Use six taels of top-grade Ren Shen (Ginseng Radix et Rhizoma), add five bowls of water, and decoct until two bowls remain; then add the dregs to two bowls of water, decoct until one bowl remains, strain out the dregs, and combine the three bowls of Ginseng juice. Simmer slowly over a gentle fire, stirring frequently with chopsticks, until the liquid becomes thick and concentrated, forming an extract. This is used for saving critical cases of deficiency collapse where Qi is about to depart; it serves as an adjunct to Liu Wei Tang (Six-Ingredient Decoction) (see "Internal Apoplexy").
It treats dryness-heat caused by Blood deficiency, presenting symptoms resembling Bai Hu Tang Syndrome. If mistakenly treated with Bai Hu Tang, it is difficult to cure.
Huang Qi (Astragali Radix) (8 qian), Dang Gui (Angelicae Sinensis Radix) (2 qian), Da Zao (Jujubae Fructus) (5 pieces). Decoct in water and take orally.
It treats restlessness and insomnia following sweating or purging therapies.
Suan Zao Ren (Ziziphi Spinosae Semen) (stir-fried thoroughly, 1 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma), Zhi Mu (Anemarrhenae Rhizoma), Mai Dong (Ophiopogonis Radix), Fu Ling (Poria), each 6 fen; Gan Jiang (Zingiberis Rhizoma) 3 fen. Decoct in water and take orally.
It treats exterior syndrome unresolved with fluid retention, manifesting as dry retching, fever, and cough.
Ma Huang (Ephedrae Herba), Gui Zhi (Cinnamomi Ramulus), Shao Yao (Paeoniae Radix Alba), each 1 qian; Ban Xia (Pinelliae Rhizoma) (stir-fried with Ginger Juice, 1.5 qian); Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma), Gan Jiang (Zingiberis Rhizoma), Xi Xin (Asari Radix et Rhizoma), each 5 fen. Decoct in water and take orally.
Xiao Yao San (Free and Easy Wanderer Powder) (see "Internal Apoplexy").
Wu Wei Yi Gong San (Five-Ingredient Decoction for Enhancing Function), Gui Pi Tang (Restore the Spleen Decoction), Shi Quan Da Bu Tang (Ten Substance Great Tonifying Decoction) (all see "Deficiency Syndromes").
(This refers to Gui Zhi Tang with doubled Gan Cao, see "Taiyang Channel Syndrome")
Dai Di Dang Wan (Substitute Di Dang Pill) (see "Strangury Syndromes").
Zhong Feng (Wind Stroke) refers to true wind stroke, which differs into involvement of the Fu organs, Zang organs, and channels and collaterals. Involvement of the Fu organs indicates the pathogen is at the exterior level; the external manifestations correspond to the Six Channels syndromes, identical to the transmission patterns of Shang Han (Cold Damage). For Taiyang channel involvement, use Gui Zhi Tang. For Yangming channel involvement, use Ge Gen Tang (Kudzu Root Decoction) added with Gui Zhi. For Shaoyang channel involvement, use Xiao Chai Hu Tang (Minor Bupleurum Decoction) added with Gui Zhi. These methods are fully detailed in the Shang Han section and will not be repeated here. Involvement of the Zang organs indicates the pathogen is at the interior level; the patient experiences vertigo, sudden collapse, coma, or phlegm-rattle sounds like sawing wood. Treatment should differentiate between Zang/Fu organs and cold/heat patterns. If the person habitually has deficiency-cold, or suddenly contracts new cold, wind and water interact, causing ice-cold penetration; thus, the wind becomes Cold Wind. If the person habitually has accumulated heat, or suppressed fire erupts suddenly, wind rides the fire's momentum, and fire borrows wind's power; thus, the wind becomes Hot Wind. Hot Wind often presents as Bi Syndrome (Closed Syndrome), characterized by locked jaw and clenched fists. The treatment method is to disperse wind and open the orifices: first blow Chu Bi San (Nose-Snuffing Powder) into the nose, then administer Niu Huang Wan (Calculus Bovis Pill). If there is constipation, abdominal fullness and distension, and extreme fire intensity, use San Hua Tang (Three Transformations Decoction) to purge. Cold Wind often presents as Tuo Syndrome (Collapsed Syndrome), characterized by flaccid hands (Spleen exhaustion), closed eyes (Liver exhaustion), open mouth (Heart exhaustion), snoring respiration (Lung exhaustion), and urinary incontinence (Kidney exhaustion). Additionally, staring eyes, head shaking upward, stiff hair, and bead-like sweat indicate complete collapse. The treatment method is to warm and tonify Yuan Qi (Original Qi); urgently administer a large dose of Fu Zi Li Zhong Tang (Aconite Decoction for Regulating the Middle). If phlegm-saliva is abundant, administer San Sheng Yin (Three Lives Drink) added with Ren Shen. Occasionally, cold-phlegm obstruction lies between closed and collapsed states, making immediate tonification unsuitable; use Ban Xia (Pinelliae Rhizoma) and Ju Hong (Citri Reticulatae Pericarpium Rubrum), each 1 liang, decoct strongly to one cup, mix with fresh Ginger Juice, and administer frequently; the patient will revive, after which tonification can be adjusted according to deficiency. However, reflecting on my life experience, I have cured many cases with Fu Zi Li Zhong Tang, and also frequently with Niu Huang Wan, but I have never used San Hua Tang. This must be due to differences in climate, geography, and circumstances. Nevertheless, cold and hot remedies stand firmly apart; ancient formulas exist, and methods cannot be erased. Therefore, both are preserved for reference. Involvement of channels and collaterals means the pathogen is within the meridians. Symptoms include facial deviation and hemiplegia. The principal formula is Da Qin Jiao Tang (Large Gentiana Root Decoction). If affected on the left, double the dosage of Si Wu Tang (Four Substance Decoction); if on the right, assist with Si Jun Zi Tang (Four Gentlemen Decoction). If both sides are affected, assist with Ba Zhen Tang (Eight Treasure Decoction) and Hu Gu Jiao Wan (Tiger Bone Gelatin Pill). This is the major principle for treating true wind stroke.
