Bi Syndrome refers to pain. When Wind, Cold, and Dampness mix and invade together, they form Bi Syndrome. When Wind predominates, it is called Moving Bi (Xing Bi), characterized by wandering pain without fixed location. When Cold predominates, it is called Painful Bi (Tong Bi), characterized by contraction and pain of sinews and bones. When Dampness predominates, it is called Fixed Bi (Zhuo Bi), characterized by edema, heaviness, and drooping sensation. Since it is termed "predominance," the pathogenic factors have specific biases. To treat Moving Bi, dispelling Wind is primary, assisted by eliminating Cold and Dampness; generally, blood-nourishing formulas are incorporated, following the principle that treating Wind requires first treating the Blood; when Blood flows, Wind extinguishes itself. To treat Painful Bi, dispelling Cold is primary, assisted by dispersing Wind and drying Dampness; generally, Fire-warming formulas are incorporated, following the principle that Heat promotes flow while Cold causes stagnation; free flow means no pain, pain means obstruction. To treat Fixed Bi, drying Dampness is primary, assisted by expelling Wind and scattering Cold; generally, Spleen-tonifying formulas are incorporated, because when Earth (Spleen) is robust, it overcomes Dampness, and sufficient Qi eliminates numbness and stiffness. The general treatment uses Juanbi Tang (Eliminate Bi Decoction) with modifications; for severe pain, Songzhi Jiu (Pine Branch Wine) is added as an adjunct. There is also a condition where Bi Syndrome persists for a long time, resulting in withered and thin legs and feet, but swollen knees, named Crane Knee Wind (Hexi Feng). This is due to deficiency of the Three Yin channels, with Cold pathogens invading the collaterals, leading to this syndrome. It is appropriate to take Hugujiao Wan (Tiger Bone Gelatin Pill) and apply Puji Wanquan Gao (Universal Salvation Plaster) externally, so that gradual recovery can occur. If this opportunity is missed and not treated, it becomes a stubborn disease, rendering the person disabled.
