wind, follow this sequence; relief comes immediately upon ignition. *Qianjin Yifang* states: "The method of curing wind diseases through fire moxibustion is uniquely wonderful, surpassing acupuncture, stone therapy, and herbal decoctions."
On the head and face, moxa cones should be small, not large; on the hands and feet, they may be thicker. Moxibustion must proceed from top to bottom, never starting from the lower body and moving up.
Aphonia (loss of voice) with preserved speech ability but inability to produce sound indicates deficiency of visceral qi. Deficiency in the Spleen: use Zishou Jieyu Tang (Longevity Speech-Unblocking Decoction), as the Spleen meridian wraps around the throat and connects to the root of the tongue. Deficiency in the Heart: use Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction), as the Heart's divergent meridian connects to the root of the tongue. Liu Junzi Tang (Six-Gentlemen Decoction) doubled with Ophiopogonis Radix (Maidong), added with Bamboo Sap (Zhuli), and further supplemented with Salviae Miltiorrhizae Radix (Danshen), Polygalae Radix (Yuanzhi), and Acori Tatarinowii Rhizoma (Shichangpu) to tonify all viscera.
Stiff tongue with inability to speak, or slurred speech, indicates obstruction by wind-phlegm. Complete muteness may result from coma or kidney deficiency where qi fails to reach under the tongue. For deficiency-heat, use Jieming Dan (Life-Connecting Pill); for deficiency-cold, use Dihuang Yinzi (Rehmannia Decoction).
Facial deviation occurs because the Foot-Yangming meridian passes through Jiache, and the Hand-Taiyang meridian passes through the neck to the cheek; wind invading these two meridians causes traction and misalignment. Drink Du Huo, Zhuli (Bamboo Sap), and Dihuang Zhi (Rehmannia Juice) from *Waitai Mi Yao*.
Pian Feng (hemiplegia/one-sided paralysis): Harmonizing yin and yang, and dredging collaterals, is the way to treat internal injury. Using potent herbs to attack pathogens, acupuncture, and sweating via moxibustion/warming are ways to treat external contraction. For warming therapy (ironing), use Gastrodiae Rhizoma (Tianma), Pinelliae Rhizoma (Banxia), and Asari Radix et Rhizoma (Xixin), each 2 liang, placed in two silk bags, steamed hot, and applied to the affected area until sweat appears, indicating cure.
Lijie Tongfeng (Gout/Joint Pain): See specific section below. Use half sheng of stir-fried Black Bean (Heidou), 2 liang of Spirit Pea Vine Stem (Weilingxian), and 1 liang of Mulberry Root Bark (Sangbaipi), decocted in 1.5 sheng of fermented wine, reduce to 8 he, and take in one dose. Alternatively, take one handful of White-headed Grass (Baitou Cao), crushed, mixed with strong wine, taken in one dose.
Feng Huan (Wind Flaccidity/Paralysis): Recent classifications distinguishing left and right paralysis are incorrect. The Spleen governs muscles and limbs; the Stomach is the Sea of Water and Grain, distributing their qi to nourish the whole body. When wind-pathogen attacks the deficiency of the Spleen and Stomach, how can the limbs not become flaccid and useless? Furthermore, the Liver governs sinews, and the Kidney governs bones; damage to the Liver and Kidneys leads to weakness in the hands and feet.
Wind-induced pruritus: Exterior deficiency and weak Wei Qi allow wind to invade and transform into heat; heat causes itching, and scratching leads to sores. Use a wash formula: Half bowl of Purple-backed Floating Duckweed (Zibei Fuping), one handful of Ligusticum Wallichii Rhizome (Gaoben - inferred from context 'cao'), 5 qian of Cnidium Fruit (Shechuangzi), 5 qian of Saposhnikovia Root (Fangfeng), and 1 liang of Cocklebur Fruit (Cangerzi). Decoct and wash several times for cure.
Lijie Feng (Joint Wind/Painful Obstruction): Pain throughout all joints. For new onset, use Wuji San (Five Accumulations Powder) to disperse pathogens; for chronic pain, tonify Qi and Blood to overcome pathogens. Treatment is similar to that for wind striking the vessels.
This disease name does not appear in *Neijing*, but exists in Zhang Zhongjing's *Jingui Yaolue*. The decoction of Cinnamon Twig (Guizhi), Peony Root (Shaoyao), Anemarrhena Rhizome (Zhimu), and Ginger (Shengjiang) is particularly effective. Fujian physicians call it "Liaohuo Feng" (Burning Fire Wind), treating it with Rhinoceros Horn (Xijiao - now banned/replaced), Antelope Horn (Lingyangjiao), Angelica Pubescens Root (Du Huo), Burdock Fruit (Niubanggen), Scrophularia Root (Yuanshen), Gardenia Fruit (Zhizi), Rhubarb (Dahuang), and Cimicifuga Rhizome (Shengma).
