Simultaneous vomiting and diarrhea, with cramps in both legs. After undergoing *Nasha* (a folk therapy involving pinching the skin) and injections, the entire body turned red and purple, with bleeding from punctured fingers and toes. The patient fainted and was unable to speak. The pulse exhibited four major principles (?), but presented as deep and unrising; there were no signs of cold extremities, abdominal pain, or thirst. The cause was diagnosed as summerheat pathogen combined with consumption of raw and cold foods. I instructed someone to find several raw taro roots (*Dioscorea esculenta*), then dig a small pit about one chi deep inside the threshold. Freshly drawn water was poured into the pit without stirring or stopping, allowing it to rotate, while preventing dirt and dust from falling in. The water was stirred by hand for twenty minutes, then scooped out with a wooden spoon into a bowl, clarified, and boiled in a clay pot. First, the patient was asked to chew a piece of raw taro. When asked if it felt numbing, the patient did not respond but showed an expression of desire to eat. Another piece was given to chew, with instructions to spit out the residue and not swallow it. Asked if it tasted sweet, the patient replied it was fresh and sweet. The patient then requested another piece to chew. After chewing, the patient let out a loud cry, complaining of a strange taste in the mouth, but was able to lift their feet, indicating the cramps had ceased. Soon after, the patient expelled a basin of sour-smelling water containing undiginned jelly noodles and glutinous rice balls. The patient was then instructed to drink the clarified water from the threshold pit along with child's urine (*Tongbian*). Three hours later, vomiting stopped, and the legs were no longer curled. However, diarrhea worsened significantly, with three basins of water expelled within ten minutes, all having a sour odor. I advised purchasing four pills of *Baohe Wan* (Preserve Harmony Pill), to be taken over two days. Due to severe stomach cold, porridge and rice were temporarily prohibited; instead, lily bulb powder and lotus root starch were allowed. Rice could only be introduced when the patient could smell its aroma; otherwise, dysentery might develop.
After taking *Baohe Wan*, all symptoms resolved, and no further medication was taken.
11. Intercourse after Drunkenness and Overeating
Male, 28 years old.
Admitted to Nanchang for high fever, cured after three doses of medicine. Upon returning home, he purchased sorghum wine and large fish. His wife, anticipating his return, bought meat and stewed it in preparation. Upon arrival, he ate excessively for temporary pleasure. Unexpectedly, the next morning, he became unconscious. He was carried to Jiangqiao Clinic. His complexion was not severely emaciated; he could move his hands and open his eyes but remained silent. His wife recounted the history. I diagnosed this as food stagnation following illness and treated with a digestive method:
Hawthorn Fruit (*Shanzha*), Malt (*Maiya*), Medicated Leaven (*Shenqu*), Prepared Pinellia Curd (*Faxia Qu*), and Atractylodes Rhizome (*Cangzhu*) each 2 qian; Magnolia Bark (*Chuanpu*) 1.5 qian; Charred Zong Leaf (*Chenzong Tan*) one piece; Charred Pork Bone (*Rugu*) one piece. Decoct together and take orally.
After administration, speech gradually returned, followed by complaints of penile pain. Recognizing intercourse after drunkenness and overeating likely caused heat injury to the urethra, the prescription was modified:
Prunella Spike (*Xiakucao*), Pteris Multifida Herb (*Fengweicao*), Amber Powder (*Hupo Xie*), Alpinia Oxyphylla Fruit (*Yizhiren*), Silkworm Frass (*Can Sha*), Goldthread Rhizome (*Haijinsha*), and *Yiyuan San* each 2 qian, 1.5 qian, 1.5 qian, 2 qian, 2 qian, 1.5 qian, and 3 qian respectively. Additionally, Pig Penis (*Zhubian*) from a butcher shop, calcined to preserve efficacy, was decocted with the herbs. Two doses resulted in recovery.
12. Kidney Diarrhea
Male, 50 years old.
From a wealthy family with a licentious lifestyle, suffering from dawn diarrhea for over two years. Multiple treatments with *Lizhong Tang* (Regulate the Middle Decoction) and *Zhenren Yangzang Wan* (True Man Nourish Viscera Pill) were ineffective. Dawn diarrhea is identified as Kidney Diarrhea (*Xunxie*), distinct from gastrointestinal disorders occurring at dawn. This is due to Kidney Yin deficiency. As stated in the classics: "For Yin deficiency, tonify with flavor." Prescribed *Qing'e Wan*:
Eucommia Bark (*Duzhong*) 3 qian, Green Salt (*Qingyan*) substituted with Alpinia Oxyphylla Fruit (*Yizhiren*) 2 qian, Psoralea Fruit (*Buguzhi*) 4 qian. Grind into powder, wrap in cloth. Daily, use one pig kidney, sliced into four pieces, stewed with the powder and consumed.
After three doses, switched to *Sishen Wan* (Four Spirits Pill):
Schisandra Berry (*Wuweizi*) 2 qian, Alpinia Oxyphylla Fruit (*Yizhiren*) 3 qian, Nutmeg Seed (*Roudoukou*) 4 qian, Evodia Fruit (*Wuzhuyu*) 3 qian. Sixteen doses resulted in recovery.
Note: Treatments for Kidney Yin and Yang deficiencies differ. Kidney Yang deficiency presents with Kidney Diarrhea and non-erectile penis, termed "Zhongqiang," indicating floating Yang syndrome. Treatment involves Psoralea Fruit (*Poguozhi*) and Leek Seed (*Jiucaizi*) each 1 liang, stewed with one pig kidney (daily dose).
Female, 51 years old.
Practiced fasting (*Bigu*), sought cold foods, resulting in diarrhea. Astringents and diuretics were ineffective. Pulse was indeterminate in depth across six positions, rapid and tight in half, tongue color yellowish-brown, tip distinctly crimson, lips dry. For three months, she consumed rice fruits, pears, apples, or cold noodles, sat in meditation nightly without sleep. This indicates diarrhea caused by cold food accumulation and lack of Kidney retention power due to prolonged sitting. The projectile nature of the diarrhea suggests acute pouring (*Baozhu*), unlike typical clear or undigested diarrhea. It is caused by cold accumulation transforming into heat, leading to Kidney Qi rebellion, resulting in Kidney Diarrhea. Prescription:
Burreed Tuber (*Sanleng*), Curcuma Rhizome (*Ezhu*), Coptis Rhizome (*Hulian*), Coptis Rhizome (*Huanglian*) 2 qian, 2 qian, 2 qian, 1.5 qian respectively; Atractylodes Rhizome (*Baizhu*) 1.5 qian, Magnolia Bark (*Chuanpu*) 1.5 qian, Tangerine Peel (*Qingpi*) 1.5 qian, Patchouli Herb (*Guangxiang*) 1.5 qian, Radish Seed (*Sheng Laifuzi*) 1.5 qian, Leek Seed (*Jiu Zi*) a pinch, Chicken Gizzard Lining (*Jineijin*) 2 qian, Malt (*Maiya*) 2 qian.
After two doses, pellet-like or white membrane stools were passed, indicating an exit for accumulated cold and heat. Pulse became deep and weak. Diagnosed as chronic Cold Damage requiring stomach nourishment. Advised switching to hot foods. Prescription:
Smoked Plum (*Wumei*) 3 pieces, Dendrobium Stem (*Jinchai Shihu*) 2 qian, Alpinia Oxyphylla Fruit (*Yizhiren*) 1.5 qian, Nutmeg Seed (*Roukou*) 2 qian, Schisandra Berry (*Wuweizi*) 1.5 qian, White Hyacinth Bean (*Bian Dou*) 2 qian, Red Halloysite Fried with Angelica Root (*Chishizhi Chao Danggui*) 2 qian, Persimmon Calyx (*Liqiao*) 1.5 qian, Hearth Earth (*Zaoxintu*) one piece. Take four doses, then switch to *Jinkui Shenqi Wan* (Golden Cabinet Kidney Qi Pill). Recovered fully. (I previously warned against foot swelling and Kidney Qi rebellion causing abdominal distension).
13. Xiao Ke (Wasting-Thirst)
Male, 8 years old.
Thirsty with polyuria. Guan and Chi pulses on six positions were absent. Finger veins penetrated all three sections, showing mixed green and red, not bright. Tongue brown, center slightly yellow. Spirit fatigued, eyelids drooping. Only large Qi visible. Periodically seeking drinks and urinating. Recognized as a critical condition, intended to decline. The mother, my aunt-by-marriage, cried incessantly, begging for a prescription. Reluctantly wrote: Excessive eating and hunger indicate Grain Thirst (*Guxiao*); irritability, drinking much and urinating much indicate Kidney Thirst (*Shenxiao*). Both are ominous. Pulses absent in two positions, remaining only at Cun, floating without root. Finger veins penetrating three sections, brown tongue, absence of Stomach Qi—how is this not critical? If forced to prescribe, have the mother taste the child's urine. Sweetness indicates exhausted Stomach Qi; lack of sweetness indicates remaining Kidney Qi. Current dirty complexion, if blue appears tomorrow, death is imminent. Wrote this, insisted on leaving. The child died the next day.
Male, 43 years old.
Unemployed, impoverished and ill. Complexion dark, emaciated, voice low, unable to finish sentences, extremely exhausted spirit. Fever, diarrhea, thirst, immediate urination after drinking. Pulse floating and surging, tongue red and dry, lips cracked, irritability. Diagnosed as critical Xiao Ke. Prescription:
Smoked Plum (*Wumei*) 1 liang, Papaya (*Mugua*) 8 qian. Decoct strongly, consume over two days.
After taking, thirst and heat reduced, but diarrhea persisted. Modified prescription:
Clam Shell (*Wenge*), Stir-fried Coix Seed (*Chao Yimi*), Raw Atractylodes Macrocephala Rhizome (*Sheng Baizhu*), Stir-fried Atractylodes Macrocephala Rhizome (*Chao Baizhu*), Raw Barley Sprout (*Sheng Guya*), Stir-fried Barley Sprout (*Chao Guya*) each 2 qian; Smoked Plum (*Wumei*) 10 pieces, Papaya (*Mugua*) 3 qian, Schisandra Berry (*Wuweizi*) 2 qian, Bamboo Leaf (*Zhuye*) 1 qian.
Three doses stopped urination and reduced diarrhea. Patient slept constantly with poor appetite. Changed prescription:
Codonopsis Root (*Xidang*) 4 qian, White Hyacinth Bean (*Bian Dou*) 3 qian, Hangzhou Peony Root (*Hangshao*) 2 qian, Schisandra Berry (*Wuweizi*) 1.5 qian, Dendrobium Stem (*Jinchai Shihu*) 2 qian, Shan Yao (*Guangshan*) stir-fried in salt water 2 qian, Anemarrhena Rhizome (*Zhimu*) 1.5 qian, Raspberry (*Fupenzi*) 2 qian, Dodder Seed (*Tusizi*) 2 qian. Cook one he of Japonica Rice (*Jingmi*) first, remove rice, then decoct herbs.
Four doses resolved previous symptoms. Later developed foot edema, prescribed *Sijunzi Wan* (Four Gentlemen Pill), taken with rice gruel, 40 pills daily. Grain Thirst among the three Xiao types is difficult to treat; must rely on Grain Qi to nourish the Stomach for efficacy.
14. Case of Parasitic Disease
Female, 40 years old.
Had not eaten rice for three years, only wheat noodles or congee. Abdominal pain and vomiting of clear fluid upon smelling food, relieved by eating hot food. Pulse hollow, not bounding. Complexion pale white. Suspected stomach collapse (*Chuzhong*), declined treatment. Husband begged repeatedly. Hesitated long, but noted patient required sugar with meals, and pain/vomiting cleared with hot food—signs of parasites. The vomited clear fluid is parasite-induced. Parasites move with cold (causing pain/vomiting) and settle with heat (relieving symptoms). Fine white spots on tongue also indicate parasites. Must use vermifuge, but considering hollow pulse and suspected stomach collapse, first administered two doses of *Shiquan Dabu Tang* (Ten Substance Great Tonifying Decoction), then vermifuge. Method:
Bark of *Toona sinensis* (female tree only, male is sterile, toxic, and lethal) 8 qian, stripped of coarse outer layer, decocted in water, set aside (do not let patient know location). Fry one eggshell-less egg in vegetable oil until fragrant, place in bowl for patient to smell, do not eat. Once patient smells fragrance and salivates profusely, have patient gulp down the *Toona* bark decoction.
Watch for dizziness due to parasite agitation. Differentiate from stomach collapse. Following this method, patient dizzy for one hour then woke, abdominal pain vanished. Advised wheat congee and lean meat broth (no meat) until Stomach Qi recovered, then normal diet. Full recovery.
(15) Insomnia
Female, 50 years old.
Contracted dual-pathogen Cold Damage. After recovery, suffered continuous insomnia. One physician attributed it to severe Yuan Qi depletion and prescribed large doses of Ginseng (*Renshen*) for ten doses. I was a guest at her home (she was my step-mother-in-law). I asked the physician: "Is this deficiency vexation?" He sneered: "You don't understand, don't talk nonsense." I told my father-in-law I could cure it. Pulse: floating and slow in all six positions, rapid at Guan. Diagnosed as deficiency vexation post-major illness. Prescription:
Whole Gardenia Fruit (*Zhizi*) 8 qian, Fermented Soybean (*Douchi*) 1 liang. Decoct secretly and take.
After four hours, patient desired sleep. Snored loudly in bed, slept seven hours, feeling refreshed and free of distress.
The other physician was still playing cards at my father-in-law's house. Hearing of the cure, he jumped up saying: "I said it was correct, but people criticize me! What nonsense!" He told my father-in-law not to listen to others. My father-in-law showed him my reasoning and case records. He had no reply and fled.
Male, age unspecified.
Fever without chills, headache, insomnia. Voice weak, contrasting sharply with usual loud speech. Appears depressed and stifled, unable to express. Pulse rapid in all six positions, skin not burning hot, tongue edges red, center slightly yellow. No food or drink for six days. Previous physician used *Dayuan Yin* and *Gegen Jieji Tang* without effect. I tentatively prescribed:
Alisma Rhizome (*Zexie*), Donkey-hide Gelatin (*Ejiao*), Polyporus (*Zuling*) each 2 qian, Egg Yolk (*Jizihuang*) one. Decoct and take. This is excessive Yang flourishing, Yin failing to enter Yang, causing insomnia.
After taking, slept for half the night. Asked for tea at dawn, indicating Stomach Qi opening. Modified prescription:
Coptis Rhizome (*Huanglian*) 1 qian, Donkey-hide Gelatin (*Ejiao*) 3 qian, Egg Yolk (*Jizihuang*) one, Night-crossing Vine (*Yejaoteng*) 2 qian, Alisma Rhizome (*Zexie*) 2 qian, Angelica Root (*Danggui*) 2 qian, Hangzhou Peony Root (*Hangshao*) 1.5 qian.
Symptoms gradually resolved, able to sleep. Self-purchased *Zeshen* (likely *Zexie* or similar) soaked in water. Fully recovered.
Male, 45 years old.
Widowed for twelve years, busy with family affairs, insomnia at night, dripping scrotum. Self-medication and medical visits with over 100 doses showed no improvement. Pulse surging and large, tongue red with little moisture, pupils dilated, crotch wet. Facial expression showed coquettishness, flattering towards others. Asked about medications shown: all were damp-draining, drying-dampness, and warming-tonic agents. Synthesizing these, diagnosed as Hyperactive Yang Kidney Heat. External application:
Hearth Earth (*Zaoxintu*) one piece (finely ground), Calcined Dragon Bone (*Duanyulonggu*) powder 2 liang. Mix evenly, put in cloth bag. Place scrotum on bag until water flows into bag and saturates it. Replace until scrotum is dry and clean.
Internal medicine: Nourishing Yin and descending fire agent.
Phellodendron Bark (*Chuanbai*) 2 qian, Anemarrhena Rhizome (*Zhimu*) 2 qian, Ligustrum Fruit (*Nvzhenzi*) 2 qian, Lycium Bark (*Digupi*), Rehmannia Root (*Shengdi*), Forsythia Fruit (*Lianqiao*), Lotus Plumule (*Lianzixin*), Moutan Cortex (*Danpi*) each 2 qian, Coix Seed (*Yiyiren*) 3 qian, Gardenia Fruit (*Zhizi*) 5 pieces.
Two packages reduced scrotal discharge, but night sleep remained restless. Next day changed prescription, explained "Hyperactive Yang." Hyperactive Yang is akin to excessive desire, not damp-heat, but Kidney heat. Due to widowhood, lack of female companionship agitated Liver Fire, causing Liver and Kidney disconnection. Prescribed *Zhibai Dihuang Wan* (Anemarrhena, Phellodendron, and Rehmannia Pill), achieving miraculous effect.
(16) Liver Depression, Emotional Frustration
Female, 46 years old.
Constant fatigue, occasional complaints, irregular eating, insomnia at night, only heard groaning. Husband was a literati, knowledgeable in medicine, so physicians avoided him. One day, husband visited a famous physician. Prescription: *Bazhen Tang* (Eight Treasure Decoction) plus Fluorite (*Zishiying*), Ligustrum Fruit (*Nvzhenzi*), Albizia Flower (*Yehua*). Ten doses followed by 1-2 jin of *Buzhong Yiqi Wan* (Tonify the Middle and Augment Qi Pill). Husband complied. After ten doses and five days of pills, gait became unstable. Husband suspected error, invited me. After hearing history, I analyzed Liver Depression among Seven Emotions. Husband nodded, privately revealed: "This woman is fierce and jealous, often scolds concubines, gets angry until face turns blue/black, and insults me." Asked for advice. I said medicine alone won't work; the balance lies with you. Classics say: "Anger is overcome by joy." Change old habits, stay close to her, give her what she likes. Best to live separately, talk often. Daily decoct 5 qian Licorice Root (*Gancao*) as tea, meaning "Sweet to relax." When smiling appears, besides normal food, add soy sauce and vinegar with jellyfish skin, meaning "Sour to astringe." Avoid Ginseng tonics. If dark circles under brows fade, steam one bird's nest daily. Later, can take *Tianwang Buxin Dan* (Heavenly Emperor Heart-Nourishing Pill). Husband followed advice, condition gradually improved. This is personal speculation, recorded for amusement.
Female, 20 years old.
Returned from theater in Nanchang, suddenly talking and laughing to herself, happy to speak but language confused, laughing only without sorrow, unaware of hunger or rest, wanted to sleep alone. Saw *Fengshi Jinang* on my desk, laughed and compared "Jinang" (brocade bag) to her own purse, expression like an actor. Pulse floating and rapid intermixed, Guan position less than four beats, pulse chaotic. Lips, tongue, diet normal. Diagnosed as emotional frustration causing Spirit not residing in heart. Treatment focused on nourishing Heart and calming Spirit:
Sour Jujube Seed (*Suanzaoren*), Biota Seed (*Baiziren*), Polygala Root (*Yuanzhi*), Curcuma Aromatica Rhizome (*Yujin*), Bamboo Shavings (*Zhuru*), Ligustrum Fruit (*Nvzhenzi*), Dendrobium Stem (*Jinchai Shihu*), Amber Powder (*Hupo Xie*) each 2 qian, Poria (*Fushen*) 3 qian, Immature Bitter Orange (*Zhishi*) 1 qian, Lotus Plumule (*Lianzixin*) 1.5 qian.
Four doses advised. Noted this girl was spoiled, had some culture (private tutor since childhood), emotions easily stirred. Suggested finding a partner promptly; delay risks epilepsy precursor. Five days later, mother and daughter returned. Said she was quieter after meds, gradually knowing shame. But frequently rubbed breasts, touched crotch. Advised larger clothes/pants to reduce friction, but marriage is essential. Diet: more vegetables, early rising, household chores, avoid idleness. Also take *Tianwang Buxin Dan*.
Soon after marrying a man surnamed Tu from neighboring village, condition disappeared.
Female, 19 years old.
Liked romance novels and tea-picking opera. Illness: self-laughing, verbose, walking outside all day, even skipping meals for 2-3 days. Plump, no sick appearance, secret smiling, pulse floating and tight, Guan position bounding too strongly, suspicious. Asked about menstruation, mother said normal. Girl often rubbed inner thighs, touched chest, frowned and sighed, seemed to have hidden concerns. Asked if engaged, mother said discussed several times but how to mention marriage now? I insisted engagement was necessary. Medicine cannot override will. Father understood, agreed. Prescription: Clear Heart and nourish Qi.
Dendrobium Stem (*Jinchai Shihu*), Lotus Plumule (*Lianzixin*), Sour Jujube Seed (*Suanzaoren*), Polygala Root (*Yuanzhi*), Poria (*Fushen*), Angelica Root (*Danggui*), Curcuma Aromatica Rhizome (*Yujin*), Bamboo Shavings (*Zhuru*) each 2 qian, Acorus Rhizome (*Jiejie Changpu*) 1.5 qian, Ginger and Jujube each 2 pieces.
Three doses, still self-laughing, but less time outside, able to return home for meals on time.
One day, Mr. Du asked if she was mad. I said she is smart, it is an emotional disorder, not madness. Du mentioned his second son wanted to engage her, worried about cure and fertility. I suggested they meet. If mutual consent, mood satisfied, temperament normalizes. Fertility unaffected.
Girl took several doses, no significant reaction. Married months later, everything normal, has four children.
Female, 30 years old.
Tidal fever for half month, muttering to self, coquettish eyes, secret smiling, obviously neurotic. Menstruates 3-4 times monthly, never clean. Sometimes sleeps with door closed, sometimes wakes at midnight, mutters, faces away. Pulse floating and rapid, lips/tongue slightly red. Diagnosed menstrual irregularity as *Cuojing* (Erratic Menses). Muttering/facing away indicates emotional instability, likely Qi Depression among Six Depressions, not epilepsy. Method: Soothe Liver and nourish Qi, formula not standard decoction.
Peppermint Herb (*Shengtoucao*), Curcuma Aromatica Rhizome (*Yujin*), Fresh Rehmannia Root (*Sheng Shengdi*) each 2 qian, Fresh Dendrobium Stem (*Xian Shihu*) 3 qian, Acorus Rhizome (*Jiejie Changpu*) 2 qian, Bamboo Shavings (*Zhuru*) 2 qian, Snake Gallbladder Tangerine Peel Powder (*Shedan Chenpi Mo*) one stick (dissolve with medicine), Long Pepper Fruit (*Bichengqie*) 2 qian, Mung Bean Coat (*Lvdouyi*) 2 qian, Gardenia Skin (*Zhizi Pi*) 2 qian, Tangerine Network (*Jieluo*) 2 qian, Lotus Leaf (*Heye*) one small piece. Decoct together. Take 3-4 packages.
Formula uses Rehmannia, Fresh Dendrobium, Mung Bean Coat to clear Stomach heat and nourish Liver Wood (patient likes fried foods). Peppermint, Acorus, Tangerine Network to penetrate collaterals, preventing blood heat from disrupting flow.
Three days later, father came to change prescription. Said she could sit in hall, avoids strangers, swallows phlegm, desires food. Couldn't ask about menstruation. Removed Dendrobium, Rehmannia, Long Pepper from previous formula. Added Lotus Plumule (*Lianzixin*) 1.5 qian, Charred Schizonepeta (*Jingjie Tan*) 2 qian, Angelica Root (*Danggui*) 2 qian, mix with child's urine (*Tongbian*).
Removed Dendrobium/Rehmannia to prevent excessive cooling damaging Qi. Added Angelica/Charred Schizonepeta to dissipate stasis. Four doses, mind gradually normalized, still muttering. Modified prescription:
Acorus Rhizome (*Jiejie Changpu*) 2 qian, Polygala Root (*Yuanzhi*), Sour Jujube Seed (*Suanzaoren*), Biota Seed (*Baiziren*), Bamboo Shavings (*Zhuru*), Curcuma Aromatica Rhizome (*Yujin*), Codonopsis Root (*Xidang*), Poria (*Fushen*) each 2 qian, Immature Bitter Orange (*Zhishi*) 1 qian, Ginger and Jujube each 2 pieces.
This formula warms the Gallbladder. Cold Gallbladder causes Liver Qi unrest. Gallbladder is the Official of Justice; if healthy, all depressions resolve. Take several doses. Advised father to send her to husband. Will recover fully. Did so, gave birth to one boy and one girl.
Female, 22 years old
