Taiyang disease (Disease of the Taiyang channel), presenting with Guizhi Tang syndrome (Syndrome suitable for Cinnamon Twig Decoction), the physician inappropriately applied purgation therapy, resulting in persistent diarrhea. If the pulse is rapid and forceful (Cu Mai), it indicates that the exterior pathogen has not yet been resolved; if accompanied by wheezing and sweating, Gegen Huanglian Huangqin Tang (Kudzu Root, Coptis, and Scutellaria Decoction) should be administered as the primary treatment.
Commentary: In this syndrome, a rapid and forceful pulse indicates heat at the exterior; wheezing and sweating indicate heat within the interior. Therefore, this condition is classified as an Exterior-Interior Heat Syndrome.
Gegen Huanglian Huangqin Tang (Kudzu Root, Coptis, and Scutellaria Decoction)
Gegen (Puerariae Lobatae Radix, 4 qian) Gancao (Glycyrrhizae Radix et Rhizoma, roasted, 1 qian) Huanglian (Coptidis Rhizoma, 1 qian) Huangqin (Scutellariae Radix, 1.5 qian). First decoct Gegen to remove the foam, then add the remaining herbs and decoct together for administration. Commentary: A rapid and forceful pulse signifies that the exterior pathogen remains unresolved. Internal wheezing and sweating indicate heat in the chest and diaphragm. Therefore, the acrid-cool nature of Gegen is used to unblock the stagnation of the Yangming channel, while the bitter-cold properties of Huangqin and Huanglian are employed to clear the Ministerial Fire and Sovereign Fire.
In Shanghan (Cold Damage) disease, if the pulse is knotted and intermittent (Jie Dai Mai) accompanied by palpitations (Xin Dong Ji), Zhigancao Tang (Honey-Fried Licorice Decoction) is the primary treatment. In Shaoyin disease, if there is body pain, coldness in the hands and feet, bone and joint pain, and a deep pulse (Chen Mai), Fuzi Tang (Aconite Decoction) is the primary treatment.
If Shaoyin disease presents after one or two days, with a normal sensation in the mouth (no bitterness or dryness) and aversion to cold specifically in the back, moxibustion should be applied, and Fuzi Tang (Aconite Decoction) should be administered internally.
Commentary: Among the aforementioned syndromes, some present with a knotted and intermittent pulse indicating deficiency of both Qi and Blood; others present with a deep pulse and aversion to cold indicating failure of True Yang. Thus, these conditions are collectively categorized as Exterior-Interior Deficiency Syndromes.
Zhigancao Tang (Honey-Fried Licorice Decoction)
Gancao (Glycyrrhizae Radix et Rhizoma, 2 qian) Guizhi (Cinnamomi Ramulus, 1.5 qian) Shengjiang (Zingiberis Rhizoma Recens, 1.5 qian) Renshen (Ginseng Radix, 1 qian) Ejiao (Asini Corii Colla, 2 qian) Dazao (Ziziphi Jujubae Fructus, 2 pieces) Mahuang (Ephedrae Herba - *Note: Text says Mahren/Huomaren contextually usually Semen Cannabis, but text says Mahren which likely refers to Huomaren/Cannabis Seed in some editions or typo for Mahuang? Context "Ma Ren" usually means Hemp Seed. WHO term: Cannabis Sativae Semen*), Maidong (Ophiopogonis Radix, 1.5 qian) Shengdi (Rehmanniae Radix, 6 qian). Decoct with two cups of water and one cup of clear wine until reduced to eight-tenths. Add Ejiao and melt it; take warm. Commentary: In this syndrome, Ministerial Fire flares upward, damaging Xin Metal (Lung); Jia Wood (Liver/Gallbladder Qi) becomes stagnant upward, and Wu Earth (Stomach) is suppressed. With both Earth and Metal compromised, the disease transmits to the Yangming channel, injuring Middle Qi. Therefore, Renshen, Gancao, and Dazao are used to benefit Stomach Qi and nourish Spleen Essence; Ejiao, Shengdi, and Mahren (Cannabis Sativae Semen) are used to moisten the meridians and hydrate dryness; Shengjiang and Guizhi are used to promote the circulation of Ying Blood and resolve stasis; Maidong is used to clear the dryness-heat of Lung Metal.
Fuzi Tang (Aconite Decoction)
Fuzi (Aconiti Lateralis Radix Praeparata, 2 qian) Fuling (Poria, 3 qian) Renshen (Ginseng Radix, 2 qian) Baizhu (Atractylodis Macrocephalae Rhizoma, 4 qian) Shaoyao (Paeoniae Radix, 3 qian). Decoct with two cups of water until reduced to eight-tenths; take warm. Commentary: Cold water causes Wood to sink, threatening the extinction of Sheng Qi (Vital Energy). Therefore, Fuzi is used to warm the cold of Gui Water (Kidney Water); Shaoyao is used to clear Wind from Yi Wood (Liver Wood); Renshen, Baizhu, and Fuling are used to fortify Earth and drain dampness.
