Treatise
The *Neijing* (Yellow Emperor's Inner Classic) states: Various heat diseases leading to dizziness and confusion, sudden aphonia, coma, restlessness, mania, cursing, and fright; swelling, pain, and soreness; rebellious qi rushing upward; tremors and chills as if losing spiritual control; vomiting, sores, and ulcers; throat obstruction; tinnitus and deafness; vomiting with overflow and inability to eat; blurred vision; acute diarrhea; sudden illness and sudden death—all these belong to Fire. Zhu Danxi said: Taiji moves to generate Yang, stillness generates Yin. Yang moves and transforms; Yin is still and combines, thereby generating Water, Fire, Wood, Metal, and Earth, each with its own nature. Only Fire has two types: Jun Fire (Ministerial Fire, referring to the Heart fire, normal physiological fire) and Xiang Fire (Assistant Fire, referring to the Mingmen fire, the heavenly endowment of life force). Fire is Yin internally and Yang externally, primarily characterized by movement; thus, all phenomena of movement belong to Fire. In terms of name, form and substance generate each other and correspond to the Five Elements, hence it is called Jun Fire. In terms of position, it arises from emptiness, maintains its station receiving commands, and manifests through movement, hence it is called Xiang Fire. Heaven governs the generation of all things, thus constantly in motion; humans possess life, also constantly in motion. The reason for this constant motion is the action of Xiang Fire, and so on. He further stated: The natures of the Five Elements are moved by external stimuli and cannot remain still. This "movement" refers to the Five Fires mentioned in the *Neijing*. Xiang Fire rises easily; if the five emotional fires and Jueyang Fire fan each other, they will move recklessly. Fire arises from reckless movement, changes unpredictably, exists at all times, and boils down True Yin. Yin deficiency leads to disease; Yin exhaustion leads to death. My commentary: The Heart is Jun Fire, while Xiang Fire resides in the Liver and Kidney organs. This corresponds to the *Neijing*'s statement that "one Water cannot overcome two Fires." The fires of the five emotions, moved by external stimuli, correspond to the *Neijing*'s statement that "one Water cannot overcome five Fires." The principles of the Five Elements are shared by Heaven and Humanity. Understanding this allows one to thoroughly comprehend the Yin and Yang of creation within one's mind, and to clearly distinguish the excess or deficiency of pathogenic Fire. By tonifying or purging accordingly and using medication to restore balance, the efficacy of curing disease will be as accurate as an arrow hitting the bullseye. How can students of medicine neglect this?
Pulse Diagnosis
A pulse that is floating, surging, and rapid indicates Deficient Fire. A pulse that is deep, excessive, and large indicates Excess Fire.
A surging, rapid pulse appearing at the Left Cun position indicates Heart Fire; at the Right Cun, Lung Fire; at the Left Guan, Liver Fire; at the Right Guan, Spleen Fire; and at both Chi positions, Kidney Meridian and Mingmen Fire. In men, a surging and large pulse at both Chi positions inevitably indicates seminal emission due to盛 (excess) Yin Fire.
Methods
Zhu Danxi said: Cases where Yin deficiency causes Fire to flare up are difficult to treat.
Excess Fire can be purged, using formulas such as Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxicity).
Deficient Fire can be tonified, using herbs such as Renshen (Ginseng), Baizhu (Atractylodes Macrocephala Rhizome), and Sheng Gancao (Raw Licorice Root). (By "Excess," we mean the pathogen is excessive; by "Deficiency," we mean the healthy Qi is deficient.)
Stagnant Fire can be dispersed, depending on which meridian is affected.
If constrained by Wind-Cold externally, dispersion can be used. For mild cases, descending methods may be used; for severe cases, ascending methods should follow its nature.
When Fire is excessively strong, cold and cooling herbs should not be used abruptly; warm dispersal must be employed first.
For urgent and severe Fire, it must be moderated first. Sheng Gancao (Raw Licorice Root) has both purgative and moderating effects; Renshen (Ginseng) and Baizhu (Atractylodes Macrocephala Rhizome) can also be used.
For individuals with robust constitution, abundant Qi, and severe Fire leading to mania, direct treatment (treating heat with cold) can be used, such as Xiaoshuo (Mirabilite), Dahuang (Rhubarb Root and Rhizome), or Ice Water.
For individuals with weak constitution and severe Fire leading to mania, Ginger Decoction should be given. If direct treatment with Ice Water is administered, death will occur immediately.
There is a method of lowering Fire by tonifying Yin, using herbs such as Chao Huangbai (Fried Phellodendron Bark) and Sheng Dihuang (Fresh Rehmannia Root).
Whenever there is an excess of Qi, it becomes Fire. Qi arising from the left side indicates Liver Fire. Qi arising from below the umbilicus indicates Yin Fire.
My commentary: "Excess of Qi" does not refer to true Qi, but rather to the Jueyang Fire of the five emotions moving and becoming pathogenic Qi.
Fever in drinkers is difficult to treat. Fever in non-drinkers caused by alcohol consumption is also difficult to treat.
If light pressure makes the heat feel more intense, but heavy pressure makes it less intense, the heat is in the muscle layer. Clearing methods should be used, such as Digupi (Lycium Bark), Maidong (Ophiopogon Tuber), and Zhuru (Bamboo Shavings).
If heavy pressure makes the heat intense and scalding, but light pressure feels no heat, the pathology is within the muscles. Dispersing methods should be used, such as Li Dongyuan's Shengyang Sanhuo Tang (Ascending Yang and Dispersing Fire Decoction) or Huoyu Tang (Fire Stagnation Decoction).
Restlessness and irritability indicate that Qi rises along with Fire.
Mutong (Akebia Stem) descends and purges Small Intestine Fire. Renzhongbai (Calcined Human Urine Deposit) purges Liver Fire. (Renzhongbai is the deposit from urine jars; Qishi [Processed Urine Sediment] is similar.)
Huangqin (Scutellaria Root) and Huanglian (Coptis Rhizome) stir-fried with Pig Bile can purge Liver and Gallbladder Fire.
Huangbai (Phellodendron Bark) added with Xixin (Asarum Root) can purge Bladder Fire.
Qingdai (Indigo Naturalis) can purge stagnant Fire of the Five Zang Organs. Xuanshen (Scrophularia Root) can purge rootless wandering Fire.
Urination reduces Fire rapidly. Shanzhizi (Gardenia Fruit) guides Fire out through urination; its property allows it to descend曲折ly (curving downwards), a fact unknown to many.
In some people, Qi rises like fire from the feet into the abdomen. This indicates extreme deficiency, as if the Fire arises from the depths of the earth. Ten out of ten such cases are incurable. The treatment method is to take Siwu Tang (Four Substances Decoction) added with Fire-lowering herbs, and externally apply Fuzi powder (Aconite Root) mixed with saliva to the Yongquan acupoint (KI1) on the sole of the foot to guide Fire downward.
My commentary: This statement is not entirely complete. If a person suffering from consumptive weakness and Yin deficiency presents this, it should indeed be treated as Yin deficiency. However, if a robust person presents this, it is a condition of Dampness stagnation transforming into Heat. I once walked barefoot in the rain with wet clothes and contracted this syndrome. I took pills made of Cangzhu (Atractylodes Rhizome) and Huangbai (Phellodendron Bark) added with Niuxi (Achyranthes Root) and Fangji (Stephania Root) and was cured. Several subsequent physicians verified this method. If mistakenly treated as Yin deficiency, it would develop into Wei Syndrome (flaccidity syndrome), which would be severe.
(Above are twenty-one treatment rules from Danxi)
Zuojin Wan (Left Metal Pill, also known as Huling Wan) Treats Liver Fire.
Huanglian (Coptis Rhizome) (6 liang) Wuzhuyu (Evodia Fruit) (1 liang, soaked in boiling water for half an hour, then dried and roasted)
Grind into fine powder, make pills with rice porridge, and swallow with decoction of Baizhu (Atractylodes Macrocephala Rhizome) and Chenpi (Tangerine Peel).
Dabu Wan (Great Tonification Pill) Treats Yin Fire.
Huangbai (Phellodendron Bark) (Remove coarse bark, cut finely, place in a new tile dish, stir-fry with salt and wine until brown)
Grind into fine powder, make pills with rice porridge or water, and swallow with decoction of Siwu Tang (Four Substances Decoction). It is also said: For Qi deficiency, swallow with Sizijunzi Tang (Four Gentlemen Decoction); for Blood deficiency, swallow with Siwu Tang (Four Substances Decoction).
Yiqing Wan (Suppressing Green Pill) Treats Liver Fire.
Huanglian (Coptis Rhizome) (Any amount)
Grind into fine powder, make pills with rice porridge, and swallow with plain boiled water.
Shigao Wan (Gypsum Pill) Purges Stomach Fire, and treats food retention and Phlegm-Fire.
Shigao (Gypsum Fibrosum) (Amount unspecified in original text)
Grind into fine powder, make pills with vinegar, size of mung beans, and swallow with thin rice gruel.
Siwu Tang (Four Substances Decoction) added with Bai Ma Jinggu (White Horse Tibia Bone) lowers Yin Fire and replaces Huangqin (Scutellaria Root) and Huanglian (Coptis Rhizome).
For fever due to Yin deficiency, use Siwu Tang (Four Substances Decoction) added with Chao Huangbai (Fried Phellodendron Bark) and Zhimu (Anemarrhena Rhizome). This is an excellent formula for lowering Fire and tonifying Yin. For severe cases, add Guiban (Tortoise Shell). If accompanied by Qi deficiency, add Renshen (Ginseng), [missing character, likely Huangqi/Astragalus], and Baizhu (Atractylodes Macrocephala Rhizome).
Heat in the palms belongs to Heat Stagnation. Use Huoyu Tang (Fire Stagnation Decoction), or pills made of Zhizi (Gardenia Fruit), Xiangfu (Cyperus Rhizome), Baizhi (Angelica Dahurica Root), Banxia (Pinellia Tuber), and Chuanxiong (Ligusticum Rhizome) bound with Qu paste.
(Above are three treatment rules from Danxi)
Huoyu Tang (Fire Stagnation Decoction) (Li Dongyuan) Treats heat in the four limbs and heat in the five centers (palms, soles, and chest). The cause is Heat hidden in the Earth element, or resulting from Blood deficiency, or from Stomach deficiency and excessive consumption of cold foods, which suppresses Yang Qi within the Spleen Earth.
Qianghuo (Notopterygium Rhizome) Shengma (Cimicifuga Rhizome) Gegen (Pueraria Root) Shaoyao (Peony Root) Renshen (Ginseng) (each 7 fen) Chaihu (Bupleurum Root) Gancao (Licorice Root) (raw, each 3 fen) Fangfeng (Saposhnikovia Root) (2.5 fen) Congbai (Green Onion White) (5 stalks)
Cut finely, as one dose. Add 1.5 cups of water, decoct to 1 cup, and take slightly warm.
Shengyang Sanhuo Tang (Ascending Yang and Dispersing Fire Decoction) (Li Dongyuan) Treats fatigue and heat in the four limbs in men and women, heat between bones and tendons, and surface heat like burning embers, scalding to the touch. This condition often results from Blood deficiency, or from Stomach deficiency and excessive consumption of cold foods, which suppresses Yang Qi in the Spleen Earth. When Fire is stagnant, disperse it.
Shengma (Cimicifuga Rhizome) Gegen (Pueraria Root) Duhuo (Angelica Pubescens Root) Qianghuo (Notopterygium Rhizome) Baishaoyao (White Peony Root) Renshen (Ginseng) (each 6 fen) Gancao (Licorice Root) (roasted, 1 fen) Chaihu (Bupleurum Root) (3 fen) Fangfeng (Saposhnikovia Root) (3.5 fen) Sheng Gancao (Raw Licorice Root) (2 fen)
Cut finely, as one dose. Add 3 slices of Shengjiang (Fresh Ginger), 1.5 cups of water, decoct to 1 cup, and take hot. Avoid raw and cold foods.
Xie Yinhuo Shengyang Tang (Purging Yin Fire and Ascending Yang Decoction) (Li Dongyuan) Treats muscle and head heat, red face, poor appetite, wheezing and cough with profuse phlegm, and a slow/weak, wiry, or floating/rapid pulse at the Right Guan position.
Qianghuo (Notopterygium Rhizome) Gancao (Licorice Root) (roasted) Huangqi (Astragalus Root) Cangzhu (Atractylodes Rhizome) (each 1 fen) Shengma (Cimicifuga Rhizome) (8 fen) Chaihu (Bupleurum Root) (1.5 qian, remove radicle) Renshen (Ginseng) Huangqin (Scutellaria Root) (each 7 fen) Huanglian (Coptis Rhizome) (wine-fried, 5 fen) Shigao (Gypsum Fibrosum) (5 fen, do not use in autumn/winter)
Cut finely, as one dose. Add 2 cups of water, decoct to 1 cup, and take warm.
Zishen Wan (Kidney Nourishing Pill) (Li Dongyuan) Lowers Kidney Fire.
Huangbai (Phellodendron Bark) (2 liang, mixed with wine and dried in shade) Zhimu (Anemarrhena Rhizome) (2 liang, remove hairs, soak in wine and dry in shade) Rougui (Cinnamon Bark) (1 qian, remove coarse bark)
The above two herbs have Yin properties and resonate with Kidney Qi, thus they can tonify the Kidney and purge Lower Jiao Fire. Rougui shares the nature of Fire, so the method of "using cold with heat" is applied. In diseases located in the Lower Jiao, thirst is usually absent.
Grind into fine powder, make pills with cooked water, and swallow with boiling water.
Liangge San (Cooling the Diaphragm Powder) (Liu Hejian)
Dahuang (Rhubarb Root and Rhizome) (soaked in wine, wrapped in paper and baked) Pomiao (Mirabilite) Gancao (Licorice Root) (each 5 fen) Lianqiao (Forsythia Fruit) (1 qian) Zhizi (Gardenia Fruit) Huangqin (Scutellaria Root) Bohe (Mint Herb) (each 2.5 fen) Danzhuye (Bamboo Leaf) (5 leaves)
Cut finely, as one dose. Add 1 cup of water, decoct to 0.8 cup, strain, add 1 spoonful of honey, mix well, and take.
Sanbu Wan (Three Tonics Pill) (Liu Hejian) Purges Triple Burner Fire.
Huangqin (Scutellaria Root) Huanglian (Coptis Rhizome) Huangbai (Phellodendron Bark) (equal parts)
Grind into fine powder, make pills with fresh drawn water, and take.
Zixue (Purple Snow) (Heji Fang) Treats unresolved internal and external烦热 (restless heat), mouth sores, mania and running about, resolves various heat toxins and pathogenic heat, and pediatric convulsions.
Huangqin (Scutellaria Root) (100 liang) Hanshuishi (Halloysitum) Cishi (Magnetitum) Shigao (Gypsum Fibrosum) Huashi (Talcum) (ground, each 2 jin)
Boil the above in 1 dan of water until reduced to 4 dou. Strain, then add the following:
Gancao (Licorice Root) (roasted, 8 liang) Qingmuxiang (Vital Tree Root) Sheng Xijiao (Rhino Horn) Lingyangjiao (Antelope Horn) Chenxiang (Aquilaria Wood) (each 5 liang) Dingxiang (Clove) (1 liang) Shengma (Cimicifuga Rhizome) Xuanshen (Scrophularia Root) (each 1 jin)
Boil again until reduced to 1.5 dou. Then add the following:
Xiaoshi (Niter) (Mangxiao/Mirabilite can be used, 2 jin) Pomiao (Mirabilite) (pure, 10 jin)
Add to the previous decoction. Simmer over low heat, stirring constantly with a willow branch. When reduced to 7 jin, pour into a basin of water. After half a day, when the medicine solidifies, add the following and mix evenly.
Zhusha (Cinnabar) (3 liang) Dangmenzi (Moschus/Musk) (1.25 liang)
Simmer until it forms a purple snow-like frost. Take 1 qian or 2 qian per dose, mixed with cool water. Dosage for adults and children should be adjusted according to circumstances. Take after meals.
Miaoxiang Wan (Wonderful Fragrance Pill) (Heji Fang) Treats epidemic cold damage, resolves five poisons, tidal heat, accumulated heat, and pediatric convulsions.
Badou (Croton Seed) (cleaned, 350 grains) Niuhuang (Calculus Bovis/Bovine Calculus) Piannao (Borneol) Nifen (Calomelas) Muxiang (Moschus/Musk) (each 7.5 qian) Chensha (Cinnabar) (9 liang) Jinbo (Gold Foil) (90 pieces)
Grind extremely fine. Refine 6 liang of wax, add 7.5 qian of honey, refine together until uniform. Each liang of powder makes 30 pills. Take 1 pill per dose; for children, 2 pills the size of a mung bean, taken with plain water. The *Bacui Fang* lacks Gold Foil but contains Mercury and Naosha (Sal Ammoniac), treating long-standing accumulations.
Commentary: Naosha (Sal Ammoniac) should not be used lightly. If not refined precisely, it harms people. Be cautious!
Xiexin Tang (Purge Heart Decoction) (Heji Fang) Treats latent heat in the Heart Meridian.
Take only Huanglian (Coptis Rhizome), grind into fine powder, mix with water, and take.
Maidong Tang (Ophiopogon Decoction) (*Qianjin Fang*) Treats post-illness Heat-Fire attacking the Lungs, causing coughing and spitting blood, distension and fullness in the chest and hypochondrium, upper asthma and急促 (rapid breathing), emaciation, heat in the five centers, thirst, and烦闷 (restlessness).
Maidong (Ophiopogon Tuber) (remove heart) Sangbaipi (Mulberry Root Bark) (honey-fried) Sheng Dihuang (Fresh Rehmannia Root) (each 7 fen) Banxia (Pinellia Tuber) (soaked in water 7 times, remove skin and navel) Ziwan (Aster Root) Jiegeng (Platycodon Root) Danzhuye (Bamboo Leaf) Mahuang (Ephedra Herb) (each 5 fen) Wuweizi (Schisandra Berry) Gancao (Licorice Root) (each 3 fen)
Cut finely, as one dose. Add 3 slices of Shengjiang (Fresh Ginger), 1.5 cups of water, decoct to 1 cup, and take warm.
My commentary: This formula states that post-illness Heat-Fire attacks the Lungs; can Mahuang (Ephedra Herb) really be used? If there is thirst and restlessness, Banxia (Pinellia Tuber) can also be omitted. It is advisable to remove these two herbs and add贝母 (Fritillaria Bulb) and Tianmendong (Asparagus Tuber) for a safer formula.
Zhizi Ren Tang (Gardenia Kernel Decoction) (Liu Hejian) Treats fever and tidal fever, mania and irritability, red face, and sore throat.
Zhizi Ren (Gardenia Kernel) Chishaoyao (Red Peony Root) Daqing (Isatis Root) Zhimu (Anemarrhena Rhizome) (each 5 fen) Shengma (Cimicifuga Rhizome) Huangqin (Scutellaria Root) Shigao (Gypsum Fibrosum) (each 1 qian) Xingren (Apricot Seed) (boiled in water, remove skin and tip, 7.5 fen) Chaihu (Bupleurum Root) (1.2 qian) Sheng Gancao (Raw Licorice Root) (1 qian) Douchi (Fermented Soybean) (50 grains)
Cut finely, as one dose. Add 2 cups of water, decoct to 0.8 cup, strain, and take warm.

Dahuang (Rhubarb Root and Rhizome) Huanglian (Coptis Rhizome) Huangqin (Scutellaria Root) (equal parts)
Grind into fine powder, make pills with refined honey, size of small beans. Take 5 pills per dose, gradually increasing to 10 pills, swallowed with plain water. After one month of use, bowel movements become rapid like a galloping horse.
(Danxi Huotao) states: To clear Damp-Heat from the Upper Jiao, one must use wine-washed Huangqin (Scutellaria Root) to purge Lung Fire. If there is Excess Heat in the Lung, it is suitable; if there is Deficient Heat and Huangqin is used, it will damage Lung Qi. One must first use Tianmendong (Asparagus Tuber) to secure Lung Qi before using it. To clear Damp-Heat and pain from the Middle Jiao, one must use Huanglian (Coptis Rhizome) to purge Heart Fire. If there is Excess Heat in the Middle Jiao, it is suitable; if there is Spleen/Stomach Qi deficiency failing to transport, or stagnant Heat in the Middle Jiao, one should use Fuling (Poria), Baizhu (Atractylodes Macrocephala Rhizome), Huangqin (Scutellaria Root), and Gegen (Pueraria Root) instead. For heat and vexation in the chest, Zhizi (Gardenia Fruit) must be used. It is appropriate for Excess Heat; for Deficient Vexation, tonifying herbs should be primary, such as Renshen (Ginseng), Baizhu (Atractylodes Macrocephala Rhizome), Huangqin (Scutellaria Root), Shaoyao (Peony Root), Fuling (Poria), Maidong (Ophiopogon Tuber), and Dazao (Jujube). For Damp-Heat swelling/pain in the Lower Jiao, and Fire pathogens in the Bladder, wine-washed Fangji (Stephania Root), Caolongdan (Gentiana Root), Huangbai (Phellodendron Bark), and Zhimu (Anemarrhena Rhizome) are indeed effective. For obese, white-complexioned individuals with Qi deficiency, Baizhu (Atractylodes Macrocephala Rhizome), Cangzhu (Atractylodes Rhizome), Nanxing (Pinellia Ternata), Huashi (Talcum), and Fuling (Poria) are suitable. For dark, thin individuals with Damp-Heat swelling/pain in the Lower Jiao, Danggui (Angelica Sinensis Root), Honghua (Safflower), Taoren (Peach Seed), Niuxi (Achyranthes Root), and Binglang (Areca Seed) must be used. Chaihu (Bupleurum Root) purges Liver Fire and must be assisted by Pian Qin (Scutellaria Root). Pian Qin also purges Lung Fire and must be assisted by Sangbaipi (Mulberry Root Bark). If the root resembles a rat's tail, it purges Large Intestine Fire. Huanglian (Coptis Rhizome) purges Heart Fire. If stir-fried with Pig Bile and assisted by Caolongdan (Gentiana Root), it greatly purges Gallbladder Fire. Baishaoyao (White Peony Root) purges Spleen Fire. If used in winter, it must be soaked in wine and fried, due to its sour and cold nature. Zhimu (Anemarrhena Rhizome) and Huangbai (Phellodendron Bark) purge Kidney Fire and Bladder Fire. Zhizi (Gardenia Fruit) purges Triple Burner Fire. For Upper and Middle Jiao, use with husk. For Lower Jiao, remove husk, wash to remove yellow liquid, fry until charred, and grind fine. Renzhongbai (Calcined Human Urine Deposit) not only purges Liver Fire but also Triple Burner and Bladder Fire, exiting via urination, as the Bladder is its natural pathway.
(Ancestral Formula)
Renzhongbai San (Human Urine Deposit Powder) Treats Yin deficiency with Fire excess, and heat in the five centers.
Renzhongbai (Calcined Human Urine Deposit) (2 liang) Huangbai (Phellodendron Bark) (stir-fried with salt and wine until brown) Sheng Gancao (Raw Licorice Root) Qingdai (Indigo Naturalis) (each 5 qian)
Grind into fine powder. Take 2 qian per dose, mixed with urine from a young boy.
(Medical Case)
Madam Luo, over forty years old, had fever at night which subsided in the morning, with persistent heat in the five centers. After six months, she sought my treatment. All six pulses were rapid, deep, firm, and unresponsive to light pressure. I prescribed Li Dongyuan's Shengyang Sanhuo Tang (Ascending Yang and Dispersing Fire Decoction). After four doses, the fever decreased by more than half, and her chest felt clearer than before. After two more doses, the fever completely subsided. Later, she took Siwu Tang (Four Substances Decoction) added with Zhimu (Anemarrhena Rhizome) and Huangbai (Phellodendron Bark), with a small amount of Chao Ganjiang (Fried Dried Ginger), taking over twenty doses until fully recovered.
